胎児発育制限(FGR)や先天性心疾患(CHD)などの胎児の病状を監視するには、胎児循環の包括的な評価が必要です1,2,3。子宮内では、出生後ケアの患者管理と計画は、胎児の病状の重症度に依存します4,5,6,7。MRIによる胎児血流定量の実現可能性と胎児の病状の評価におけるその応用が最近実証されました3,8,9。しかし、イメージング法は、高い時空間分解能を達成するためのイメージング時間の増加、心臓同期法の欠如、予測不可能な胎児の動きなどの課題に直面しています10。
胎児血管系は小さな構造(下行大動脈、動脈管、上行大動脈、主肺動脈、上大静脈を構成する主要血管の直径~5mm)で構成されています11,12,13)。これらの構造を分解し、流れを定量化するためには、高い空間分解能でのイメージングが必要です。さらに、胎児の心拍数は成人の約2倍です。したがって、胎児の心周期全体の動的な心臓運動と血流を解決するには、高い時間分解能も必要です。この高い時空間分解能での従来のイメージングには、比較的長い取得時間が必要です。この問題に対処するために、加速胎児MRI14,15,16が導入されました。簡単に言うと、これらの加速技術には、データ収集中の周波数領域でのアンダーサンプリングと、反復技術を使用した遡及的な高忠実度再構成が含まれます。そのようなアプローチの1つは、圧縮センシング(CS)再構成であり、再構成された画像が既知のドメインでまばらであり、アンダーサンプリングアーティファクトがインコヒーレントである場合に、高度にアンダーサンプリングされたデータから画像を再構成することを可能にする17。
胎児イメージングにおける動きは大きな課題を提示します。運動の腐敗は、母親の呼吸運動、母親のバルク運動、または胎児の肉眼的運動から発生する可能性があります。母親の呼吸は胎児の周期的な翻訳につながりますが、胎児の動きはより複雑です。胎児の動きは、局所的または肉眼的10,18として分類できます。局所的な動きには、体のセグメントのみの動きが含まれます。それらは通常約10〜14秒間続き、その頻度は妊娠とともに増加します(正期産期では1時間あたり~90)10。これらの動きは一般に小さな破損を引き起こし、関心のあるイメージング領域には影響しません。ただし、胎児の全体的な動きは、平面運動コンポーネントを介した深刻な画像の破損につながる可能性があります。これらの動きは、脊椎によって媒介される全身の動きであり、60〜90秒間続きます。
胎児の動きによるアーティファクトを避けるために、最初に母親の動きを最小限に抑えるための措置が取られます。妊娠中の女性は、スキャナーベッドの支持枕を使用してよりリラックスし、快適なガウンを着て、閉所恐怖症を軽減するためにスキャナーの横にパートナーがいる場合があります19,20。母親の呼吸運動の影響を軽減するために、研究では母親の息止め下で胎児MR検査を実施しました21,22,23。ただし、そのような取得は、妊娠中の被験者の息止め耐性の低下を考えると、短く(~15秒)でなければなりません。.最近、胎児MRI14,15,16に対して遡及的動き補正法が導入されている。これらの方法は、登録ツールキットを使用して胎児の動きを追跡し、動きを補正するか、取得したデータの修正不可能な部分を破棄します。
最後に、出生後の心臓MR画像は、従来、心電図(ECG)ゲーティングを使用して取得され、データ収集を心周期に同期させます。ゲーティングなしでは、心周期全体からの心臓の動きと拍動の流れが組み合わされ、アーティファクトが生成されます。残念なことに、胎児心電図信号は、母体心電図信号24からの干渉および磁場25からの歪みに苦しんでいる。したがって、セルフゲーティング、メトリック最適化ゲーティング(MOG)およびドップラー超音波ゲーティング21,26,27,28を含む、胎児心臓ゲーティングに対する代替の非侵襲的アプローチが提案されている。
