同時tDCS +VRを確保するためには、2つの側面が重要です。1) tDCS のセットアップ中に達成されるインピーダンスと 2) VR の開始に近接して tDCS デバイスを開始します。後者の問題は比較的簡単であり、2 mA強度が25分の期間20で適用される場合、tDCSの安全限界内に十分に残っている間、tDCSが継続的にVRプレゼンテーション全体に適用されることを保証する必要があります。インピーダンスに関しては、低インピーダンスが望ましい。適切なインピーダンス、または接触品質が達成されるかどうかは、使用されるtDCSデバイスによって異なります。一部のデバイスは、低い方が良いオームでインピーダンスを表示しますが、他のデバイスは接触品質を表す10ポイントまたは20ポイントのディスプレイスケールを使用し、より高い方が優れています。特定の装置に関係なく、通常の生理食動物の使用は、0.9%NaCl溶液、電極スポンジを湿らせるための規則的な水道水とは対照的に、インピーダンス35を改善する。tDCSのより深刻な副作用の1つである35、36の小さな皮膚病変の発生に関連しているため、通常の水道水の使用はさらに避けるべきである。また、皮膚病変は、電極の下の皮膚がtDCS37より前に激しく擦り付けられ、導電性ゲルが使用されている場合にも起こり得るため、35、38、乾燥させることができ、したがって避けるべきである。最後に、tDCS を起動する前に高いインピーダンスを行うと、デバイスの所定の安全パラメータに達するか、それを超え、デバイスが VR 管理の途中でシャットダウンするトリガーとなります。電極スポンジを十分に湿らせ、充分なインピーダンスを確保することは重要ですが、VRヘッドセットを設置すると生理食音が漏れたり滴したりする可能性があるため、電極を過度に浸さないようにバランスを取る必要があります。生理の漏れは、電流が大きな領域に「広がる」ため、tDCSの強度(mA)および電極のサイズ(cm2)に依存する、より低いが未知の電流密度39をもたらす可能性がある。同様に、VRヘッド搭載ディスプレイは、参加者が頭を動かすにつれて電流の流れや電極のシフトの中断を避けるために、スポンジ/電極に物理的に触れないことが重要です。
このプロトコルでは、皮膚の伝導度は主要な結果尺度と考えられる。皮膚伝導度は、交感神経系活動40の精神生理学的尺度である。環境温湿度の影響、加齢、喫煙状態、カフェインの使用、抗コリン作用を有する薬物の使用など、皮膚のコンダクタンス獲得に関連する典型的な要因を考慮する必要がありますが、必ずしも排除することはできません。例えば、VRセッションの前にカフェイン含有製品の使用を控えるように参加者に求めることは可能ですが、抗うつ薬を中止するよう求めるのは倫理的ではありません。さらに、必ずしも明確ではない理由から、個人の一部は、非常に低いまたは測定不可能な皮膚伝導度レベルおよび/または皮膚の導電応答を示す、図4で強調されている。したがって、データの損失または不在を許容するために十分なサンプル サイズを登録することが重要です。このプロトコルの実装に固有の、それはまた、イベントマーカーが現在、精神生理学的データキャプチャ中に手動で入力されていることを言及すべきである。これは制限事項ですが、病院システムでは、非病院管理コンピュータ(この場合はVR環境を実行するコンピュータ)を暗号化された病院情報技術ネットワークに接続できないことは珍しくありません。これは、VR環境を実行するコンピュータが病院ネットワーク上にある精神生理学的データキャプチャコンピュータに信号(TTLパルスを介して)送信させることは不可能であることを意味します。あまりエレガントではありませんが、1つの解決策は、各VRセッション中に2人の研究チームメンバーを出席することです。VR 管理を制御する 1 つと、各図の上部に表示されるように、手動でイベント マーカーを入力するサイコ生理学的トレース (図 1、図 2、図 3、図 4を参照) を参照してください。ただし、VR コントローラによって VR イベントが開始され、2 人目がイベント マーカーに入ったときから、わずかな時間差が存在する場合、この問題は、わずかな時間差の存在に対処していません。今後の調査では、イベント マーカーを自動的に登録できるように、この問題を軽減する必要があります。しかし、VR環境を運営する人とは異なる第2の研究チームメンバーの存在を、セッション中に参加者を観察できる人を強くお勧めします。一部の参加者は、研究中に強い感情的な反応を持っているか、サイバー病気関連の副作用を経験する可能性があることを期待する必要があります。.このような状況に迅速に対応する研究チームの能力は、可能な限り最善のケアを保証します。

PTSDのコア成分は、不適応性の恐怖応答2,3を阻害できないことである。扁桃体および後側帯状皮質における病的に上昇した活性は、恐怖応答を促進する領域、PTSDにおいて一貫して報告されている。これは、心室前頭前野(VMPFC)、恐怖応答3、4、5、6、7をダウン調節すると考えられている領域での活動の減少と一緒である。したがって、恐怖誘発刺激の処理中の内因性VMPFC活性の増加は、恐怖の阻害および暴露ベースの治療の有効性を改善する有望な方法であり得る。

参加者Bの生皮膚コンダクタンストレーシングの目視検査(図2)は、第1のドライブスルー(赤い四角)と第3のドライブスルー(緑色の四角)を比較する場合、セッション内の習慣化を示しているように見える。これまでの研究では、セッション内の習慣化は重要であるが、セッション間の習慣化は、PTSD33,34に対する長期暴露ベースの治療成功のより良い予測源である可能性があることを示唆している。

参加者C生皮膚コンダクタンスデータの目視検査(図3)は、参加者A(図1)に比べてあまり完全な習慣性プロファイルを示していないように見えるが、この参加者は、セッション間とセッション内の両方の習慣性を示している。さらに、参加者Aと同様に、皮膚コンダクタンスレベルは、残りの5回のセッションと比較して、最初のVRセッション中に数値的に高くなります。

参加者D(図4)の生の皮膚の導通データは、視覚的に検出可能な皮膚伝導率応答がない場合に、適切な分析のために低すぎると考えることができる皮膚の伝導レベルを示しています。そのため、これらのデータはデータ収集の失敗を表します。生データはアーティファクトの存在および電極信号損失も明らかにしているが、6回のVRセッションすべてにおいて持続的に低い皮膚伝導度および視覚的に検出可能な皮膚導電率応答の欠如は、この個体にとって明らかである。

非侵襲的な脳刺激とバーチャルリアリティの組み合わせは、心的外傷後ストレスや不安の治療を求める人々のための感情的な学習と習慣を増強するための新しいアプローチを提示します。これは、神経科学に根ざした非常に没入型でありながらポータブルなアプローチです。この技術は治療的に有効であり、個々の患者に合わせて調整することができる。この非侵襲的な方法は、心的外傷後ストレスの患者が以前に不安を引き起こす状況が安全であることを学ぶのに役立ちます。この手順を実証することは、私の研究室の研究助手であるシドニー・ブリギドです。シドニーは、プロトコルの様々なステップを支援します。経頭蓋直流刺激のために参加者を準備するために、参加者にヘッドストラップを置いた後、参加者の後ろに立って、以前に計算した頭囲の10%を使用して陰極電極の位置を確立し、頭のイニオンから右にこの距離を測定する。マスタイドプロセス上で右耳の後ろになるように測定に従って陰極電極を配置します。アノダル電極の位置を確立するには、以前に計算した頭囲の10%を、鼻上から測定し、前に計算したヘッド円周の5%を右に測定します。アノーダル電極を配置し、アノードが10~20個のEEG電極AF3FP1位置に接触していることを確認します。電極を設置したら、tDCSデバイスの電源を入れ、電極を差し込みます。設定Aをロードするには、右上ボタンを押して学習モードを終了し、左上ボタンと下ボタンを使用してデバイスのマスターコードを入力します。[OK] をクリックし、矢印がトリガーを指していることを確認します。右上ボタンを使用して、ロード設定が表示されるまで設定を移動します。左矢印を使用して、右上矢印を使用して画面の下部まで矢印をスクロールし、設定Aに戻り、左上矢印をクリックして設定Aをロードします。刺激を開始し、刺激の前、中、および後のインピーダンスを記録します。刺激が完了したら、デバイスの電源を切る前にデバイスから電極を取り外します。参加者の非支配的手のテナー領域に2つの自己接着可処分電極活性電極パッチを配置し、電極活性ガルバニック皮膚応答データ取得ソフトウェアを開く。以前に生成されたデータ取得テンプレートを開き、[レコードの作成] をクリックして新しい実験を作成します。電極活性信号を較正するには、電極を電極パッチに取り付け、ソフトウェアの指示に従って一度に1つの電極を校正します。両方の電極が較正されたら、参加者に深呼吸をして10秒間保持してから呼吸を止め、適切なガルバニック皮膚応答信号を確認してもらいます。tDCSを管理するには、アシスタントにバーチャルリアリティシステムをオンにし、患者アプリケーションプログラムを開いてください。画面の解像度が 1280 ~ 720 に設定されていることを確認し、[再生] をクリックします。臨床医のコントローラー プログラムを開き、参加者の展開に最も関連するシーンに基づいて、運転シナリオを選択します。患者アバターウィンドウで、ドライバーの位置を選択し、音量を最大65%に設定します。参加者の助けを借りて、アシスタントにヘッドマウントディスプレイを参加者の頭に置き、ディスプレイが電極を脱臼しないように注意し、快適さを確認します。アシスタントにヘッドホンを参加者の頭の上に置き、快適さを確認してもらいます。参加者に対して、ベースライン電極活性データを収集できるように2分間静かに座るように指示し、F1を押してベースライン期間の開始をマークします。2 分後に F3 キーを押して、ベースライン期間の終了をマークします。ベースライン電極活動収集が完了したら、tDCSデバイスの電源を入れ、電極を差し込みます。デバイスは、Bを設定するための学習モードである必要があります。設定Bがプログラムされていることを確認し、30秒のランプアップと30秒のランプダウンで合計25分の2ミリアンペア強度を適用するには、[OK]をクリックします。ランダム化ソフトウェアから取得した参加者固有のランダム化コードを入力し、[OK]をクリックします。左上ボタンを押して「はい」と表示し、刺激を開始し、オフをクリックしてドライブを開始します。各ドライブスルーは、VR 環境でのみ 30 秒以上の運転で開始するようにします。最初のセッションでは、先にアップなどの最初のドライブスルー中に口頭プロンプトを使用してVRイベントの発生を通して参加者を導く、3、2、1、行く道待ち伏せがあります。あなたは3、2、1で前方に待ち伏せ道路を見るつもりです。VRメニューで適切なシーンを選択しながら。各 VR イベントを、各イベント間で 10 秒以上の運転で管理します。VR イベントの管理中に、VR イベントが管理されるたびに、アシスタントがキーボードの F2 キーを押して、スキンコンダクタンス データ取得を監視するようにします。参加者Aは、皮膚のコンダクタンストレースを目視で調べると、第3回VRセッションから最終6回目のVRセッションの間に、最初のVRセッションからプロトコルの中間点までのセッション間の習慣化の兆候を示しているように見える。参加者Bの生の皮膚コンダクタンストレースの目視検査は、最初のドライブスルーと第3のドライブスルーを比較する際のセッション内の習慣を示しているように見える。参加者Cの生皮膚伝導率データの目視検査は、参加者Aと比較してあまり厳しい習慣性プロファイルを示していないように見えるが、それにもかかわらず、参加者Cはセッション内の習慣化の間および内の両方を示す。さらに、参加者Aと同様に、皮膚コンダクタンスレベルは、残りの5回のセッションと比較して、最初のVRセッション中に数値的に高くなります。参加者Dからの生の皮膚の伝導度データは、視覚的に検出可能な皮膚伝導応答がない場合に適切な分析のために低すぎると考えることができる皮膚伝導レベルを示す。アーティファクトの持続性と電極信号損失があっても、持続的に低い皮膚伝導レベルと視覚的に検出可能な皮膚伝導率応答の欠如は、この個体にはまだ明らかである。プロトコルでは、重要なステップは、tDCS電極が正しい位置に留まることを保証し、インピーダンスが許容値内にあることを確認し、参加者の安全上の不快感を監視することです。tDCSは、解剖学的MRIに基づく電極と投薬の位置を含むモンタージュを個別化することによって最適化することができます。MRIは、時間の経過に伴う神経変化を評価するためにさらに使用することができます。
