위식도 역류 질환 (GERD)은 서구 국가1에서 사람들의 20 %까지 영향을 미치는 가장 흔한 양성 질환 중 하나입니다. 가슴 앓이 또는 역류와 같은 전형적인 증상 외에도 일부 환자는 기침, 쉰 목소리 또는 천식 2,3과 같은 비정형 증상으로 고통받을 수 있습니다. 만성 기침, 만성 후두염 및 천식이 역류 관련 기원을 가질 수 있고 후두인두 역류(LPR)와 유의하게 연관된다는 합의에도 불구하고, 정확한 병리기전은 여전히 불분명하다. 이러한 증상은 일반적으로 다인자 과정의 일부이기 때문에 훌륭한 진단 및 치료 과제를 묘사합니다4.
하부 식도 괄약근보다 5cm 높은 원위 식도 pH 모니터링은 일반적으로 GERD2가 의심되는 환자에서 비정상적인 식도 산 노출을 결정하는 데 사용됩니다. 동일한 기술을 사용하려는 시도에서, 근위 종래의 pH 모니터링은 프로브가 UES에 또는 약간 위에 배치됨에 따라 상부 식도 괄약근(UES)에서의 비정상적인 산 노출을 측정하기 위한 진단 장치로서 1990년대 후반에 도입되었다. 그러나, 이 방법은 프로브가 구인두 환경에 대해 설계되지 않았기 때문에 프로브(5,6)의 건조에 의한 유사 역류 사건과 같은 유효하지 않은 아티팩트의 측정을 유도하기 때문에 항상 유효하고 정확한 결과를 제공하는 것은 아니다.
최근 후두인두 pH 모니터링은 구인두에서의 산 노출을 측정하도록 특별히 설계된 새로운 진단 장치로 도입되었으며, 프로브는 UES 위에 포도막에 약간 옆으로 배치됩니다 (그림 1). 이전의 연구는 주로 비정형 증상을 가진 환자에서 역류 수술 후 성공적인 결과에 대해 80 %의 긍정적 인 예측 값을 갖는 것으로 밝혀 졌기 때문에이 새로운 도구는 선택된 환자의 진단 경로에 귀중한 추가가되었습니다. 눈물 방울 센서에는 액체 및 에어로졸화 된 산을 감지하는 안티몬 기술이 장착되어 있으며 유효한 결과를 측정하기 위해 점막 접촉이 직접 필요하지 않습니다. 또한, 센서는 근위 pH 모니터링과는 달리, 건조에 저항할 수 있어 보다 신뢰할 수 있는 결과(7,8)로 이어질 수 있다.
종래의 식도와 후두인두 pH 측정의 상관관계에 관한 현재 문헌은 드물다. 이전의 연구에는 소수의 환자 만 포함되었거나 동시에 9,10,11 건의 측정을 수행하지 않았습니다. 우리는 최근에 GERD가 의심되는 101 명의 환자로 구성된 대규모 코호트에서 두 pH 측정 간의 상관 관계에 대한 데이터를 발표했습니다. 우리는 후두인두 및 식도 pH 측정이 다양한 역류 시나리오12의 존재로 인해 반드시 대응할 필요는 없다는 결론을 내렸다. 우리는 또한 식도 절제술 및 재건 후 환자와 함께 인간 역류 모델을 개발하여 부피 환류기13에서 두 pH 모니터링 방법 간의 100 % 상관 관계를 보여주는 위 교재를 사용했습니다.
