血管内血栓摘出術とtPA媒介性血栓溶解療法は、米国で年間~70万人の患者を苦しめている急性虚血性脳卒中に対して、米国食品医薬品局(FDA)が承認した唯一の治療法です1。血栓摘出術の適用は大血管閉塞症(LVO)に限定されていますが、tPA血栓溶解療法は小血管閉塞を緩和する可能性があるため、どちらも急性虚血性脳卒中の貴重な治療法です2。さらに、両方の治療法の組み合わせ(例:脳卒中発症から4.5時間以内にtPA血栓溶解療法を開始し、その後血栓摘出術を行う)により、再灌流と機能的転帰が改善されます3。このように、血栓溶解の最適化は、血栓摘出術の時代になっても、脳卒中研究にとって重要な目標であり続けています。
血栓塞栓症モデルは、血栓溶解療法の改善を目的とした前臨床脳卒中研究に不可欠なツールです。これは、機械的血管閉塞モデル(例:管腔内縫合MCA閉塞)が血栓を作らず、機械的閉塞除去後の脳血流の迅速な回復が過度に理想化されているためです4,5。現在までに、主要な血栓塞栓症モデルには、光血栓症6,7,8、局所塩化第二鉄(FeCl3)アプリケーション9、MCA枝へのトロンビンのマイクロインジェクション10,11、MCAまたは総頸動脈(CCA)へのex vivo(マイクロ)塞栓の注入12,13,14、および一過性低酸素虚血(tHI)15,16が含まれます。17,18。これらの脳卒中モデルは、その後の血栓の組織学的組成とtPAを介した溶解療法に対する感受性が異なります(表1)。それらはまた、開頭術の外科的要件(in situトロンビン注射およびFeCl3の局所適用に必要)、梗塞のサイズと位置の一貫性(例、微小塞栓のCCA注入は非常に異なる結果をもたらす)、および心血管系への全体的な影響(例:tHIは心拍数と心拍出量を増加させ、低酸素誘発性末梢血管拡張を補う)にもさまざまである。
RB色素ベースの光血栓性脳卒中(PTS)モデルには、開頭術を使わない簡単な外科的処置、低い死亡率(通常<5%)、梗塞のサイズと位置の予測可能(MCA供給領域内)など、多くの魅力的な特徴がありますが、2つの大きな制限があります。8 最初の注意点は、tPAを介した血栓溶解治療に対する反応が弱いかゼロであることであり、これもFeCl3モデル7,19,20の欠点です。PTSおよびFeCl3脳卒中モデルの2番目の注意点は、その後の血栓が少量のフィブリンを含む高密度の血小板凝集体で構成されており、tPA溶解療法に対する回復力につながるだけでなく、急性虚血性脳卒中患者における血小板:フィブリン血栓の混合パターンからも逸脱していることです21,22。対照的に、in situトロンビン−マイクロインジェクションモデルは、主に重合フィブリンと不確実な血小板の含有量を含む10。

患者サンプルに類似した血栓を産生し、臨床的に関連する血栓溶解療法に応答する脳卒中モデルを作成したいと考えています。そこで、古典的な光血栓性脳卒中モデルを修正し、光活性化の前にトロンビンと光力学的色素であるローズベンガルを混合します。この手順により、tPAを介した血栓溶解に非常に敏感な血栓が実際に作成されることを嬉しく思います。この修正された光血栓性脳卒中モデルは、死亡率の低い非常に単純な外科的処置を使用しており、非常に一貫した梗塞のサイズと位置を生成しました。また、急性脳卒中患者と同様のフィブリンと血小板の混合血栓も生成します。したがって、前臨床脳卒中研究において、より効果的な血栓溶解療法の開発は非常に有用であると考えています。より多くの方にご利用いただければ幸いです。手順を実演するのは、私たちのグループの研究助教であるYu-Yo Sun博士です。手術の30分前に、鎮痛剤を注射してマウスを準備します。マウスに麻酔をかけた後、つま先をつまんで、完全に麻酔されていることを確認します。次に、左首と頭の毛を脱毛クリームで取り除きます。マウスを小動物用アダプターに仰臥位で置き、ポビドンヨードと70%エタノールを交互に3回スワイプして手術皮膚領域を滅菌します。解剖顕微鏡下で、正中線から外側約0.2センチメートルで、マイクロハサミとまっすぐな鉗子を使用して0.5センチの左頸部切開を行います。次に、一対の細い鋸歯状鉗子を使用して、軟部組織と筋膜を引き離してLCCAを露出させます。次に、一対の細かく滑らかな鉗子を使用して、LCCAを迷走神経から慎重に分離します。5-0シルク縫合糸を使用して、LCCAの周囲に恒久的なダブルノット縫合糸を配置します。マウスをうつ伏せの位置にひっくり返し、ノーズクリップロールを15度回転させます。次に、ベタジンと70%エタノールを交互に3回スワイプして皮膚を拭いて、手術部位を滅菌します。左目と耳に沿ってマイクロハサミとまっすぐな鉗子を使って頭皮を0.8センチ切開し、側頭筋を露出させます。次に、左頭頂骨の側頭筋の端に沿って0.5センチの切開を行います。次に、側頭筋に2回目の0.3センチメートルの垂直切開を行い、筋肉を引っ込めて頭頂骨と扁平上皮骨の端を露出させます。前頭骨と頭頂骨の間の冠状縫合糸のランドマークを必ず視覚化してください。次に、滅菌生理食塩水を塗布して頭蓋骨を湿らせて左MCAを明らかにし、扁平上皮骨の近位MCA枝にマーカーで印を付けます。このマークされた領域を空気圧式歯科用ドリルで囲む直径1ミリメートルの円をそっと描きます。次に、頭蓋骨を下の硬膜に触れずに約0.2ミリメートルの深さに薄くします。骨の非常に薄い層が残ったら、穴あけを中止します。次に、マウスの体重に基づいてトロンビンとローズベンガル溶液を混合し、31ゲージの針で眼窩後洞にゆっくりと混合物を注入します。乾燥を防ぐために両目に軟膏を塗ります。532ナノメートルのレーザー光線でイルミネーターを2インチ以内の穴あけ部位に20分間照射します。MCAの近位枝の照明をレーザー投影ゴーグルで可視化します。20分後、レーザー照射を停止します。頭蓋骨の頭頂骨に直径3ミリの頭蓋窓を作り、その上にカバーガラスを置きます。次に、遠位MCAを20倍の水浸対物レンズの下に配置します。イメージングの5分前に、DyLight 488標識抗GP1b-β抗体を尾静脈注射して循環血小板を標識します。次に、トロンビンとローズベンガル溶液の混合物を後眼窩的に注入します。直径10マイクロメートルのレーザービームを備えた532ナノメートルレーザーシステムを使用してMCAを光活性化し、血栓形成までの画像を記録します。tPAを投与する場合は、麻酔をかけた動物を摂氏37度の温熱パッドの上に置きます。選択した光活性化後の時点で、摂氏45度の温かい湿ったガーゼで尾を1分間包みます。次に、組換えヒトtPAを尾静脈から注入します。ボーラスとして50%を注入し、輸液ポンプを使用して残りの50%を30分かけて注入します。.脳血流を監視するには、頭蓋骨を露出させて頭皮を正中切開します。滅菌生理食塩水で頭蓋骨に潤いを与え、超音波ジェルを優しく塗布し、ジェル内の髪の毛や気泡を避けます。レーザースペックルコントラストイメージャーの下で、両方の大脳半球の脳血流を10分間監視します。免疫蛍光標識は、ローズベンガル色素ベースの光血栓症において、MCA枝がCD41陽性血小板とほとんどフィブリンで密集していたことを示している。対照的に、トロンビンとバラベンガル光血栓症では、MCA枝はランダムに混合された血小板とフィブリンの血栓によって閉塞されました。イムノブロッティング分析では、ローズベンガル単独と比較して、トロンビンとローズベンガル光血栓症の同側半球でのフィブリン沈着が2倍以上増加することが示されました。共焦点顕微鏡を用いた生体内イメージングでは、トロンビンの静脈内注射では、レーザー照射下でも血小板凝集体を誘導できないことが示されました。血小板は、バラベンガル光血栓症モデルでは均質な血栓を形成しましたが、トロンビンとローズベンガルモデルでは複数のかすかな領域を伴う不均一な凝集体を形成しました。tPA治療前およびtPA治療後24時間における同じマウスのCBFとビヒクル治療をレーザースペックルコントラストイメージングで測定し、対側半球に正規化しました。バラベンガル光血栓症では、tPA処理により、ビヒクル治療マウスと比較して、特に虚血性境界領域でCBF回復の傾向が見られました。トロンビンとバラベンガル光血栓症では、tPA処理マウスにおけるCBFの回復がより顕著であり、近位のMCA枝が24時間で見えることが多かった。バラベンガル光血栓症では、ビヒクル処理マウスとtPA処理マウスで同様の梗塞サイズが検出されました。対照的に、トロンビンとバラベンガル光血栓症では、tPA溶解療法は、ビヒクル治療マウスと比較して、光活性化後0.5時間、1時間、または2時間で梗塞を有意に減少させましたが、光活性化後6時間では減少しませんでした。この方法は、血栓組成の調査や血栓溶解治療のさらなる研究に使用できます。この手順に続いて、血栓溶解療法の他の方法を適用し、後の有効性を決定することができます。トロンビンを添加することで、血栓組成をフィブリン不足からフィブリン増加に調整することが可能になり、研究者は臨床的に関連する治療戦略を開発することができます。
