末梢動脈疾患(PAD)は、血液供給の制限を伴う慢性および進行性の血管閉塞を特徴とする1。世界規模では、下肢のPADは高齢者人口の約10%に影響を及ぼし、その最大7%は四肢切断2、3に提出される。
クリティカルリム虚血(CLI)はPAD1の最も深刻なプレゼンテーションを表します。患者は通常、閉塞した動脈に起因する安静時、潰瘍、または壊疽で痛みを経験する。臨床予後は好ましくない一方で、1年3年、4、5の間に四肢切断と死亡の30%のリスクによってマークされています。
血管形成術は、CLI患者の下肢への血流を回復させることができる低侵襲血管内処置である。しかし、血管形成術1,5の後でも、一部の患者は必然的に大きな四肢切断を必要とする。血管形成術後の不利な結果の早期同定は、治療執行の可能性のために非常に貴重である。
従来の危険因子は、血管形成術6を受けるCLI患者における主要な四肢切断に対して限られた予測能力を提供するかもしれない。病態生理性バイオマーカーは、潜在的な臨床応用を有する新規の方法を表し、血管損傷に関連する疾患に特に有用な結果をもたらす可能性がある7。今日では、内皮修復特性を所有する細胞集団の参加は、アテローム硬化性プラークの部位において、8,9にますます認められている。
単核前駆細胞(MPC)は、血管再生能力を有する幹細胞の骨髄および独自の構造的および機能的特性に由来する。増殖するMPCの能力のために、移行し、血管の遵守を示す;これらの細胞は虚血10、11、12に応答して内皮修復を反映する良い候補となっている。また、血管損傷の根底にあるメカニズムへの継続的な関心は、局所的に生じるバイオマーカーの予後的役割を探求する動機となっている。
本研究の目的は、血管形成術を受けているCLI患者の血管閉塞に近い循環するPPCの量を決定する方法を説明することです。内皮機能不全と四肢切断の指標を持つMpC間の関係を評価する方法。
併存症と固有の血管特徴に基づく予後と比較して、局所的なPPCの量は、内皮機能不全および四肢切断に関する臨床結果を予測する特定の能力を示す。一貫して、いくつかの研究は、PAD15、16を有する患者の評価中に同様のバイオマーカーの予後の役割を説明している。
