卵巣がんは、世界で8番目に多い女性悪性腫瘍であり、年間約30万人の新規診断と推定18万人の死亡があります1。最初の診断では、卵巣がんはしばしば重篤な症状を呈し、患者の約75%がすでにステージIII〜IVにあります。したがって、5年生存率は<30%であり、死亡率は婦人科がんの中で最も高く2、卵巣がんの治療効率は、減量手術の成功、化学療法に対する抵抗性、初期治療後の再発などの臨床的要因に大きく依存しています。
卵巣がん組織は階層的に組織化されており、すべての腫瘍成分が等しく子孫を生成できるわけではありません。自己複製して不均一な腫瘍細胞集団を産生できる唯一の細胞は、癌幹細胞(CSC)を表すと考えられています3。CSCの自己複製および腫瘍開始は、支持ニッチを維持する目的でそれらの腫瘍微小環境を再構築するための血管新生の促進を伴う。しかし、従来のモデルは、複数回継代後のスフェロイドの破壊により、臨床サンプル由来のCSCの培養の再現性が限られていたため、in vitro分析に利用できませんでした。最近では、患者からCSCを培養する実験方法がいくつかの用途のために開発されている4、5、6、7。特に、無血清培地を用いた超低接着プレートでスフェロイドを形成して増殖するCSCの特性を利用して、培養したCSCは、多系列分化能を有する正常な腫瘍細胞では発現しない幹細胞表面マーカーを発現するように誘導される8,9。
最近のデータでは、腹膜での播種として視覚化された休眠中の卵巣(O)CSCの持続が再発腫瘍としての再生に関連していることが示されています10。したがって、OCSCの分子的および生物学的特徴を理解することで、これらの細胞の効果的な標的化と根絶が可能になり、腫瘍の寛解が期待できます。特に、血管新生におけるCSCの役割の細胞的および分子的機構的特徴に関してはほとんど知られていない11。したがって、本プロトコルでは、患者由来のOCSCをin vitro設定で使用し、臨床現場での転移部位のCSCと内皮細胞の腫瘍微小環境を模倣する可能性のある共培養モデルを使用して内皮細胞の血管新生特性を調査しました。最終的に、新生血管形成は腫瘍の成長と転移をサポートするために必要な重要なプロセスを構成するため、そのメカニズムをよりよく理解することで、転移部位のOCSCに対する新しい標的療法の開発が可能になります。
ここでは、CSCsを取り囲む新生血管形成ステップを経時的に可視化するためのプロトコールを提示する。このプロトコルの利点には、3D共培養システムNICO-1を使用して完全に再現可能な調査を可能にし、それによって内皮細胞の血管新生中のOCSC由来の腫瘍開始能力の患者への影響を観察できることが含まれます。
