Charité的肾脏移植中心照顾来自柏林和勃兰登堡大都市区的KTR。许多患者需要几个小时才能进行咨询。长旅行时间是KTR14护理中的一个重要问题，特别是对于老年人和体弱患者，以及那些必须管理家庭和正在工作的人。其他障碍是旅行费用，不便和工作时间损失15。因此，柏林肾移植中心和当地肾脏病学家（私人执业医生）在肾移植后共享护理，这引发了咨询期间信息缺失或不完整的问题。为了最大限度地减少信息丢失，需要自动安全地交换关键数据16.但是，到目前为止，数据一直存储在不同的数据孤岛中，没有互操作性。如今，数据交换依赖于数据保护有限的电话、信件、传真或电子邮件，并且高度依赖于个人。因此，信息丢失和数据不完整是常见问题，根据欧洲（EU）通用数据保护条例（GDPR）自动，安全的数据交换仍然是一个罕见的例外。
已经提出了几种电子医疗解决方案，以支持移植后的患者，以更好地利用数字化的潜力，为这个脆弱的患者群体提供医疗保健17。早期发现并发症允许远程医疗团队进行早期干预，从而减少严重的并发症，减少住院治疗或缩短住院时间，如其他远程医疗项目18，19，20，21所示。在移植人群中观察到高住院率22。每年约有三分之一的KTR住院，每次住院的平均费用约为6，600欧元。因此，远程医疗驱动的早期干预措施提供了减少住院治疗的机会，并通过这种方式降低成本和提高QoL。一个有趣的目标是提高依从性，例如，借助应用程序或远程医疗概念。由于智能手机应用程序的永久可用性，这些应用程序可以包含在旨在提高依从性的干预措施中。DeVito等人在一项随机对照试验（RCT）中证明，以用户为中心的肺移植受者应用程序具有定期自我评估，提醒功能，远程生命体征监测和自动决策支持工具可以提高对治疗的依从性。但他们没有观察到12个月住院率和死亡率的显着差异23。
Schmid等人在肾移植后进行了具有综合远程医疗概念的RCT。他们发现依从率显着提高，住院和费用显着降低20，21。这些结果得到了Lee等人的证实，他们报告说，在肝移植后的前90天内，通过智能平板电脑使用额外的远程医疗支持，再入院率明显低于标准护理19。他们的远程医疗功能包括使用蓝牙设备远程监控生命体征，药物提醒，定期自我评估，以及访问教育会议，短信和视频会议工具。在远程医疗组的患者中观察到更好的QoL，一般健康和身体功能。远程生命体征的依从性非常好（86%），但消息或视频会议的依从性仅为45%。然而，并非所有研究都能证明应用程序或电子健康解决方案的积极作用17，19。Han等人调查了一个带有药物提醒，摄入量文档和共享实验室值的应用程序，该应用程序还提供了有关免疫抑制治疗的信息。他们没有观察到KTR中干预组和对照组之间的依从性有任何显着差异，这很可能是由于高辍学率。在这个RCT中，只有47%的人在1个月后使用该应用程序24。
