En sambandsbar ortopisk musmodell av bukspottskörtelcancer är viktig eftersom det möjliggör testning av adjuvans- och neoadjuvantbehandlingar. Bukspottskörteln duktal adenocarcinom (bukspottskörteln cancer [PC]) är associerad med en dålig prognos1. Kirurgiska samband är fortfarande den enda potentiellt botande behandlingen för PC och bör övervägas för patienter som presenterar med tidig sjukdom. Tyvärr, även med R0 samband (dvs. samband marginaler fri från tumör), är återkommande frekvensen (lokal eller från oupptäckt ögonbevarande sjukdom) hög2,3. Därför är systemisk adjuvansbehandling indicerad hos nästan alla patienter som genomgår samband4. Dessutom, medan neoadjuvant terapi nu rekommenderas endast för borderline-resectable cancer, dess indikationer expanderar så att dess rutinmässiga användning är i fokus för mycket klinisk forskning5,6,7,8. För att utveckla nya terapeutiska metoder för PC som involverar samband, dessa metoder måste först bedömas i prekliniska modeller som korrekt rekapitulera kliniska inställningar.
Ortopediska musmodeller av PC har använts ofta tidigare för att testa läkemedelsbehandlingar9,10. Många av dessa producerades genom injektion av cancerceller ensam i mus bukspottkörtel, vilket resulterar i tumörer som saknade den framträdande stroma som är karakteristisk för PC. Mer nyligen har ortopiska modeller med injektion, såsom den vi först utvecklade genom att injicera en blandning av mänskliga PC och mänskliga bukspottskörtel stellatceller (PSC, de primära producenterna av kollagenet stroma i PC), kommit i regelbunden användning11,12. Tumörer som produceras av sådan saminjektion av cancer och stromal celler uppvisar (i) både cancerelementen och den karakteristiska stromal (desmoplastiska) komponenten i PC, och (ii) förbättrad cancer cell spridning och metastasering11. Således liknar denna modell nära mänsklig DATOR. Medan ett antal sambandsmodeller av ortopisk PC harbeskrivits 13 , 14,15,16, ingen har återspeglat de kliniska realiteterna av bukspottskörteln samband hos människor så exakt som denna modell, och därför har varit suboptimal för att testa adjuvant eller neoadjuvant behandlingar.
Syftet med musmodellen som presenterades var att visa hur man: (i) framgångsrikt implantera ortopisk bukspottkörtelcancer samtidigt som oavsiktlig peritoneal spridning minimeras och (ii) därefter helt återförsluta cancern. Papperet belyser tips och potentiella fallgropar av denna teknik.

an orthotopic resectional mouse model of pancreatic cancer

Det saknas tillfredsställande djurmodeller för att studera adjuvans och/eller neoadjuvant terapi hos patienter som övervägs för kirurgi av bukspottskörtelcancer (PC). För att ta itu med denna brist beskriver vi en mus modell som omfattar ortopic implantation av PC följt av distala bukspottkörtelektomi och splenectomy. Modellen har visat sig vara säker och lämpligt flexibel för studier av olika terapeutiska metoder i adjuvans och neo adjuvans inställningar.
I denna modell genereras först en bukspottskörteltumör genom att implantera en blandning av mänskliga bukspottskörtelcancerceller (luciferasmärkt AsPC-1) och human cancer associerade bukspottskörtel stellate celler i de distala bukspottkörteln av Balb/c athymic naken möss. Efter tre veckor, cancern resected av re-laparotomy, distala bukspottkörtelektomi och splenectomy. I denna modell kan bioluminescensavbildning användas för att följa utvecklingen av cancer och effekterna av samband/behandlingar. Efter samband kan adjuvansbehandling ges. Alternativt kan neoadjuvant behandling ges före samband.
Representativa data från 45 möss presenteras. Alla möss genomgick framgångsrika distala bukspottkörtelektomi/splenectomy med några problem med hemostas. En makroskopisk proximal bukspottskörtelmarginal större än 5 mm uppnåddes i 43 (96%) möss. Den tekniska framgångsgraden för bukspottskörteln samband var 100%, med 0% tidig dödlighet och sjuklighet. Inget av djuren dog under veckan efter sambandet.
Sammanfattningsvis beskriver vi en robust och reproducerbar teknik för en kirurgisk sambandsmodell av bukspottskörtelcancer hos möss som efterliknar det kliniska scenariot. Modellen kan vara användbar för testning av både adjuvanta och neoadjuvant behandlingar.

Femtionio möss i följd genomgick implantationskirurgi. Bruttoläckage inträffade i åtta (14%) möss. Graden av läckage vid injektions tidpunkten uppskattas enligt beskrivningen ovan i protokollavsnittet. Efter tre veckor för att tillåta dessa implanterade tumörer att växa, utfördes pre-resection bioluminescens imaging för att utesluta möss med brutto ögonbevarande sjukdom före samband. Fyrtiofem (76 %) möss genomgick kirurgiska samband.
