We demonstreren en beschrijven een relatief nieuwe chemische strategie om therapeutische peptidemoieties toe te brengen aan een roestvrijstalen oppervlak met het overkoepelende doel om de reactiviteit van het oppervlak te verminderen met ontstekingscellen in bloed. De hierin beschreven bisfosfonaatchemie omvat coördinerende bindingsvorming tussen de metaaloxiden en bisfosfonaatgroepen van PABT. Percutane coronaire interventie is de eerste lijn van de therapie voor de behandeling van coronaire hartziekten (CAD) en in de eerste plaats gaat stenting de zieke slagaders. Echter, in-stent restenose (ISR) en stent trombose zijn veel voorkomende complicaties in verband met stent inzet1. Bloedinteractie bij de bloedstentinterface wordt gekenmerkt door een bijna onmiddellijke adsorptie van plasma-eiwitten op het metalen oppervlak, gevolgd door bloedplaatjes en ontstekingscelbevestiging en activering2. Het vrijkomen van de inflammatoire cytokines en chemokines uit geactiveerde ontstekingscellen leidt tot de fenotypische modificatie van de vasculaire gladde spiercellen (VSMC's) in de tunica-media en activeert hun centrimentale migratie naar het intimale compartiment. Proliferatie van geactiveerde VSMC in de intima resulteert in intimale laagverdikking, lumenvernauwing en in-stent restenose3. Drug eluting stents (DES) werden ontwikkeld om VSMC proliferatie te voorkomen; deze geneesmiddelen hebben echter een off-target cytotoxisch effect op de endotheelcellen4,5. Daarom is late stenttrombose een veel voorkomende complicatie in verband met DES6,7. Stents gemaakt van biologisch afbreekbare polymeren, zoals poly-L-lactide hebben aangetoond veel belofte in de dierproeven en de eerste klinische proeven, maar werden uiteindelijk herinnerd toen de "real-life" klinisch gebruik aangetoond hun minderwaardigheid aan de3e generatie DES8. Daarom is er een noodzaak om de biocompatibiliteit van bare metal stents te verbeteren voor betere patiëntresultaten.
CD47 is een alomtegenwoordig uitgedrukt transmembrane eiwit dat de aangeboren immuunrespons remt wanneer gebonden aan zijn cognate receptor Signal Regulatory Protein alpha (SIRPα)9. De SIRPα-receptor heeft een itosineremmend motief (ITIM) en de signaleringsgebeurtenissen op SIRPα - CD47-interactie resulteert uiteindelijk in de downregulatie van ontstekingscelactivering10,11,12,13. Onderzoek in ons lab heeft aangetoond dat recombinant CD47 of zijn peptidederivaat, wat overeenkomt met het 22 aminozuur Ig-domein van het extracellulaire gebied van CD47 (pepCD47), de immuunrespons van de gastheer kan verminderen op een reeks klinisch relevante biomaterialen14,15,16. Onlangs hebben we aangetoond dat pepCD47 kan worden geïmmobiliseerd tot roestvrijstalen stentoppervlakken en de pathofysiologische respons die gepaard gaat met restenose aanzienlijk verminderen. Van belang, de pepCD47 gemodificeerde oppervlakken zijn vatbaar voor relevante gebruiksomstandigheden zoals langdurige opslag en ethyleenoxide sterilisatie17. Daartoe kan pepCD47 een nuttig therapeutisch doel zijn om de klinische beperkingen van endovasculaire stents aan te pakken.
De strategie voor de covalente bevestiging van pepCD47 aan een metalen oppervlak omvat een reeks nieuwe chemische modificaties van het metalen oppervlak. De metalen oppervlakken worden eerst bekleed met polyallylamine bisfosfonaat met latente thiolgroepen (PABT), gevolgd door de deprotectie van de thiolen en de bevestiging van polyethyleenimine (PEI) met geïnstalleerde pyridyldithiogroepen (PDT). PDT-groepen PEI die niet zijn ingenomen in de reactie met onbeschermde PABT-thiolen worden vervolgens gereageerd met pepCD47 waarin thiolen in de terminale cysteïneresiduen zijn verwerkt, wat resulteert in binding van pepCD47 aan het metalen oppervlak via een disulfide-binding14,17,18. We gebruikten een fluorofofoër geconjugeerde pepCD47 (TAMRA-pepCD47) om de inputconcentratie van peptide te bepalen die resulteert in de maximale oppervlakteimmobilisatie van het peptide. Ten slotte evalueerden we de acute en chronische ontstekingsremmende capaciteit van de met pepCD47 gecoate metalen oppervlakken, ex vivo, met behulp van respectievelijk het Chandler-lusapparaat en monocytenbevestiging/macrofaagexpansietest.
Dit document biedt een systematisch protocol voor de bevestiging van thiolated peptiden aan het metalen oppervlak; het bepalen van de maximale immobilisatiedichtheid van het peptide; en het beoordelen van de ontstekingsremmende eigenschappen van gecoat metalen pepcD47-oppervlakken die zijn blootgesteld aan volbloed en geïsoleerde monocyten.

De belangrijkste complicaties geassocieerd met bare metal stents en drug eluting stents zijn in-stent restenose en late stent trombose, respectievelijk. Het verbeteren van de biocompatibiliteit van metaalstenten blijft dus een belangrijke uitdaging. Het doel van dit protocol is om een robuuste techniek van metalen oppervlaktemodificatie te beschrijven door biologisch actieve peptiden om de biocompatibiliteit te vergroten van bloedcontacten met medische implantaten, waaronder endovasculaire stents. CD47 is een immunologische soortspecifieke marker van het zelf en heeft ontstekingsremmende eigenschappen. Studies hebben aangetoond dat een 22 aminozuur peptide die overeenkomt met het Ig-domein van CD47 in het extracellulaire gebied (pepCD47), ontstekingsremmende eigenschappen heeft, zoals het volledige eiwit. In vivo studies bij ratten, en ex vivo studies in konijn en menselijk bloed experimentele systemen van ons lab hebben aangetoond dat pepCD47 immobilisatie op metalen verbetert hun biocompatibiliteit door het voorkomen van inflammatoire cel gehechtheid en activering. Dit artikel beschrijft het stapsgewijze protocol voor de functionalisering van metalen oppervlakken en peptidebevestiging. De metalen oppervlakken worden gewijzigd met behulp van polyallylamine bisfosfaat met latente thiolgroepen (PABT), gevolgd door deprotectie van thiolen en versterking van thiol-reactieve locaties via reactie met polyethyleenimine geïnstalleerd met pyridyldithio groepen (PEI-PDT). Ten slotte worden pepCD47, waarin terminale cysteïneresten zijn opgenomen die via een dubbele 8-amino-3,6-dioxa-octanoyl-spacer op het metalen oppervlak zijn aangesloten, aan het metalen oppervlak bevestigd. Deze methode van peptide bevestiging aan metalen oppervlak is efficiënt en relatief goedkoop en kan dus worden toegepast om de biocompatibiliteit van verschillende metalen biomaterialen te verbeteren.

De metalen oppervlakken worden thiol-reactief gemaakt voor peptidebevestiging via een reeks chemische modificaties, zoals geïllustreerd in figuur 1. PABT incubatie gevolgd door PEI-PDT behandeling maakt het metalen oppervlak vatbaar voor peptide bevestiging. Dit protocol bevat een nieuwe chemische strategie om therapeutische peptiden toe te brengen aan metalen oppervlakken, en heeft het potentieel om de biocompatibiliteit van een breed scala van metalen biomaterialen te verbeteren. Dit is een universele en robuuste techniek om peptiden en therapeutische eiwitten toe te brengen aan de kale metalen oppervlakken van bioimplanten. We hebben deze technologie gebruikt om de biocompatibiliteit van metalen stents te verbeteren door cd47-peptiden toe te dienen om de ontstekingsreacties te voorkomen die worden geactiveerd door stenting.Begin met het wassen van de roestvrijstalen folie coupons of mesh schijven met 2-isopropanol in een shaker gedurende vijf minuten. Voer deze was twee keer uit en was de monsters twee keer met chloroform gedurende 10 minuten per wasbeurt. Plaats de gereinigde roestvrijstalen monsters gedurende 30 minuten in een oven op 220 graden Celsius.Dompel de gebakken folies of mesh schijven onder in 0,5% waterige oplossing van PABT en broed ze een uur in een shaker. Was de PABT-gemodificeerde monsters vijf keer met gedeïsmiseerd gedestilleerd water. Breng de exemplaren over naar een nieuw flesje en herhaal de wasbeurten.Behandel de monsters met TCEP gedurende 15 minuten bij kamertemperatuur op een shaker. Ontgast onteerd gedestilleerd water in een ronde bodemkolf met een vacuümgentilator, zoals een lyofielieverelaar, en was de folies of gaasschijven vijf keer met het ontgaste water. Breng de monsters over naar een nieuw flesje en herhaal de wasbeurten.Bereid vijf milliliter 1%PEI-PDT-oplossing voor door 212,5 microliter van het bestand PEI-PDT en 125 microliter van 0,4 molaire natriumacetaat in het ontgassen water te verdunnen. Incubeer de gewassen roestvrijstalen exemplaren met 1%PEI-PDT. Vervang lucht door argongas, sluit de flesjes luchtdicht af en meng ze een uur lang op een shaker.Ga onmiddellijk verder met de peptide vervoeging of bewaar de monsters op vier graden Celsius voor maximaal een week. Bereid een milligram per milliliter menselijke peptide CD47 voorraad oplossing door het oplossen van menselijke peptide CD47 poeder in ontgast 50%azijnzuur, dan bereiden de werkoplossing door het oplossen van 500 microliter van de voorraad in 4,5 milliliter van ontgast PBS. Incubeer de gewassen PEI-PDT gecoate monsters met de werkoplossing van peptide CD47 bij kamertemperatuur met schudden gedurende een uur.Als u klaar bent, was de peptide CD47-gecoate monsters zoals beschreven in de tekst manuscript. Na het coaten van de metalen folies met peptide CD47 of roerei peptide, snijd kwart inch PVC-buizen in drie 38 centimeter lange stukken en voeg tot acht ongewijzigde, roerei peptide, of peptide CD47-gemodificeerde metalen folies in drie verschillende buizen. Verzamel 30 milliliter bloed van gezonde menselijke donoren vrij van anti-bloedplaatjes medicijnen, pre-loading de spuit met een milliliter van 4% natriumcitraat om stolling van het verzamelde bloed te voorkomen.Doe 10 milliliter bloed in elke buis met behulp van een spuit van 10 milliliter en sluit de uiteinden aan met metalen adapters. Plaats de met bloed gevulde buizen op de wielen van het Chandler-lusapparaat en geef het bloed vier uur lang door de metalen folies door wielrotatie op 37 graden Celsius. Giet het bloed uit de buizen en gooi het volgens IRB eisen, snijd dan de buizen met een scalpel om de folies te halen.Bereid 2%glutaraldehydeoplossing door de 70%-oplossing in natriumcacanoodylaatbuffer te verdunnen met 0,1 molaire natriumchloride. Incubeer de folies in de 2%glutaraldehydeoplossing gedurende 15 minuten en bewaar ze 's nachts op vier graden Celsius. Voor het analyseren van de metalen folies, was ze drie keer met PBS.Warme acht milliliter PBS in een 37 graden Celsius waterbad. Bereid 10 millimolar CFDA kleurstof voorraad oplossing door het toevoegen van 90 microliter DMSO aan een CFDA flacon, dan bereiden de werkoplossing door het toevoegen van 75 microliters van de voorraad CFDA aan de warme PBS. Keer de buis een paar keer om te mengen en te bedekken met aluminiumfolie.Doe elke folie in een milliliter CFDA kleurstof in een 24-well plaat. Bedek de plaat met aluminiumfolie en broed de plaat 15 minuten lang op 37 graden Celsius af, was de folies vervolgens drie keer in PBS en beeld ze met een fluorescentiemicroscoop. De concentratie van covalently-gebonden TAMRA geconjugeerde peptide CD47 werd gekwantificeerd om maximale immobilisatiedichtheid van het peptide op het metaaloppervlak te bepalen.De maximale peptideretentie was ongeveer 180 nanogram per vierkante centimeter, wat werd bereikt met een invoerconcentratie van 100 microgram per milliliter. Immobilisatie van TAMRA geconjugeerde peptide CD47 op de gemodificeerde metalen oppervlakken werd verder bevestigd door fluorescentie microscopie, waaruit bleek een uniforme fluorescentie uitgezonden van het oppervlak van de TAMRA geconjugeerde peptide CD47-behandelde mesh schijven. Slechts minimale fluorescentie werd gedetecteerd op het oppervlak van de controle mazen die de wijziging ontbrak, waardoor een niet-specifiek gebonden TAMRA geconjugeerde peptide CD47 als de belangrijkste bron van fluorescentie.Het werd ook bevestigd dat de peptide CD47-gecoate oppervlakken tonen een drastische vermindering van bloed overgedragen cel gehechtheid in vergelijking met de roerei gemodificeerde en ongewijzigde peptide controles. Monocyten geïsoleerd van rat buffy vachtcellen werden gekweekt op bare metal en peptide CD47-gemodificeerde roestvrijstalen folies. Een 58%-demping van acute monocytenbevestiging, hun omzetting in macrofagen en macrofaagproliferatie werd waargenomen op de gefunctionaliseerde oppervlakken.Tijdens een poging tot dit protocol is het erg belangrijk om de stappen uit te voeren waarbij ontgasseerd water of reagentia zo snel mogelijk moeten worden uitgevoerd en om de blootstelling aan zuurstof te minimaliseren. Onze technologie moduleert selectief de cellulaire interactie met metalen oppervlakken en een typische celtype identificatietest, zoals RT-PCR, flow cytometrie en immunofluorescentie, zijn compatibel met deze techniek. Wanneer ontwikkeld voor een reeks bioactieve peptide, kan deze technologie instrumenteel zijn in het verhogen van de biocompatibiliteit op de weefselmateriaalinterface.
