他の温熱療法の落とし穴の1つは、正確な細胞ターゲティングの欠如です。温熱療法は本質的に高い治療比を持たないため、慎重な体温測定とターゲティングが必要です6。mNPHは、mNPの全身的または腫瘍内注射を可能にし、mNPが位置する場所でのみ熱が発生するため、腫瘍に直接治療を標的にします。mNPHは、磁性ナノ粒子が細胞の内側または外側にある場合に有効です。癌治療の場合、mNPHの一般的な概要は、磁性ナノ粒子が注入され(腫瘍内または静脈内に)、次に交番磁場が印加され、ナノ粒子磁極が絶えず再整列し、ナノ粒子に関連する細胞および組織の局所的な加熱をもたらすことです7,8.ナノ粒子の体積と交流磁場(AMF)の周波数/強度を調整することにより、組織内で発生する温度を注意深く制御することができます。
この治療法は、より深い腫瘍がより強いAMFを必要とするため、渦電流加熱のリスクが高まるため、体表面近くの腫瘍でうまく機能します9。ハイパーサーミアが単剤療法として臨床的に使用されているという証拠がありますが、多くの場合、ハイパーサーミアは放射線療法または化学療法と組み合わされ、より標的を絞った抗がん効果につながります10,11,12。放射線療法と組み合わせて機能する温熱療法の臨床的証拠は、以前の出版物でレビューされています13。私たちの研究室では、mNPH法12,14,15を用いて、マウスからブタ、イヌの自然がんまで、さまざまな動物の治療に成功しています。このプロトコルは、単独で、または他の治療法と組み合わせて、局所的な温熱療法の効果を調査することに関心のある人のために設計されています。
温熱療法の最も重要な要素の1つは、標的/腫瘍組織に送達される熱線量をリアルタイムで測定および理解できることです。線量を計算および比較する標準的な方法は、43°Cでの加熱の累積等価分数のデモンストレーションによるものです。このアルゴリズムは、送達システム、最高温度および最低温度(特定の範囲内)、および加熱/冷却パラメータ5,16とは無関係の用量の比較を可能にする。CEM計算は、39〜57°C5の温度に最適です。たとえば、私たちが実施したいくつかの研究では、CEM43 30(つまり、43°Cで30分)の熱線量を選択しました。この線量を選択することで、単独で、または単回線量の放射線と組み合わせて、in vitroで安全で効果的な免疫遺伝学的効果を調べることができました17。
