La MIV est un TAR dans lequel des ovocytes immatures humains sont cultivés in vitro jusqu’à maturation pour la FIV-ET ou FP. Dans la MIV, les médicaments d’induction de l’ovulation ne sont pas utilisés, réduisant ainsi la douleur, le fardeau financier et les complications telles que le syndrome d’hyperstimulation ovarienne (SHO)1,2. En outre, la MIV est particulièrement adaptée à la PF des patientes atteintes de cancer et au traitement de l’infertilité des patientes hormono-sensibles qui peuvent être incapables ou n’avoir pas le temps de recevoir un traitement d’induction de l’ovulation3. Par conséquent, bien que le nombre d’ovocytes prélevés, le taux de grossesse clinique (RCR) et le taux de naissances vivantes (LBR) soient inférieurs à ceux de la FIV4,5,la MIV a ses propres avantages uniques.
Les patientes infertiles présentant des lésions de l’endomètre, de l’hydrosalpinx ou des kystes ovariens ont généralement subi une chirurgie gynécologique avant le traitement par TAR, et leurs ovocytes sont généralement immatures. OP-IVM utilise l’échographie transvaginale guidée pour récupérer les ovocytes immatures et les cultiver in vitro jusqu’à maturation pour la FIV-ET ou FP. OP-IVM combine IVM après prélèvement d’ovocytes et chirurgie gynécologique, réduisant ainsi les complications courantes dans les cycles d’hyperstimulation ovarienne contrôlée et économisant du temps et de l’argent. Pour les patientes fertiles, OP-IVM pourrait servir d'«assurance fertilité » tout en subissant une chirurgie gynécologique de routine.
En outre, les dommages causés par les chirurgies gynécologiques, telles que l’électrocautérisation6,7 et la résection de la tumeur ovarienne, pourraient être réduits par le prélèvement d’ovocytes avant la chirurgie gynécologique. Par conséquent, par rapport à la chirurgie gynécologique de routine, OP-IVM pourrait réduire le nombre d’opérations pendant le traitement de l’infertilité et prévenir la perte d’ovocytes fonctionnels pendant la chirurgie ovarienne.
Une étude antérieure a montré que la procédure supplémentaire de prélèvement d’ovocytes n’augmenterait pas les complications chirurgicales et les issues défavorables de la grossesse, ni ne prolongerait le séjour à l’hôpital8. Certaines patientes ont donné naissance vivante par OP-IVM8,ce qui indique la faisabilité de cette méthode. Cet article décrit les caractéristiques des patients qui peuvent bénéficier de l’OP-IVM ainsi que les procédures et les points critiques de l’OP-IVM et discute de l’évaluation de la maturité des ovocytes humains.

op-ivm: combining in vitro maturation after oocyte retrieval with gynecological surgery

L’utilisation de la maturation in vitro (MIV) avant opération gynécologique (OP-IVM) est une extension de la MIV conventionnelle qui combine la MIV après prélèvement d’ovocytes avec la chirurgie gynécologique de routine. OP-IVM convient aux patientes subissant une chirurgie gynécologique bénigne qui ont besoin de la préservation de la fertilité (PF) ou de traitements de l’infertilité tels que la fécondation in vitro et le transfert d’embryons (FIV-ET). Dans la salle d’opération, les patientes subissant une chirurgie gynécologique bénigne sont d’abord anesthésiées et reçoivent un traitement par aspiration foliculaire immature guidée par échographie (IMFA). Au fur et à mesure que la chirurgie gynécologique ultérieure est effectuée, les complexes cumulus-ovocytes (COC) sont examinés et les COC immatures sont transférés dans le milieu IVM et cultivés pendant 28 à 32 heures dans le laboratoire de FIV. Après évaluation, les ovocytes matures au stade MII seront sélectionnés et cryoconservés dans de l’azote liquide pour la FP ou fécondés par injection intracytoplasmique de spermatozoïdes (ICSI) pour la FIV-ET. La technologie, OP-IVM, est une extension du protocole IVM conventionnel qui combine la MIV après le prélèvement d’ovocytes avec la chirurgie gynécologique de routine. Il convient aux femmes infertiles ou hypofertiles qui ont besoin d’une chirurgie gynécologique avant le traitement antirétroviral ou la préservation de la fertilité, aux patients atteints du syndrome des ovaires polykystiques présentant une résistance au clomifène qui ont besoin d’une chirurgie de forage ovarien laparoscopique, aux patients infertiles qui ont besoin de chirurgies gynécologiques bénignes telles que la myomectomie hystéroscopique, la polypectomie, la résection transthoracique du septum et la chirurgie du tube laparoscopique, et l’ovariectomie avant le traitement par TAR, ainsi que les patients atteints de cancer et de maladie hématologique qui reçoivent une chimiothérapie ou une radiothérapie, ils peuvent faire cette chirurgie pour préserver la fertilité. Placez la sonde à ultrasons à l’intérieur du vagin pour scanner et enregistrer le nombre de follicules dans les deux ovaires.Trouvez l’emplacement le plus proche de l’ovaire comme site de ponction, en évitant l’intestin, la vessie et les gros vaisseaux sanguins. Lavez l’aiguille d’aspiration à lumière unique de calibre 19 avec un milieu de manipulation au pH stable, puis injectez l’aiguille dans les ovaires sous la direction d’une échographie. Aspirer le liquide folliculaire avec l’aiguille sous une pression de 80 à 90 millimètres de mercure, en faisant pivoter légèrement l’aiguille pour aspirer autant de liquide folliculaire que possible.Utilisez l’héparine pour réduire la viscosité du liquide folliculaire pendant l’aspiration. Ajustez la position de la sonde pour la garder aussi près que possible des ovaires en tout temps et appuyez sur le fornix vaginal avec la force appropriée pour réduire les blessures et les saignements. Assurez-vous de terminer l’aspiration du liquide folliculaire dans les 25 à 30 minutes.Retirez l’aiguille après avoir fini dans un ovaire, lavez l’aiguille avec le milieu de manipulation et perforez l’autre côté en utilisant la même méthode. En travaillant dans un appartement homothermique de 37 degrés Celsius, préparez le milieu de manipulation, la boîte de Petri et la passoire cellulaire. Rincez le tube de culture et la passoire avec un milieu de manipulation préchauffé au pH stable.Filtrer le liquide follicule aspiré avec une passoire à cellules en nylon de 70 micromètres, en rinçant à plusieurs reprises le tube de culture et la passoire avec un milieu de manipulation au pH stable préchauffé. Assurez-vous que tous les COC immatures sont complètement transférés dans le plat de culture. Transférer le sang dans un milieu de manipulation propre.Ensuite, transférez la passoire cellulaire dans un nouveau plat et lavez-la. Aspirer et transférer le milieu de manipulation avec les COC, les cellules de granulosa et les tissus dans un nouveau plat. Examinez les COC sous le stéréoscope avec un grossissement de 40 fois.Transférer rapidement les immatures dans un milieu ovocytaire MIV préchauffé. Ensuite, examinez les COC dans le caillot sanguin et dans la passoire cellulaire. Transférer les COC et les milieux de manipulation dans un milieu ovocytaire MIV préchauffé.Culturez les COC immatures pendant 28 à 32 heures à 37 degrés Celsius dans un air humidifié contenant 5% de dioxyde de carbone et 5% d’oxygène. En décembre 2019, OP-IVM avait été utilisé pour la préservation de la fertilité de 274 patients. Un exemple était un patient de 28 ans qui a reçu un diagnostic d’infertilité primaire, de kyste annexique gauche et de SOPK.Elle a reçu une cystectomie laparoscopique et une OP-IVM en septembre 2016 et 27 COC immatures ont été obtenus. Après 28 heures en culture, sept ovocytes au stade MII ont été sélectionnés et fécondés par ICSI. Après cinq jours, deux blastocystes ont été congelés.L’un d’eux a été décongelé en février 2017 et transféré dans l’utérus. La patiente a accouché d’une fille en bonne santé en novembre. OP-IVM a d’abord été appliqué à la récupération d’ovocytes de femmes infertiles atteintes de SOPK qui subissent une opération de forage ovarien, et maintenant ses applications peuvent être étendues aux femmes hypofertiles qui ont besoin de recevoir une opération gynécologique bénigne, même les femmes cancéreuses pour la préservation de la fertilité.Il indique que la gamme d’applications de la MIV peut être élargie en la combinant avec d’autres opérations gynécologiques traditionnelles pour la préservation de la fertilité féminine.
