혈관 내 혈전제거술과 tPA 매개 혈전용해술은 미국 식품의약국(FDA)이 승인한 급성 허혈성 뇌졸중 치료법 중 유일하게 두 가지로, 미국에서 매년 ~700,000명의 환자를 괴롭히고 있습니다1. 혈전절제술은 대혈관 폐색(LVO)으로 제한되기 때문에 tPA 혈전용해술은 소혈관 폐색을 완화할 수 있지만, 둘 다 급성 허혈성 뇌졸중의 중요한 치료법이다2. 또한, 두 치료법의 조합(예: 뇌졸중 발병 후 4.5시간 이내에 tPA-혈전용해술을 시작한 후 혈전절제술을 시행)은 재관류와 기능적 결과를 개선한다3. 따라서 혈전용해술을 최적화하는 것은 혈전제거술의 시대에도 뇌졸중 연구의 중요한 목표로 남아 있습니다.
혈전색전증 모델은 혈전용해 요법을 개선하는 것을 목표로 하는 전임상 뇌졸중 연구에 필수적인 도구입니다. 이는 기계적 혈관 폐색 모델(예: 내강 내 봉합사 MCA 폐색)은 혈전을 생성하지 않으며, 기계적 폐색 제거 후 대뇌 혈류의 빠른 회복이 지나치게 이상화되어 있기 때문이다 4,5. 현재까지 주요 혈전색전증 모델에는 광혈 전증 6,7,8, 국소 염화제2철(FeCl3) 적용9, MCA 분지(10,11)로의 트롬빈 미세주입, MCA 또는 총경동맥(CCA)에 체외(미세)색전증 주입(CCA)12,13,14 및 일과성 저산소증-허혈(tHI)15,16, 17,18. 이러한 뇌졸중 모델은 혈전의 조직학적 조성과 tPA 매개 용해 요법에 대한 민감도가 다릅니다(표 1). 또한 개두술의 외과적 요구 사항(제자리 트롬빈 주사 및 FeCl3의 국소 적용에 필요), 경색 크기 및 위치의 일관성(예: 미세색전의 CCA 주입은 매우 다양한 결과를 산출함) 및 심혈관계에 대한 전반적인 효과(예: tHI는 저산소증으로 인한 말초 혈관 확장을 보상하기 위해 심박수 및 심박출량을 증가시킴)에 따라 다릅니다.
RB 염료 기반 광혈전 뇌졸중(PTS) 모델은 개두술이 필요 없는 간단한 수술 절차, 낮은 사망률(일반적으로 < 5%), 예측 가능한 경색의 크기 및 위치(MCA 공급 영역에서) 등 많은 매력적인 기능을 가지고 있지만 두 가지 주요 제한 사항이 있습니다. 8 첫 번째 주의 사항은 tPA 매개 혈전용해 치료에 대한 반응이 약하거나 전무하다는 것인데, 이는 FeCl3 모델 7,19,20의 단점이기도 합니다. 위와 같은 추론을 감안할 때, 얇아진 두개골을 통한 MCA 표적 광활성화를 위해 RB와 트롬빈의 아혈전 용량을 혼합하면 결과 혈전의 피브린 성분이 증가하고 tPA 매개 용해 치료에 대한 민감도가 높아질 수 있다는 가설을 세웠습니다. 우리는 이 가설을 확인했으며,23 여기에서 변형된(T+RB) 광혈전 뇌졸중 모델의 세부 절차를 설명합니다.
