概要 軽度の外傷性脳損傷(mTBI)は、年間約160万人から380万人のアスリートに影響を与えます1.副脳震盪および脳震盪傷害を含むこれらの傷害は、患者に一時的な身体的、感情的、心理的および認知的症状を残す可能性がある2。さらに、「脆弱性の窓」内で持続する反復的なmTBI(rmTBI)は、単一のmTBI単独の影響よりも長く続く認知的結果の累積的な重症度および持続時間をもたらし得3、そして最終的には機能の永久的な喪失につながることさえある4,5,6。多くの患者は比較的短い時間枠(<1週間)内に回復するが、患者の10〜40%はmTBIのより長い持続効果に1ヶ月>苦しみ、一部は1年まで持続する3、7、8、9。これらの傷害の高い有病率および永続的な結果にもかかわらず、傷害メカニズムはほとんど理解されておらず、効果的な治療戦略は存在しない。

軽度外傷性脳損傷の体重減少モデル 反復軽度のTBIの多くの優れた前臨床モデルが存在するが、12,13,14,15,16,17,18、我々は十分に確立され、臨床的に関連する体重減少閉鎖性頭部損傷モデルを採用している。このモデルの主な特徴には、(1)無傷の頭蓋骨/頭皮の鈍い衝撃に続いて首周りの頭部の無制限の回転、(2)明白な構造的脳損傷、浮腫、血液脳関門損傷、急性細胞死、または慢性脳組織喪失、および(3)複数回のヒット後にのみ現れる持続性(最大1年)認知障害19(図2)が含まれる。
びまん性相関分光法による脳血流の評価 びまん性相関分光法(DCS)は、血流5、20、21を測定する非侵襲的な光学技術です。DCSでは、近赤外光源を組織表面に配置する。検出器を組織表面の光源から一定の距離に配置して、組織を透過して散乱した光を検出します(図3)。移動する赤血球を散乱させると、検出された光強度が時間とともに変動します。相関拡散理論として知られる単純な分析モデルを使用して、これらの強度変動を血流の指標に関連付ける(CBFi、図4)。CBF i(cm2/s)の単位は従来の流れの単位(mL/min/100g)ではないが、マウスでの以前の研究では、CBFiが動脈スピン標識MRI 21によって測定された脳血流と強く相関することが示されている。
参考までに、ここで使用したDCS機器は社内で製造されたもので、長さ852nmのコヒーレンス長レーザ、4光子カウントアバランシェフォトダイオードのアレイ、およびハードウェア自己相関器ボード(シングルタウ、8チャネル、100ns最小サンプル時間)21,22で構成されています。データは、LabViewで書かれた自家製ソフトウェアで取得されます。このデバイスの動物インターフェースは、400 μmのマルチモードソースファイバ(400-2200 nmの波長範囲、純粋なシリカコア、TECSハードクラッディング)と780 nmのシングルモード検出器ファイバ(780-970 nmの波長範囲、純粋なシリカコア、TECSハードクラッディング、730 ± 30 nmのセカンドモードカットオフ)で構成され、6 mm間隔で黒の3Dプリントセンサー(4 mm x 8 mm、 図3)。
