虚血再灌流傷害(IRI)は、肝臓移植後の一般的な合併症である。肝疾患は、虚血媒介性細胞損傷および肝再灌流の異常な悪化を伴う病理学的プロセスである。肝性IRIと局所的な自然免疫応答は、熱と冷間のIRIに分けることができる、臨床環境の違いに応じて1。ホットIRIは、通常、肝臓移植、ショック、および外傷2の結果として、幹細胞傷害によって誘発される。冷たいIRIは、内皮細胞および末梢循環によって引き起こされる肝臓移植の合併症である3.臨床報告によると、肝性IRIは初期臓器不全の10%に関連しており、急性および慢性拒絶反応4,5の発生率を増加させる可能性がある。また、肝性のIRIはまた、多臓器不全症候群または全身性炎症反応症候群を誘発し、高死亡率6を有する。肝外臓器の関与を有する患者は、病院に長く滞在し、より多くのお金を費やし、より悪い予後7を持っている傾向があります。合併症の発症は、肝臓移植8の肝相の長さと密接に関連している。
ラットにおける異形性肝移植(OLT)は、1973年にアメリカの教授リーによって最初に報告された。実験手術では、縫合法を用いて、臨床肝移植のステップと血管の吻合と共通胆管(CBD)をシミュレートした。この手順は難しく、時間がかかり、成功率が低い9.1979年、KamadaらのラットのOLTは、26分以内に動脈性相を制御する門脈の吻合に対する「ツーカフ法」を創造的に用いることで、ラットのOLTを大幅に改善した。同年、ツィンマーマンは「単一胆管ステント法」を提案した。リーの研究に基づいて、ツィンマーマンは、直接ドナーとレシピエントのCBDを吻合するためにポリエチレンチューブを使用し、CBDの再構築を簡素化し、括約筋の機能を維持し、この方法は、OLTモデル11の胆道再建のための標準となった。1980年、宮田らは、門脈静脈(PV)、上皮性大静脈(SVC)、および肝内大静脈(IVC)がカフ法によって吻合された「三カフ法」を提案した。しかし、この方法ではカニューレの歪みが生じるおおありで、これは、下の大静脈逆流12の閉塞を招く可能性がある。1983年には、PVとIVCの吻合にカフ法を用いて「ツーカフ法」が提案されたが、SVC13には縫合法を採用した。この方法は、OLTモデルを確立するために世界的に学者によって採用されました。それ以来、カフ吻合のステップは、肝相を短くし、ラット14の生存率を向上させるために改善された。同様に、改善された方法は、アン肝相15を短縮するために臨床現場で使用される。しかし、肝臓移植後のIRIに関する基礎研究は、生存率が肝外器官への傷害の程度に反比例することを示している。そのため、さらなる研究が必要であり、肝臓移植後のIRIをシミュレートするためには、簡単で再現性の高い動物モデルが必要です。
肝相の定義に基づいて、ラットの肝外器官のIRIをもたらす肝臓移植における血行力学的変化をシミュレートした。本明細書では、肝臓移植後のIRIの基礎研究を容易にするために、ラットにおけるアン肝相(肝臓虚血)の動物モデルを構築する方法の詳細な説明を提供する。

ラットにおける正体異形肝移植(OLT)は、肝外器官の虚血再灌流傷害(IRI)を含む術前、術中および術後の研究に使用される実証済みの動物モデルである。このモデルには、数多くの実験とデバイスが必要です。肝相の持続時間は、移植後にIRIを発症する時間と密接に関連している。本実験では、血行力学的変化を用いてラットの肝外臓器損傷を誘発し、最大許容時間を決定した。最も重度の臓器損傷までの時間は、異なる器官のために変化した。この方法は、容易に複製することができ、また、肝臓移植後の肝外器官のIRIを研究するために使用することができる。

Organ Ischemia-Reperfusion Injury by Simulating Hemodynamic Changes in Rat Liver Transplant Model

ラット肝移植モデルにおける血行力変化をシミュレートすることにより、臓器虚血再灌流傷害虚血再灌流傷害は、肝移植後の一般的な適用である。肝外臓器関与の患者は、入院時間が長く、より多くを費やし、予後が悪くなる傾向がある。合併症の発症は、肝臓移植の肝相の長さと密接に関連している。しかし、肝移植後の虚血再灌流傷害に関する基礎研究は、肝移植後の生存率が、外核臓器への傷害の程度に反比例することを示している。解剖期の定義に基づき、肝移植における血行力学的変化をシミュレートし、ハプティック外器官の虚血再灌流傷害を引き起こした。本論文の中で、ラットにおけるアンハプティック相の動物モデルを構築する方法について、肝移植後の虚血再灌流傷害に関する基礎研究について詳細に説明する。元元博士は、すべてのビデオ動物の操作を担当しています。2つの眼圧力、ストレートおよびベンディング、および眼間レンズ、IOL鉗子、眼科用はさみ、1~2つのレトラクター、綿棒、3〜0個の外科縫合糸、ヨドフォームガーゼ、縫合針、必要に応じてバイオシステム、および眼間レンズ、IOL鉗子は、円形のために血管を分離するために使用される。動物麻酔機械、脱水装置、 埋め込み機械、病理学的スライサー、冷凍機、組織スライサー、空気乾燥オーブン、神経光学顕微鏡、スキャナー、一般麻酔用のエンフルラン、4%パラホルムアルデヒド溶液、任意のエタノール、キシレン、高効率パラフィンワックス、ヘマトキシリンエオシン色素溶液、ハイドロリン酸、アンモニアスライド、ガラスカバー、中性、およびガムスライタ、および死ぬ材料、ピペット、遠心分離機、自動生化学分析装置、アラニンアミノトランスファーゼ、ALTキット、アスパラギン酸、アミノトランスレシーゼ、ASTキット、クレアチニンキット。体重を量った後、ラットは動物麻酔装置によってイオフルランで麻酔される。腹部消毒後、鉗子とはさみを使用して、xiphoidプロセスの下に3センチメートルの中央値切開を行います。腹腔を開き、レトラクターを使用して肝臓を露出させ、肝胃靭帯を動員する。綿棒を使用して肝臓の中央葉を優しく反転させ、上向きに回してポルタの習慣を露出させます。胆管、門脈、肝動脈を特定します。綿のスワップを使用して左下腹腔に向かって小腸を押し、下の静脈を右腎静脈に移動させ、門脈、肝動脈、眼内レンズで右腎静脈の上の下静脈を分離し、IOL、それぞれ3-0シルク糸でマークされた鉗子をスリップノットで覆う。左右下肢の皮膚を切り開き、眼用鉗子を使用して大腿静脈を露出させる。低分子量ヘパリン625 IU/kgを大腿静脈からゆっくりと注入し、全身をヘパリン化する。3-0の縫合糸を持つ右腎静脈の上に門脈、肝動脈、および下の静脈をリゲートし、45分間持続する。腹腔内の小腸を交換し、ガーゼで覆います。吸入麻酔はこれらの期間の間に減少する。45分後、門脈、肝動脈、右腎静脈の上の下静脈を解放する。筋肉と皮膚層を層状に縫合し、吸入麻酔を終了する。4時間ごとに5mg/kgの皮下モルヒネを使用して術後鎮痛を提供する 目を覚まし、25プラスマイナス2度の温度で供給し続け、湿度50プラスマイナス10%の動物暖房ランプが必要です。この動物モデルでは、ラットを15分間の虚血に従って5群に無作為に分け、I15群、30分、I30群、45分、I45群、60分、I60、及び各群に10匹のラットを有するシャム群を用いた。各群の生存率は、手術後14日目に観察された。すべてのラットはI 15、I 30およびシャム群で生き残った。8人はI 45グループで14日間生き残り、I 60グループで生き残ったのはわずか2人でした。結果は、ラットがせいぜい45分間、肝相を許容できることを示唆した。実験の間、バイオシステムは、無肝期前後の心拍数および血圧、右内頸動脈挿管の変化を記録するために使用された。この結果、心拍数と平均動脈圧であるラットのMAPは、血管結紮後に劇的に変化することがわかりました。結紮後、腸内の肝虚血、鬱血、および浮腫、胃静脈瘤および脾腫が明らかであった。80匹のラットを45分間虚血を含む8群に無作為に分け、T0、6時間の再灌流、T6、12時間、T12、24時間、T24、48時間、T48、72時間、T72、7日間、T7、T7、それぞれ14日間、T14。腎臓、膵臓、小腸、心臓および肺組織は、ラットが屠殺された後にHEで採取され、染色された。心臓を除いて、病理学的スコアはそれぞれ参照に従って与えられた。結果は、肝外器官の最大傷害の時間が変化することを示した。膵臓の手術後6~24時間、肺では24~48時間であった。腸管と腎臓は、虚血の45分後に最も重傷を負った。手術後24時間で腸粘膜の明らかな異常はなく、腎臓は48時間後に回復した。心臓に関しては、再灌流後の時間の経過とともに、局所心筋細胞ナルコシス、細胞の断片化および溶解、炎症細胞浸潤、および局所血管拡張および鬱血が手術後24〜48時間までに組織中に発見された。血清を採取し、ALT、AST、クレアチニン、およびアミラーゼのレベルを自動生化学分析装置によって検出した。すべての指標は、病理学的変化とは異なり、24〜48時間でピークに達した。レベルは手術後48時間は基本的に正常であったが、病理学的損傷はまだ続いていた。要約すると、ラットのモデルは単純で、マイクロサージャリーなしで使いやすい、肝虚血後の肝外器官の虚血再灌流傷害の基礎的な研究に基礎を提供する。
