IVM is een ART waarbij menselijke onrijpe eicellen in vitro worden gekweekt tot rijping voor IVF-ET of FP. Bij IVM worden ovulatie-inductiemedicijnen niet gebruikt, waardoor pijn, financiële lasten en complicaties zoals ovarieel hyperstimulatiesyndroom (OHSS)1,2wordenverminderd. Daarnaast is IVM bijzonder geschikt voor de FP van kankerpatiënten en de onvruchtbaarheidsbehandeling van hormoongevoelige patiënten die mogelijk niet in staat zijn of geen tijd hebben om ovulatie-inductietherapie te ontvangen3. Daarom, hoewel het aantal opgehaalde eicellen, het klinische zwangerschapspercentage (CPR) en het levend geboortecijfer (LBR) lager zijn dan die van IVF4,5,heeft IVM zijn eigen unieke voordelen.
Onvruchtbare patiënten met endometriumlaesies, hydrosalpinx of ovariumcysten hebben meestal gynaecologische chirurgie vóór art-behandeling en hun eicellen zijn meestal onvolgroeid. OP-IVM maakt gebruik van geleide transvaginale echografie om de onrijpe eicellen op te halen en ze in vitro te laten groeien tot rijping voor IVF-ET of FP. OP-IVM combineert IVM na het ophalen van eicellen en gynaecologische chirurgie, waardoor complicaties die vaak voorkomen bij gecontroleerde ovariële hyperstimulatiecycli worden verminderd en tijd en geld wordt bespaard. Voor vruchtbare patiënten kan OP-IVM dienen als een "vruchtbaarheidsverzekering" tijdens het ondergaan van routinematige gynaecologische chirurgie.
Bovendien kan schade veroorzaakt door gynaecologische operaties, zoals elektrocauterie6,7 en resectie van eierstoktumoren, worden verminderd door het ophalen van eicellen vóór gynaecologische chirurgie. Daarom kan OP-IVM, in vergelijking met routinematige gynaecologische chirurgie, het aantal operaties tijdens de onvruchtbaarheidsbehandeling verminderen en het verlies van functionele eicellen tijdens ovariële chirurgie voorkomen.
Een eerdere studie heeft aangetoond dat de aanvullende procedure van het ophalen van eicellen de chirurgische complicaties en ongunstige zwangerschapsuitkomsten niet zou verhogen, noch het ziekenhuisverblijf zou verlengen8. op-ivm: combining in vitro maturation after oocyte retrieval with gynecological surgery

Het gebruik van in vitro rijping (IVM) vóór gynaecologische chirurgie (OP-IVM) is een uitbreiding van conventionele IVM die IVM na het ophalen van eicellen combineert met routinematige gynaecologische chirurgie. OP-IVM is geschikt voor patiënten die een goedaardige gynaecologische operatie ondergaan en die behoefte hebben aan vruchtbaarheidsbehoud (FP) of onvruchtbaarheidsbehandelingen zoals in-vitrofertilisatie en embryotransfer (IVF-ET). In de operatiekamer worden patiënten die een goedaardige gynaecologische operatie ondergaan eerst verdoofd en krijgen ze een echogeleide onvolgroeide follikelaspiratie (IMFA) behandeling. Terwijl de daaropvolgende gynaecologische chirurgie wordt uitgevoerd, worden de cumulus-oöcytencomplexen (COC's) onderzocht en worden de onrijpe COC's overgebracht naar het IVM-medium en gedurende 28-32 uur in het IVF-laboratorium gekweekt. Na beoordeling zullen volwassen eicellen in het MII-stadium worden geselecteerd en gecryopreserveerd in vloeibare stikstof voor FP of bevrucht door intracytoplasmatische sperma-injectie (ICSI) voor IVF-ET. Door IVM te combineren met gynaecologische chirurgie, kunnen onrijpe eicellen die zouden zijn weggegooid, worden opgeslagen en gebruikt voor geassisteerde voortplantingstechnologie (ART). De procedure, betekenis en kritische aspecten van OP-IVM worden in dit artikel beschreven.

Tot december 2019 werd OP-IVM gebruikt voor vruchtbaarheidsbehoud van 274 patiënten. Embryologische en reproductieve uitkomsten van 158 patiënten tussen 2014 en 2016 werden gepubliceerd in een eerder artikel8. De technologie, OP-IVM, is een uitbreiding van het conventionele IVM-protocol dat IVM na het ophalen van eicellen combineert met routinematige gynaecologische chirurgie. Het is geschikt voor onvruchtbare of subfertiele vrouwen die gynaecologische chirurgie moeten ondergaan vóór ART-behandeling of vruchtbaarheidsbehoud, patiënten met polycysteus ovariumsyndroom met clomifeenresistentie die een laparoscopische ovariële booroperatie nodig hebben, onvruchtbare patiënten die goedaardige gynaecologische operaties nodig hebben, zoals hysteroscopische myomectomie, polypectomie, transthoracale resectie van septum en de laparoscopische buischirurgie, en de oophorocystectomie vóór ART-behandeling, en ook patiënten met kanker en hematologische ziekte die chemoradiotherapie of radiotherapie krijgen, kunnen ze deze operatie doen voor vruchtbaarheidsbehoud. Plaats de ultrasone sonde in de vagina om het aantal follikels in beide eierstokken te scannen en te registreren.Zoek de locatie die het dichtst bij de eierstok ligt als de punctieplaats, vermijd de darm, blaas en grote bloedvaten. Was de 19 gauge enkele lumen aspiratienaald met een pH-stabiel hanteringsmedium en injecteer de naald vervolgens in de eierstokken onder begeleiding van echografie. Aspirateer folliculaire vloeistof met de naald onder een druk van 80 tot 90 millimeter kwik, waarbij de naald iets wordt gedraaid om zoveel mogelijk folliculaire vloeistof op te zuigen.Gebruik heparine om de viscositeit van folliculaire vloeistof tijdens de aspiratie te verminderen. Pas de positie van de sonde aan om deze te allen tijde zo dicht mogelijk bij de eierstokken te houden en druk met de juiste kracht op de vaginale fornix om letsel en bloedingen te verminderen. Zorg ervoor dat u de aspiratie van folliculaire vloeistof binnen 25 tot 30 minuten voltooit.Trek de naald eruit na het afwerken in één eierstok, was de naald met het behandelingsmedium en prik de andere kant met dezelfde methode. Werk in een homothermische flat van 37 graden Celsius en bereid de handlingmedia, petrischaal en celzeef voor. Spoel de kweekbuis en zeef af met voorverwarmde pH-stabiele hanteringsmedia.Filter de aangezogen follikelvloeistof met een nyloncelzeef van 70 micrometer, spoel de kweekbuis en zeef herhaaldelijk met voorverwarmd pH-stabiel hanteringsmedium. Zorg ervoor dat alle onvolgroeide COC's volledig worden overgebracht naar de kweekschaal. Breng het bloed over in schone behandelingsmedia.Breng vervolgens de celzeef over naar een nieuwe schaal en was deze. Aspirateer en breng de behandelingsmedia met de COC's, granulosacellen en weefsels over naar een nieuwe schaal. Onderzoek de COC's onder de stereoscoop met 40 keer vergroting.Breng de onvolwassenen snel over in een voorverwarmd IVM-eicelmedium. Onderzoek vervolgens de COC's in het bloedstolsel en in de celzeef. Breng de COC's en de behandelingsmedia over in voorverwarmd IVM-eicelmedium.Kweek de onrijpe COC's gedurende 28 tot 32 uur bij 37 graden Celsius in bevochtigde lucht met 5% koolstofdioxide en 5% zuurstof. In december 2019 was OP-IVM gebruikt voor vruchtbaarheidsbehoud van 274 patiënten. Een voorbeeld was een 28-jarige patiënt die werd gediagnosticeerd met primaire onvruchtbaarheid, linker adnexale cyste en PCOS.Ze kreeg laparoscopische cystectomie en OP-IVM in september 2016 en 27 onrijpe COC's werden verkregen. Na 28 uur in cultuur werden zeven eicellen in het MII-stadium geselecteerd en bevrucht door ICSI. Na vijf dagen werden twee blastocysten ingevroren.Eentje werd in februari 2017 ontdooid en overgebracht naar de baarmoeder. De patiënte beviel in november van een gezond meisje. OP-IVM werd voor het eerst toegepast op het ophalen van eicellen van onvruchtbare vrouwen met PCOS die een ovariële booroperatie ondergaan, en nu kunnen de toepassingen ervan worden uitgebreid naar de subfertile vrouwen die een goedaardige gynaecologische operatie moeten ondergaan, zelfs kankervrouwen voor vruchtbaarheidsbehoud.Het geeft aan dat het toepassingsgebied van IVM kan worden uitgebreid door het te combineren met andere traditionele gynaecologische operaties voor het behoud van de vrouwelijke vruchtbaarheid.
