Cardiale anatomie is van oudsher bestudeerd met behulp van dunne histologische secties11. Aritmieën zijn veel voorkomende ziekten die miljoenen mensen treffen en zijn de oorzaak van aanzienlijke morbiditeit en mortaliteit wereldwijd. Ondanks enorme vooruitgang in behandeling en preventie, zoals de ontwikkeling van pacemakers, blijft de behandeling van aritmieën een uitdaging, voornamelijk vanwege de zeer beperkte kennis over onderliggende ziektemechanismen 1,2,3. Een beter begrip van zowel de normale elektrofysiologie als de pathofysiologie van aritmieën kan helpen om in de toekomst nieuwe, innovatieve en causale behandelingsstrategieën te ontwikkelen. Om aritmogenese uitgebreid te bestuderen, is het bovendien belangrijk om het specifieke hartgeleidingssysteem in diermodellen zoals de muis te lokaliseren en te visualiseren, omdat muizen veel worden gebruikt in elektrofysiologisch onderzoek.
De belangrijkste onderdelen van het hartgeleidingssysteem zijn de sinoatriale knoop (SAN), waar de elektrische impuls wordt gegenereerd in gespecialiseerde pacemakercellen, en de atrioventriculaire knoop (AVN), de enige elektrische verbinding tussen de boezems en de ventrikels4. Wanneer de elektrofysiologische eigenschappen van SAN en AVN worden gewijzigd, kunnen aritmieën zoals sick sinus syndroom of atrioventriculaire blok optreden die kunnen leiden tot hemodynamische achteruitgang, syncope en zelfs de dood, en zo de essentiële rol van zowel SAN als AVN in elektrofysiologie en aritmogenese onderstrepen5.
Uitgebreide studies over SAN of AVN vereisen een nauwkeurige lokalisatie en visualisatie van beide structuren, idealiter binnen hun fysiologische omgeving. Vanwege hun kleine omvang en locatie binnen het werkende myocardium, zonder een duidelijke macroscopisch zichtbare structuur vast te stellen, is het bestuderen van de anatomie en elektrofysiologie van SAN en AVN echter een uitdaging. Anatomische oriëntatiepunten kunnen worden gebruikt om ruwweg het gebied te identificeren dat SAN en AVN 6,7,8 bevat. Kortom, SAN bevindt zich in het inter-cavalgebied van het rechter atrium naast de musculaire crista terminalis (CT), AVN bevindt zich in de driehoek van Koch die wordt gevormd door de tricuspidalisklep, het ostium van de coronaire sinus en de pees van Todaro. Tot nu toe werden deze anatomische oriëntatiepunten voornamelijk gebruikt om SAN en AVN als individuele structuren te lokaliseren, te verwijderen en vervolgens te bestuderen (bijvoorbeeld door conventionele histologie). Om de complexe elektrofysiologie van SAN en AVN (bijv. Regulerende effecten van aangrenzende cellen van het werkende myocardium) beter te begrijpen, is het echter noodzakelijk om de geleidingssystemen binnen de fysiologische 3D-omgeving te bestuderen.
Immunofluorescentiekleuring met volledige mount is een methode die wordt gebruikt om anatomische structuren in situ te bestuderen met behoud van de integriteit van het omliggende weefsel9. Gebruikmakend van confocale microscopie- en beeldanalysesoftware, kunnen SAN en AVN worden gevisualiseerd met fluorescerend gelabelde antilichamen die gericht zijn op ionkanalen die specifiek in deze regio's tot expressie komen.
In dit volgende protocol worden de noodzakelijke stappen uitgelegd om een gevestigde kleuringsmethode voor de hele montage uit te voeren voor SAN- en AVN-microscooplokalisatie en -visualisatie. In het bijzonder beschrijft dit protocol hoe (1) SAN en AVN moeten worden gelokaliseerd door anatomische oriëntatiepunten om deze monsters voor te bereiden op kleuring en microscopieanalyse (2) om immunofluorescentiekleuring van de referentiemarkers HCN4 en Cx43 uit te voeren (3) om SAN- en AVN-monsters voor te bereiden voor confocale microscopie (4) om confocale beeldvorming van SAN en AVN uit te voeren. We beschrijven ook hoe dit protocol kan worden aangepast om extra kleuring van omringend werkend myocardium of niet-myocytencellen zoals autonome zenuwvezels op te nemen, wat een grondig onderzoek van het hartgeleidingssysteem in het hart mogelijk maakt.

Het elektrische signaal dat fysiologisch wordt gegenereerd door pacemakercellen in de sinoatriale knoop (SAN) wordt geleid door het geleidingssysteem, dat de atrioventriculaire knoop (AVN) omvat, om excitatie en contractie van het hele hart mogelijk te maken. Elke disfunctie van SAN of AVN resulteert in aritmieën, wat wijst op hun fundamentele rol in elektrofysiologie en aritmogenese. Muismodellen worden veel gebruikt in aritmieonderzoek, maar het specifieke onderzoek naar SAN en AVN blijft een uitdaging.
Het SAN bevindt zich op de kruising van de crista terminalis met de superieure vena cava en AVN bevindt zich aan de top van de driehoek van Koch, gevormd door de opening van de coronaire sinus, de tricuspidalis annulus en de pees van Todaro. Vanwege het kleine formaat blijft visualisatie door conventionele histologie echter een uitdaging en het maakt de studie van SAN en AVN binnen hun 3D-omgeving niet mogelijk.
Hier beschrijven we een immunofluorescentiebenadering met volledige mount die de lokale visualisatie van gelabelde muis SAN en AVN mogelijk maakt. Whole-mount immunofluorescentiekleuring is bedoeld voor kleinere delen van weefsel zonder de noodzaak van handmatige secties. Daartoe wordt het muizenhart ontleed, waarbij ongewenst weefsel wordt verwijderd, gevolgd door fixatie, permeabilisatie en blokkering. Cellen van het geleidingssysteem binnen SAN en AVN worden vervolgens gekleurd met een anti-HCN4-antilichaam. Confocale laserscanmicroscopie en beeldverwerking maken differentiatie mogelijk tussen knooppuntcellen en werkende cardiomyocyten en kunnen SAN en AVN duidelijk lokaliseren. Bovendien kunnen extra antilichamen worden gecombineerd om ook andere celtypen te labelen, zoals zenuwvezels.
In vergelijking met conventionele immunohistologie behoudt immunofluorescentiekleuring van de hele montering de anatomische integriteit van het hartgeleidingssysteem, waardoor het onderzoek van AVN mogelijk is; vooral in hun anatomie en interacties met de omringende werkende myocardium en niet-myocytencellen.

Door gebruik te maken van het hierboven beschreven protocol kan confocale microscopie beeldvorming van zowel SAN als AVN betrouwbaar worden uitgevoerd. Specifieke kleuring van het geleidingssysteem met behulp van fluorescerende antilichamen gericht op HCN4 en kleuring van het werkende myocardium met behulp van fluorescerende antilichamen gericht op Cx43 maakt de duidelijke identificatie van SAN (figuur 5, video 1) en AVN (<strong class="xfig"...
