Alle procedures werden goedgekeurd door de Animal Care and Ethics Committee van de University of New South Wales (17/109A). Vrouwelijke athymische Balb/c naakte muizen, in de leeftijd van 8-10 weken met een gewicht van 16-19 g, werden gebruikt voor dit protocol. Een resectie orthotopisch muismodel van alvleesklierkanker is belangrijk omdat het het testen van adjuvante en neoadjuvante behandelingen mogelijk maakt. Dit is vooral belangrijk bij alvleesklierkanker, waar chirurgie de meest effectieve behandeling blijft, maar geassocieerd is met een hoog risico op recidief. Dit artikel beschrijft een methode die op betrouwbare wijze een alvleesklierkanker zal produceren die mogelijk kan worden genezen met resectie, waarbij het klinische scenario wordt gerepliceert waarin neoadjuvante ...

Pancreas ductaal adenocarcinoom (alvleesklierkanker [PC]) wordt geassocieerd met een slechteprognose 1. Chirurgische resectie blijft de enige potentieel curatieve behandeling voor pc en moet worden overwogen voor patiënten met een ziekte in een vroeg stadium. Helaas is zelfs bij R0-resectie (d.w.z. resectiemarges vrij van tumor) het recidiefpercentage (lokaal of van onopgemerkte gemetastaseerde ziekte) hoog2,3. Daarom is systemische adjuvante therapie geïndiceerd bij bijna alle patiënten die resectie4ondergaan . Bovendien, terwijl neoadjuvante therapie nu alleen wordt aanbevolen voor borderline-reseceerbare kankers, breiden de indicaties zich zodanig uit dat het routinematige gebruik ervan de focus is van veel klinisch onderzoek5,6,7,8. Om nieuwe therapeutische benaderingen voor pc's met resectie te ontwikkelen, moeten deze benaderingen eerst worden beoordeeld in preklinische modellen die de klinische instellingen nauwkeurig samenvatten.
Orthotopische muismodellen van PC zijn in het verleden vaak gebruikt om medicamenteuze behandelingen te testen9,10. Veel van deze werden geproduceerd door injectie van kankercellen alleen in de alvleesklier van de muis, wat resulteerde in tumoren die het prominente stroma misten dat kenmerkend is voor pc. Meer recentelijk, co-injectie orthotopische modellen, zoals degene die we voor het eerst ontwikkeld door het injecteren van een mengsel van menselijke PC en menselijke pancreas stellate cellen (PSC's, de primaire producenten van de collageenachtige stroma in PC), zijn gekomen in regelmatig gebruik11,12. De tumoren die worden geproduceerd door een dergelijke co-injectie van kanker en stromale cellen vertonen (i) zowel de kankerelementen als de karakteristieke stromale (desmoplastische) component van pc, en (ii) verbeterde proliferatie van kankercellen en metastase11. Dit model lijkt dus sterk op menselijke pc. Hoewel een aantal resectiemodellen van orthotopische PC zijn beschreven13,14,15,16, geen enkele hebben weerspiegeld de klinische realiteit van pancreas resectie bij de mens zo nauwkeurig als dit model, en daarom zijn suboptimaal voor het testen van adjuvante of neoadjuvante behandelingen.
Het doel van het gepresenteerde muismodel was om te laten zien hoe: (i) orthotopische alvleesklierkanker met succes kan worden geïmplanteerd terwijl onbedoelde peritoneale verspreiding wordt geminimaliseerd en (ii) vervolgens de kanker volledig kan worden gereseceerd. an orthotopic resectional mouse model of pancreatic cancer

Er is een gebrek aan bevredigende diermodellen om adjuvante en/of neoadjuvante therapie te bestuderen bij patiënten die in aanmerking komen voor een operatie van alvleesklierkanker (PC). Om dit tekort aan te pakken, beschrijven we een muismodel met orthotopische implantatie van pc gevolgd door distale pancreatectomie en splenectomie. Het model is veilig en passend flexibel gebleken voor de studie van verschillende therapeutische benaderingen in adjuvante en neo-adjuvante omgevingen.
In dit model wordt een pancreastumor voor het eerst gegenereerd door een mengsel van menselijke pancreaskankercellen (luciferase-gelabeld AsPC-1) en met menselijke kanker geassocieerde pancreas stellate cellen te implanteren in de distale alvleesklier van Balb / c athymische naakte muizen. Na drie weken wordt de kanker gereseceerd door re-laparotomie, distale pancreatectomie en splenectomie. In dit model kan bioluminescentiebeeldvorming worden gebruikt om de voortgang van de ontwikkeling van kanker en de effecten van resectie/ behandelingen te volgen. Na resectie kan adjuvante therapie worden gegeven. Als alternatief kan een neoadjuvante behandeling worden gegeven voorafgaand aan de resectie.
Representatieve gegevens van 45 muizen worden gepresenteerd. Alle muizen ondergingen een succesvolle distale pancreatectomie/splenectomie zonder problemen met hemostase. Een macroscopische proximale pancreasmarge van meer dan 5 mm werd bereikt in 43 (96%) Muizen. Het technische slagingspercentage van pancreasresectie was 100%, met 0% vroege mortaliteit en morbiditeit. Geen van de dieren stierf in de week na de resectie.
Samenvattend beschrijven we een robuuste en reproduceerbare techniek voor een chirurgisch resectiemodel van alvleesklierkanker bij muizen dat het klinische scenario nabootst. Het model kan nuttig zijn voor het testen van zowel adjuvante als neoadjuvante behandelingen.

Negenenvijftig opeenvolgende muizen ondergingen een implantatieoperatie. Bruto lekkage trad op bij acht (14%) Muizen. De mate van lekkage op het moment van injectie wordt geschat zoals hierboven beschreven in de protocolsectie. Na drie weken om deze geïmplanteerde tumoren te laten groeien, werd pre-resectie bioluminescentie beeldvorming uitgevoerd om muizen met grove gemetastaseerde ziekte uit te sluiten voorafgaand aan resectie. Vijfenveertig (76%) muizen ondergingen een chirurgische resectie.
