神経リハビリテーションは、感覚運動障害後の機能回復において重要な役割を果たす。神経可塑性関連機能回復のメカニズムを解明するために、機能的磁気共鳴画像法(fMRI)、陽電子放出断層撮影(PET)、脳波(EEG)、機能近赤外分光法(fNIRS)など、様々な神経イメージング技術が使用されています。異なるイメージングモダリティには、異なる長所と短所があります。fMRIは最も代表的な装置ですが、磁界の影響を受け、コストが高く、物理的に制約が高く、感覚運動タスク1、2、3、4が限られています。fNIRSデバイスは、非侵襲的な光学ニューロイメージングとして際立っており、比較的低い空間分解能を有するが、fMRI4よりも優れた時間分解能を有する。fNIRSは、介入前と介入後の効果を比較し、動的運動タスクを有し、移植性があり、fMRI1、2、4よりも自然環境においてより多く機能するため、治療効果を検証する際に適している。NIRSは、脳血管疾患、てんかん障害、重度の脳損傷、パーキンソン病、および認知障害1,5の分野においてより適していると報告されている。感覚運動タスクに関しては、歩行と立ちバランス6、7、8、上肢機能(手をつかむ、指を叩く)8、9、複雑な運動技能トレーニング10、ロボット12、13、14、15、および脳コンピュータインターフェース16、17、18に広く使用されています。fNIRSは、皮質代謝活性、血流の増加、そして結果的に皮質活性を二次シグナル19として測定する、光学神経イメージングおよび神経血管結合の原理に基づいている。fNIRSシグナルは血中酸素レベル依存性fMRI20のシグナルと強い相関を有することが報告されている。連続波fNIRSは、変更されたビール・ランバート法を使用して、広帯域近赤外光減衰21,22の測定変化に基づいて、酸素化ヘモグロビン(HbO2)および脱酸素ヘモグロビン(HHb)皮質濃度濃度の変化を決定する。連続波NIRSシステムを用いて差動長率(DPF)を測定することはできないため、DPFは一定であり、ヘモグロビン信号の変化はミリモルミリ(mM x mm)2,18の任意の単位で示されていると仮定した。
fNIRS実験では、プローブの設定、実験計画、解析方法など、最も適切な方法を選択する必要があります。プローブ設定に関しては、脳科学測定で使用される国際的な10-20法は、多くの研究者が神経イメージングで使用する設定基準です。近年、モントリオール神経研究所(MNI)座標に基づく標準脳に基づく座標設定が使用されている。実験では、一般に感覚運動タスクに使用されるブロック設計とイベント関連の設計を使用します。これは、休息中および作業中のヘモグロビン濃度の変化を比較する方法です。HbO2濃度レベルが増加し、HHb濃度レベルは、タスク依存性皮質活性に関連する脳血流の変化に伴って減少する。様々な解析方法がありますが、NIRS-SPMフリーソフトウェアはfMRIの統計的パラメトリックマッピング(SPM)と同様の分析を可能にします。NIRSデータの処理は、一般線形モデル(GLM)に基づく質量1変量法を使用します。タスク依存性脳活動解析を行う場合、fNIRS測定は、脳及び脳外コンパートメント23における呼び起こされるまたは非誘発神経活動および全身的な生理学的干渉(心拍数、血圧、呼吸数、および自律神経系活動)によって影響を受けることができる。したがって、事前解析処理、フィルタリング、ウェーブレット変換、主成分分析が有用である23.NIRS-SPMを用いたデータ処理のフィルタリングおよびアーティファクトに関しては、ローパスフィルタリング9およびウェーブレット最小記述長(Wavelet-MDL)24のトレンドまたはその他のノイズ/アーティファクト源を克服するために使用した。この分析方法の詳細については、Yeら25の報告書を参照してください。SPM のみを使用するレポートがありますが、画像解析による定性的な指標に過ぎず、NIRSの空間分解能が低いため、グループ分析には細心の注意が必要です。また、DPFが一定の場合、チャンネルと個人間の数値比較は行うべきではありませんが、各チャンネルの変化の違いを検証することができます。上記の条件に基づき、NIRS-SPMグループ解析結果を補完するために、空間登録の精度を向上させた後のマルチチャネル解析にオリジナルの解析手法を用いた。このマルチチャネル分析では、固定介入(前後)、固定期間(休止またはオンタスク)、ランダムな個々の効果を持つ階層混合モデルを使用して、処理の前後の各チャネルでの残りの期間とタスク上の期間間のHb2およびHHbレベルの変化の振幅を比較しました。

神経イメージング研究は、リハビリテーションや外科的治療などの介入前と介入後の神経学的状態の評価において極めて重要な役割を果たす。脳活動の測定に使用される多くの神経イメージング技術の中で、機能近赤外分光法(fNIRS)は、機能的磁気共鳴画像法(fMRI)と同様の局所ヘモグロビンレベルを測定することにより、動的皮質活動の評価を可能にする。また、fNIRSの物理的な制限が少ないため、複数の感覚運動タスクの変異体を評価することができます。多くの研究所は、fNIRSデータ分析のためのいくつかの方法を開発しました。しかし、一般的な原則は同じであるという事実にもかかわらず、普遍的に標準化された方法はありません。ここでは、ブロック設計を用いた多チャンネルfNIRS実験から得られたデータの定性的および比較分析方法を提示する。定性解析では、一般化線形モデルに基づく1変量系の一般手法としてNIRS用のソフトウェアを用いた。NIRS-SPM分析では、タスク中に活性化領域を可視化することで、各セッションの定性的な結果を表示します。さらに、非侵襲的な三次元デジタイザーを使用して、脳に対するfNIRSチャネル位置を推定することができる。NIRS-SPMの所見を裏付けるために、感覚運動タスクによって誘導されるヘモグロビンレベルの変化の振幅を、マルチチャネル階層混合モデルを用いて同一研究対象の2つの異なるセッション(介入前後)から得られたデータを比較することによって統計的に分析することができる。我々の方法は、運動障害、脳血管疾患、神経精神疾患などの様々な神経疾患における介入前対後の分析を測定するために使用することができる。
