Les infections cornéeennes sont d’importantes causes de cécité et se produisent dans des proportions épidémiques dans les pays à revenu faible ou intermédiaire. L’étiologie de la maladie varie d’une région à l’autre, mais les bactéries représentent une grande majorité de ces cas. Pseudomonas aeruginosa est un agent pathogène important qui provoque une maladie rapidement progressive. Dans de nombreux cas, les patients sont laissés avec des cicatrices stromales, astigmatisme irrégulier, nécessitent une greffe ou dans le pire des cas, perdreun œil 1,2.
La kératite bactérienne causée par P. aeruginosa est une infection oculaire difficile à traiter en particulier en raison de l’émergence croissante de souches résistantes aux antimicrobiens de P. aeruginosa. Au cours de la dernière décennie, il est devenu évident que l’essai et le développement de nouveaux traitements pour les infections cornéeennes, en général, et ceux causés par Pseudomonas sp., en particulier, sont essentiels pour lutter contre la tendance actuelle de la résistanceaux antibiotiques 3.
Pour tester l’efficacité de nouveaux traitements pour les infections cornéeennes, les méthodes microbiologiques in vitro conventionnelles sont un substitut pauvre en raison de la différence de physiologie bactérienne pendant la culture de laboratoire et pendant les infections in vivo ainsi que dues au manque de l’interfacehôte 4,5. Les modèles animaux in vivo, cependant, sont coûteux, prennent beaucoup de temps, ne peuvent fournir qu’un petit nombre de répliques et soulever des préoccupations au sujet du bien-être des animaux.
Dans cet article, nous démontrons un modèle organotypique simple et reproductible ex vivo porcine de kératite qui peut être employé pour tester divers traitements pour des infections aiguës et chroniques. Nous avons utilisé P. aeruginosa pour cette expérience, mais le modèle fonctionne également bien avec d’autres bactéries, et des organismes tels que les champignons et la levure qui causent la kératite.

Lors du développement de nouveaux antimicrobiens, le succès des essais sur les animaux dépend de l’extrapolation précise de l’efficacité antimicrobienne des tests in vitro aux infections animales in vivo. Les tests in vitro existants surestiment généralement l’efficacité antimicrobienne, car la présence de tissu hôte comme barrière de diffusion n’est pas prise en compte. Pour surmonter ce goulot d’étranglement, nous avons développé un modèle cornéen ex vivo porcin de kératite bactérienne utilisant Pseudomonas aeruginosa comme organisme prototypique. Cet article décrit la préparation de la cornée porcine et le protocole pour l’établissement de l’infection. Les moules en verre sur mesure permettent la configuration simple de la cornée pour des études d’infection. Le modèle imite l’infection in vivo car la prolifération bactérienne dépend de la capacité de la bactérie à endommager le tissu cornéen. L’établissement de l’infection est vérifié comme une augmentation du nombre d’unités formant des colonies évaluées au moyen de dénombrements viables des plaques. Les résultats démontrent que l’infection peut être établie d’une manière hautement reproductible dans les cornées ex vivo en utilisant la méthode décrite ici. Le modèle peut être étendu à l’avenir pour imiter la kératite causée par des micro-organismes autres que P. aeruginosa. Le but ultime du modèle est d’étudier l’effet de la chimiothérapie antimicrobienne sur la progression de l’infection bactérienne dans un scénario plus représentatif des infections in vivo. Ce faisant, le modèle décrit ici réduira l’utilisation d’animaux pour les tests, améliorera les taux de réussite dans les essais cliniques et, en fin de compte, permettra la traduction rapide de nouveaux antimicrobiens à la clinique.

La conception des moules en verre sont une idée innovante et originale, dont l’utilisation nous a permis de mettre en place le modèle d’une manière cohérente avec un minimum / pas de problèmes avec la contamination.
