op-ivm: combining in vitro maturation after oocyte retrieval with gynecological surgery

婦人科手術(OP-IVM)前のインビトロ成熟(IVM)の使用は、定期的な婦人科手術と卵母細胞の検索に続くIVMを組み合わせた従来のIVMの延長である。OP-IVMは、不妊治療(FP)または体外受精や胚移植(IVF-ET)などの不妊治療の必要性を有する良性婦人科手術を受けている患者に適しています。手術室では、良性婦人科手術を受けている患者は、最初に麻酔を受け、超音波誘導未熟卵性卵胞吸引(IMFA)治療を受ける。その後の婦人科手術が行われると、cumulus-oocyte複合体(COC)を検査し、未熟なCOCをIVM培地に移し、IVF実験室で28〜32時間培養する。評価後、MII段階の成熟卵母細胞を選択し、FP用の液体窒素中で凍結保存するか、IVF-ETの細胞質内精子注入(ICSI)によって受精する。IVMと婦人科手術を組み合わせることで、廃棄されたであろう未熟な卵母細胞を保存し、生殖補助技術(ART)に使用することができます。OP-IVM の手順、重要性、および重要な側面については、この記事で説明します。

この技術OP-IVMは、卵母細胞の検索に続くIVMと日常的な婦人科手術を組み合わせた従来のIVMプロトコルの拡張です。それは、ART治療または不妊治療の前に婦人科手術を受ける必要がある不妊または不妊亜女性、腹腔鏡下卵巣穿孔手術を必要とするクロミフェン耐性を有する多嚢胞性卵巣症候群患者、子宮鏡下筋切除術、ポリペクトミー、経胸部開胸術などの良性婦人科手術を必要とする不妊患者および中胸部外科手術、および中胸部手術、および ART治療前の大腸嚢胞切除術、および化学放射線療法または放射線療法を受けている癌および血液疾患の患者は、不妊治療の保存のためにこの手術を行うことができます。超音波プローブを膣内に置いて、両方の卵巣の卵胞の数をスキャンして記録します。腸、膀胱、および大きな血管を避けて、穿刺部位として卵巣に最も近い場所を見つける。pH安定した取り扱い媒体で19ゲージの単一の内腔の吸引針を洗い、次に超音波の指導の下で卵巣に針を注入する。80〜90ミリメートルの水銀の圧力で針を持つ濾胞液を吸引し、針をわずかに回転させ、できるだけ多くの濾胞液を吸引する。ヘパリンを使用して、吸引中の濾胞液粘度を低下させます。プローブの位置を調整して、常に卵巣にできるだけ近づけ、膣の深膜を適切な力で押して、傷害と出血を軽減します。25~30分以内に濾胞性流体吸引を完了してください。一方の卵巣で仕上げ終えた針を引き出し、ハンドリング媒体で針を洗い、もう一方の側を同じ方法で穿刺する。37°Cのホモサーマルフラットで作業し、ハンドリングメディア、ペトリ皿、およびセルストレーナーを準備します。培養管とストレーナーを、あらかじめ温めたpH安定したハンドリング媒体でリンスします。70マイクロメートルのナイロンセルストレーナーで吸引した卵胞液をフィルターし、培養管とストレーナーを事前に温めたpH安定な取扱い媒体で繰り返しすすます。すべての未熟なCOCが完全に培養皿に移されることを確認します。きれいな取り扱い媒体に血液を移す。その後、セルストレーナーを新しい皿に移し、洗います。吸引し、新しい皿にCOC、顆粒球細胞およびティッシュと取り扱い媒体を移す。40倍の倍率でステレオスコープの下のCOCを調べます。未熟者を温めるIVM卵母細胞培地に素早く移します。次に、血栓と細胞ストレーナーのCOCを調べる。COCと取り扱いメディアを、温め込んだIVM卵子培地に転送します。5%の二酸化炭素と5%の酸素を含む加湿空気で摂氏37度で28〜32時間未熟なCOCを培養する。2019年12月までに、OP-IVMは274人の患者の不妊治療に使用されました。一例は、原発性不妊症、左腺嚢胞、およびPCOSと診断された28歳の患者であった。彼女は2016年9月に腹腔鏡下膀胱切除術とOP-IVMを受け、27の未熟なCOCCが得られた。培養28時間後、MIIステージで7つの卵母細胞が選択され、ICSIによって受精した。5日後、2つの胚盤胞が凍結した。1つは2017年2月に解凍され、子宮に移された。患者は11月に健康な女の子を出産した。OP-IVMは、卵巣掘削手術を受けているPCOSを持つ不妊女性の卵母細胞検索に最初に適用され、現在、その適用は、不妊治療の保存のための癌女性でさえ、良性の婦人科手術を受ける必要がある不妊女性に拡大することができます。これは、IVMのアプリケーション範囲は、女性の不妊治療の保存のための他の伝統的な婦人科の操作と組み合わせることによって拡大することができることを示しています。
