En reseksjonell ortotopisk musemodell av kreft i bukspyttkjertelen er viktig fordi den tillater testing av adjuvante og neoadjuvante behandlinger. Bukspyttkjertelkanal adenokarsinom (kreft i bukspyttkjertelen [PC]) er forbundet med en dårlig prognose1. Kirurgisk reseksjon er fortsatt den eneste potensielt kurative behandlingen for PC og bør vurderes for pasienter som presenterer med tidlig stadium sykdom. Dessverre, selv med R0 reseksjon (dvs. reseksjonsmarginer fri for svulst), er tilbakefallsraten (lokal eller fra uoppdaget metastatisk sykdom) høy2,3. Derfor er systemisk adjuvant terapi indikert hos nesten alle pasienter som gjennomgår reseksjon4. Videre, mens neoadjuvant terapi nå anbefales bare for borderline-resectable kreft, dens indikasjoner utvides slik at den rutinemessige bruken er fokus for mye klinisk forskning5,6,7,8. Ortotopiske musemodeller av PC har tidligere blitt brukt til å teste narkotikabehandlinger9,10. Mange av disse ble produsert ved injeksjon av kreftceller alene i musen bukspyttkjertelen, noe som resulterte i svulster som manglet den fremtredende stroma som er karakteristisk for PC. Mer nylig har koinjeksjons ortotopiske modeller, som den vi først utviklet ved å injisere en blanding av human PC og humane bukspyttkjertel stellate celler (PSCer, de primære produsentene av kollagen stroma i PC), kommet i regelmessig bruk11,12. an orthotopic resectional mouse model of pancreatic cancer

Det er mangel på tilfredsstillende dyremodeller for å studere adjuvans- og/eller neoadjuvantbehandling hos pasienter som vurderes for kirurgi av kreft i bukspyttkjertelen (PC). For å løse denne mangelen beskriver vi en musemodell som involverer ortotopisk implantasjon av PC etterfulgt av distal pankreatektomi og praktektomi. Modellen har vist seg å være trygg og passende fleksibel for studiet av ulike terapeutiske tilnærminger i adjuvante og neo adjuvante omgivelser.
I denne modellen genereres en bukspyttkjertelsvulst først ved å implantere en blanding av humane kreftceller i bukspyttkjertelen (luciferase-tagget AsPC-1) og human kreft forbundet bukspyttkjertel stellate celler i distal bukspyttkjertelen av Balb / c athymic naken mus. Etter tre uker blir kreften resected av re-laparotomi, distal pankreatektomi og praktektomi. I denne modellen kan bioluminescensavbildning brukes til å følge utviklingen av kreft og effekter av reseksjon/behandling. Etter reseksjon kan adjuvant terapi gis. Alternativt kan neoadjuvant behandling gis før reseksjon.
Representative data fra 45 mus presenteres. Alle mus gjennomgikk vellykket distal pankreatektomi / praktektomi uten problemer med hemostase. En makroskopisk proksimal bukspyttkjertelmargin større enn 5 mm ble oppnådd hos 43 (96 %) mus. Den tekniske suksessraten for reseksjon i bukspyttkjertelen var 100%, med 0% tidlig dødelighet og sykelighet. Ingen av dyrene døde i løpet av uken etter reseksjon.
Oppsummert beskriver vi en robust og reproduserbar teknikk for en kirurgisk reseksjonsmodell av bukspyttkjertelkreft hos mus som etterligner det kliniske scenariet. Modellen kan være nyttig for testing av både adjuvans- og neoadjuvantbehandlinger.

59 påfølgende mus gjennomgikk implantasjonskirurgi. Det oppstod grov lekkasje hos åtte (14 %) mus. Graden av lekkasje på injeksjonstidspunktet er estimert som beskrevet ovenfor i protokolldelen. Etter tre uker for å tillate disse implanterte svulstene å vokse, ble pre-reseksjon bioluminescence imaging utført for å utelukke mus med grov metastatisk sykdom før reseksjon. Førtifem (76 %) mus gjennomgikk kirurgisk reseksjon.
