このプロトコルは、機能TCD実験の基礎となるのは、ほとんどの機能TCD実験では、長期間にわたって安定した信号を記録するために固定デバイスの配置が必要となるからです。機能TCDの主な利点は、脳血流の変化の高い時間分解能測定である。TCDを使用して中大脳動脈を見つけることは練習を取る.トランスデューサを安定させ、非常にゆっくりと動かすることが重要です。トランスデューサの位置と方向の小さな調整を行うために必要な細かいモーター制御は開発に時間がかかります。あなたが見つけることができる限り多くのボランティアに練習してください。視覚的なデモンストレーションは、2つの理由から重要です。まず、トランスデューサを配置する場所を正確に確認すると便利です。第二に、機能TCDを学習する上で重要な部分は、異なる動脈に関連する音を学習することです。経頭蓋ドップラー超音波またはTCDのパラメータを設定して開始します。中大脳動脈またはMCAの最初の探索中に、電力を適度に高い値に保ちます。MCA信号が見つかったら、良好な信号を維持しながら、可能な限り電力を下げます。MCA 信号の最初の検索時に、サンプルの体積を 8 ~ 12 ミリメートルに設定します。信号を見つけるのが難しい場合は、ゲートサイズを大きくして信号の強度を高めます。バックグラウンドノイズを最小限に抑える目標を掲げ、ゲインを中程度のレベルに設定します。ハイパスフィルターカットオフを50~150ヘルツの間に設定します。被験者が成人の場合は、深さを 50 ミリメートルに設定します。トランスデューサの表面を覆うのに十分な超音波ゲルを塗布してください。ゲルが冷たく感じるかもしれないことを被験者に警告し、その後、経時的な窓にトランスデューサーを置きます。頭皮にトランスデューサを配置した後、一般的に最初のトランスデューサ配置の位置からわずかに前部とロストラルに位置するMCA信号を検索します。TCDスペクトル信号がすぐに明らかでない場合は、頭皮に対して同じ位置に保ちながらトランスデューサの角度を調整します。プローブをロストラルから尾角、後部から前部にゆっくりと角度を付けます。信号がまだ存在しない場合は、カラー M モードディスプレイの MCA のフローを確認します。信号深さを5ミリのステップで増減し、検索を続けます。フローが M モードで表示され、ドップラースペクトルでは表示されない場合は、ドップラースペクトルにフロー信号が表示されるまで深さを増減します。それでも満足のいく信号が得られない場合は、トランスデューサを頭皮の少し前の位置に移動し、検索を繰り返します。最適な MCA 信号が得られたら、深さと最大速度に注意してください。洗えるメイクペンを使って、最適な信号が見つかった頭皮にマークを付けます。次に、分岐を検索します。内部頸動脈の分岐から中大脳動脈および前大脳動脈への分岐からの信号が注目されるまでの深さを増し、典型的には51〜65ミリメートルの深さで。最適な分岐スペクトル信号を検索し、可能な限り最高速度のスペクトル信号を求めます。最適な分岐信号が得られたら、分岐の深さに注意してください。固定装置を被験者のおおよそのヘッドサイズに合わせて調整します。ヘッドセットを頭に置く前に、被写体に警告します。ヘッドセットを装着した後、固定デバイスのフィット感を調整します。そして、デバイスがきつすぎるかどうかを被験者に尋ねます。トランスデューサが自由に動くことができるように固定装置の機構を緩めます。トランスデューサの顔を覆うのに十分な超音波ゲルをトランスデューサに塗布してください。トランスデューサが以前に作成されたマークの上に配置されるように固定デバイスを調整します。最適な MCA スペクトル信号を検索し、可能な限り最高速度のスペクトル信号を探します。最適なMCA信号が見つかったら、固定装置の機構を締めてトランスデューサを所定の位置にロックします。深さやその他すべての設定に注意してください。最大速度を正確にトレースするスペクトルエンベロープを維持しながら、電力をできるだけ低くします。TCD ソフトウェアでの録音を開始します。良好なベースライン記録を達成し、脳血流速度が以前の実験や刺激から安定するように、被験者に3分間正常に呼吸するように指示する。3からゆっくりとカウントダウン。1のカウントで、通常のインスピレーションの後に息を止め始めるように被験者に依頼します。TCD記録にマーカーを置いて、息を止める開始を示します。被験者に30秒間息を止めさせるか、息を止めるのが快適でなくなるまで呼吸を続けてください。被検者が吸い込むとき、TCD記録にマーカーを置いて、息の終わりを示す。呼吸終了後少なくとも30秒間、TCDを使用して脳血流速度をモニタリングし続け、速度がベースライン値に戻ることを確認します。中大脳動脈またはMCAのM1セグメントの中間点からのドップラースペクトルおよび色Mモードを以下に示す。スペクトルは頭皮上の同じ位置で撮影されましたが、異なる角度で撮影されました。角度の非常に小さな変化はドップラー信号強度を大幅に向上させることができることに注意することが重要です。ICAの分岐からACAとMCAへの単純なドップラースペクトルとMモードを示します。重複する赤と青のシェーディング領域と M モードイメージは、それぞれ MCA と ACA を示します。ドップラースペクトルとMモード画像は、呼吸保持操縦の異なる時点で撮影されます。ベースラインの平均血流速度は毎秒56センチメートルで、呼吸の終わりまでに毎秒70センチメートルに増加し、呼吸保持が終了した後、毎秒47センチメートルにアンダーシューティングされた。息を止める実験全体をここに示します。白線で示されるエンベロープは、呼吸保持の間に徐々に増加し、呼吸保持終了後約15秒間上昇し、約20秒間アンダーシュートを行い、ベースライン値に回復する。二国間FTCDに適した二国間TCDスペクトルおよびMモードのいくつかの例を、ここに示す。MCA 信号を見つけることは、非常に制御された、細かい運動を伴うことを覚えておくことが重要です。MCAを見つけるのが習熟する唯一の方法は、できるだけ多くの異なる人々に練習することです。この技術は、研究者が以前はアクセスできない環境で脳活動を測定する道を開いた。例えば、MRI内とは異なり、TCD固定装置を装着した被験者は自由に動くことができるので、アクティブなタスク中の脳の横化を研究することができます。
