Hjerteanatomi har tradisjonelt vært studert ved hjelp av tynne histologiske snitt11. Arytmier er vanlige sykdommer som rammer millioner av mennesker, og er årsaken til betydelig sykelighet og dødelighet over hele verden. Til tross for enorme fremskritt innen behandling og forebygging, som utvikling av hjertepacemakere, er behandling av arytmier fortsatt utfordrende, først og fremst på grunn av svært begrenset kunnskap om underliggende sykdomsmekanismer 1,2,3. En bedre forståelse av både normal elektrofysiologi og patofysiologi av arytmier kan bidra til å utvikle nye, innovative og kausale behandlingsstrategier i fremtiden. I tillegg, for å studere arytmogenese grundig, er det viktig å lokalisere og visualisere det spesifikke hjerteledningssystemet i dyremodeller som musen, da mus er mye brukt i elektrofysiologiforskning.
De viktigste delene av hjerteledningssystemet er sinoatrialknuten (SAN), hvor den elektriske impulsen genereres i spesialiserte pacemakerceller, og atrioventrikulær knute (AVN), som er den eneste elektriske forbindelsen mellom atriene og ventriklene4. Når de elektrofysiologiske egenskapene til SAN og AVN endres, kan arytmier som syk sinus syndrom eller atrioventrikulært blokk forekomme som kan føre til hemodynamisk forverring, synkope og til og med død, og dermed understreke den essensielle rollen til både SAN og AVN i elektrofysiologi og arytmogenese5.
Omfattende studier på SAN eller AVN krever en presis lokalisering og visualisering av begge strukturer, ideelt innenfor deres fysiologiske miljø. På grunn av deres lille størrelse og plassering i arbeidsmyokardiet, uten å etablere en klar makroskopisk synlig struktur, er det imidlertid utfordrende å studere anatomien og elektrofysiologien til SAN og AVN. Anatomiske landemerker kan brukes til å grovt identifisere regionen som inneholder SAN og AVN 6,7,8. Kort fortalt ligger SAN i interkavalområdet i høyre atrium ved siden av muskelcrista terminalis (CT), AVN ligger innenfor trekanten av Koch etablert av trikuspidalklaffen, ostium av koronar sinus og senen til Todaro. Så langt ble disse anatomiske landemerkene hovedsakelig brukt til å lokalisere, fjerne og deretter studere SAN og AVN som individuelle strukturer (f.eks. Ved konvensjonell histologi). For bedre å forstå den komplekse elektrofysiologien til SAN og AVN (f.eks. regulatoriske effekter av tilstøtende celler i arbeidsmyokardiet), er det imidlertid nødvendig å studere ledningssystemene i det fysiologiske 3D-miljøet.
Helmontert immunfluorescensfarging er en metode som brukes til å studere anatomiske strukturer in situ samtidig som integriteten til det omkringliggende vevet 9 opprettholdes. Ved å dra nytte av konfokalmikroskopi og bildeanalyseprogramvare, kan SAN og AVN visualiseres med fluorescerende merkede antistoffer rettet mot ionkanaler spesielt uttrykt i disse regionene.
Denne følgende protokollen forklarer de nødvendige trinnene for å utføre en veletablert helmontert fargemetode for lokalisering og visualisering av SAN- og AVN-mikroskop. Spesifikt beskriver denne protokollen hvordan (1) å lokalisere SAN og AVN ved anatomiske landemerker for å forberede disse prøvene for farging og mikroskopianalyse (2) for å utføre helmontert immunfluorescensfarging av referansemarkørene HCN4 og Cx43 (3) for å forberede SAN- og AVN-prøver for konfokalmikroskopi (4) for å utføre konfokal avbildning av SAN og AVN. whole-mount immunofluorescence staining, confocal imaging and 3d reconstruction of the sinoatrial and atrioventricular node in the mouse

Det elektriske signalet fysiologisk generert av pacemakerceller i sinoatrialknuten (SAN) utføres gjennom ledningssystemet, som inkluderer atrioventrikulær knutepunkt (AVN), for å tillate eksitasjon og sammentrekning av hele hjertet. Enhver dysfunksjon av enten SAN eller AVN resulterer i arytmier, noe som indikerer deres grunnleggende rolle i elektrofysiologi og arytmogenese. Musemodeller er mye brukt i arytmiforskning, men den spesifikke undersøkelsen av SAN og AVN er fortsatt utfordrende.
SAN ligger ved krysset mellom crista terminalis med den overlegne vena cava og AVN ligger ved toppen av trekanten Koch, dannet av åpningen av koronar sinus, tricuspid annulus og senen til Todaro. På grunn av den lille størrelsen er visualisering ved konvensjonell histologi imidlertid fortsatt utfordrende, og det tillater ikke studier av SAN og AVN i deres 3D-miljø.
Her beskriver vi en helmontert immunfluorescenstilnærming som tillater lokal visualisering av merket mus SAN og AVN. Helmontert immunfluorescensfarging er beregnet for mindre seksjoner av vev uten behov for manuell seksjonering. Til dette formål dissekeres musehjertet, med uønsket vev fjernet, etterfulgt av fiksering, permeabilisering og blokkering. Celler i ledningssystemet i SAN og AVN blir deretter farget med et anti-HCN4 antistoff. Konfokal laserskanningsmikroskopi og bildebehandling tillater differensiering mellom nodalceller og arbeidende kardiomyocytter, og å tydelig lokalisere SAN og AVN. Videre kan ytterligere antistoffer kombineres for å merke andre celletyper også, for eksempel nervefibre.
Sammenlignet med konvensjonell immunohistologi bevarer helmontert immunfluorescensfarging den anatomiske integriteten til hjerteledningssystemet, og tillater dermed undersøkelse av AVN; Spesielt så inn i deres anatomi og interaksjoner med det omkringliggende arbeidsmyokardiet og ikke-myocyttceller.

Ved å bruke protokollen som er skissert ovenfor, kan konfokalmikroskopiavbildning av både SAN og AVN utføres pålitelig. Spesifikk farging av ledningssystemet ved bruk av fluorescerende antistoffer rettet mot HCN4 og farging av arbeidsmyokard ved bruk av fluorescerende antistoffer rettet mot Cx43 tillater klar identifisering av SAN (figur 5, video 1) og AVN (figur 6, video 2) innenfor intakt anatomi.</...
