1. 治疗规划

	<li>在工作站上从指定的挂钩上挂上一袋盐水：还附有一个废盐水返回袋。</li>

</ol>3. TRUS 探头准备

</ol>4. 患者准备
<ol><li>在活检的早晨，指示患者用清片库清洗和灌肠。</li>

</ol>5. 前列腺神经阻滞的管理

</ol>6. MRI-美国融合

</ol>7. 目标收购

</ol>8. 放置激光导管和温度探针
<ol><li>将 14 口径激光导管放入多通道导引的中心腔室。 注意：多通道导引弯曲以提供轻微的摩擦，从而防止治疗元素在消融过程中向后滑动。通过来回旋转来克服这种摩擦，同时将激光导管推进到前列腺。</li>
	<li>推进激光导管，直到四个回声带可视化并与屏幕上的深度标记对齐。对于外围区域消融，标记物将在前列腺胶囊外几毫米（图 2）。 注意：在纤维后胶囊的情况下，激光导管可能会偏转，而不是穿过前列腺胶囊。插入过程中，超声波上将可见任何偏转。如果发生偏转，取出激光导管并插入一个引线，如坚固的热探头，在胶囊中创建一个导引开口。然后，激光导管可以按计划推进。</li>
	<li>根据处理计划，将热探头插入激光导管的左侧或右侧。在正确的深度下，热探头与激光导管手柄联锁，使其以正确的方向对齐。 注意：放置后，检查热探头上的插槽是否位于激光导管的手柄上。热探头将磁性锁定到位，防止处理过程中的旋转。</li>

</ol>9. 执行安全检查表
<ol><li>在处理监控屏幕上，选择所需的消融站点。选择适当的消融后，按"确认选择"。安全检查表现在将占据工作站监视器的左侧。 注：此步骤将开始通过激光导管循环盐水。连接静脉输液管时引入的小气泡最初在激光周围的超声波上可见。这可以作为激光导管位置的额外检查。</li>
	<li>按照安全检查表，在完成后勾选框。</li>
	<li>确保房间里的所有人员都戴上激光安全护目镜，包括患者。</li>
	<li>工作站计算机系统将自动检查热探头，以确保所有热联体读取 30 - 40 °C 之间的均匀体温。</li>
	<li>确认激光导管的四个回波带位于前列腺胶囊，因为热探针的位置可能会将前列腺从激光导管中推开。 注：消融区开始向回声标记发出5毫米的分形，并延伸27毫米长，直径为18毫米（最大）。</li>
</ol>10. 执行激光组织消融
<ol><li>一旦安全检查表完成，请按"START LASER"开始消融。</li>
	<li>使用温度读数、超时器和损坏图实时监控治疗进度。 注：使用前列腺模型左侧的多线图评估组织温度。直肠温度以白色标记，不应超过 42 °C。 激光导管尖端的温度以蓝色标记（图3）。如果激光尖端超过 75 °C 或直肠壁超过 42°C，激光将自动关闭。 注意：激光激活后，使用屏幕顶部的红色条来监控每个消融部位的处理时间。损伤图根据温度和时间提供处理组织的 3D 表示。 注：注意B模式超声波上的更改。在激光消融过程中，前列腺组织通常外观不会发生变化。通过连续治疗，组织可能会出现低血压的外观，但美国可视化主要用于激光定位。 注：有关超声波特征的监视器如下：<ol>

</li>
	<li>激光在定时器用完后自动停止，但从业者可以通过按下"停止激光"来选择手动结束消融。盐水将继续流动，冷却激光尖端。 注意：如果温度稳定在 55 °C 以上超过 60 秒，请考虑尽早停止消融。</li>
	<li>保持激光导管和热探针到位，直到激光导管温度降至42°C以下，从而防止直肠壁在激光导管退出时加热。</li>
</ol>11. 随后的消融
<ol><li>使用融合装置提供的数字目标将超声波定位在下一个消融站点。</li>

FLA的组织效应在两项研究中得到了澄清，其中计划中的激进前列腺切除术在激光手术（N=9）后1-3周进行，即"治疗和切除"模型（图4）。15、27在全部9名患者中，前列腺中的坏死组织体积接近MRI在治疗结束后获得的体积。当整个前列腺被分割时，在激光消融区域（图1）之外，坏死和完整的细胞之间突然发生转变1-5毫米。消融区的清晰边距和精度对 MRI/美国注册和治疗规划的准确性具有重要影响。
复仇者系统的主要部分是治疗规划软件。为了有效的FL，规划不仅需要包括病变的位置，而且还需要包括完全肿瘤破坏所需的组织体积。消融量不能简单地是MRI-病变体积，因为实际肿瘤体积比MRI可见病变平均多3倍。21此外，癌症通常以不规则的手指状投影而扩展，这会使仅基于成像（例如，超出 MRI 可见病变边界 1 厘米）的任何均匀安全边际变得不可靠。Avenda 系统包括治疗规划软件，不仅利用 MRI 可见病变，还利用活检站点的 3D 跟踪（正和负），以提供完全包括癌症的最小消融量的准确放置。这种治疗规划的例子见图3。

前列腺癌（PCa）是美国男性最常见的内在恶性肿瘤。预计到2020年，将新增约19万例病例和3.3万例死亡，成为男性癌症死亡的第二大常见原因。大多数病例的PCa是可以治愈的，如果治疗，而局部前列腺。然而，通过传统的超声波（美国）成像，通常无法识别前列腺内的癌症：因此，治疗传统上涉及整个腺体的手术或放射。随着磁共振成像（MRI）的引入，"全腺"模式已经改变，与美国相比，它实现了PCa的本地化和有针对性的活检3、4、5、6。虽然MRI可能低估了PCa7的多焦度，并可能错过小病变8，它可以可靠地识别指数病变，这几乎总是转移性疾病9，10的驱动因素。

focal laser ablation of prostate cancer: an office procedure

FLA 公布的结果显示在表2 中。包括各种方法和技术。在 SEER 数据库中发现超过 400 名接受过各种形式的 FLA 治疗的 PCa 患者。25为了量化文献中报告的 FLA 的数量和特征，我们对梅德琳和科克伦图书馆进行了系统回顾。我们的搜索使用全场搜索词执行，包括"聚焦激光消融"和"前列腺癌"。总共审查了247个标题和摘要。仅包括报告焦激光消融、MRI 和肿瘤结果的病例。13份符合列入条件的同行评审出版物，占患者总数的333份（表1）。
除2项研究之外，所有研究均使用980纳米二极管激光进行治疗。26，27 处理参数包括功率水平在 6 - 18 瓦之间 ， 处理时间跨越 1 - 4 分钟每个消融点 （表 1）。治疗温度监测由MRI热计学在9项研究中提供，直接温度探针测量在3项研究 （表1）。除林德纳和纳塔拉詹后来的研究外，所有研究都是在孔内进行的。20，26，27
该队列的基线 PSA 中位数为 5.7（范围 1.1 - 14.8）。继FL之后，PSA在3个月、6个月、12个月和24个月的中位数分别为3.9、5.5、3.8和3.9。该队列的基线IPS中位数为6。继 FLA 之后，IPSS 在 3 个月、6 个月、12 个月和 24 个月的中位数分别为 5 个月、5.5 个月、7.3 个月和 11.5 个月。该队列的基线 SHIM 中位数为 20。继 FLA 之后，SHIM 在 3 个月、6 个月、12 个月和 24 个月的中位数分别为 19 个月、18 个月、20 个月和 19 个月。
在所有研究中，并发症的报告不一致：然而，只有一个并发症被作者归类为三级（尿路感染）。16 作者没有具体说明将其归类为三级事件的功能。两个直肠-尿道管，在长时间导管化后都自发关闭，16 个报告为II级不良事件。
肿瘤随访按随访期分组：少于6个月、1年和2年（表2）。后续活检由MRI指导的孔内活检在4项研究中进行，MRI-US融合活检在6项研究中进行。两项研究采用系统活检，两项研究进行了前列腺切除标本评估的"治疗和切除"调查。治疗成功是根据德尔福共识协议确定的。28场内成功被定义为在之前的消融站点内没有≥ GG2 PCa。场外故障被定义为在先前消融区域之外的≥ GG2。在后续活检结果患者中，治疗6个月（N=83）、1年（N=64）和2年（N=39）的现场成功率分别为83%、83%和59%（表2）。
在加州大学洛杉矶分校，从2014年开始，前列腺的FL已经连续进行了三次临床试验。29-31 18名患有中度风险前列腺癌的男性接受了FL治疗，8名在孔内和10名在诊所接受过手术，没有任何三级不良事件。目前，另有10名男子正在使用演示装置进行FL检查。31 所有患者在 FLA 之前接受 3T MRI（身体线圈）和 MRI-US 融合活检的评估，并在治疗后 6 个月内从 ROI 和系统活检中取样。基线和后续活检都是在MRI/美国融合指导下使用阿特米斯融合系统对所有活检部位进行跟踪的。
