שיטת השתלת האיים המכוסים בשומן משלבת טכניקות משתי טכניקות השתלה שונות: השתלת איון תוך-צפקי והשתלת איון רקמת שומן. מכיוון שקרום פני השטח של רקמת השומן הלבן האפידידימלי נחשב לרקמת השומן הלבנה המכוסה על ידי הצפק ומחוברת לאפידידימיס, ניתן לסווג אנטומית את שיטת השתלת האיים המכוסים בשומן כסוג ?...

השתלת איים היא טיפול תחליפי תאים לחולים עם סוכרת קשה. דיווחים אחרונים הראו כי שיעורי עצמאות האינסולין בשלוש שנים לאחר ההשתלה משתפרים עד 44%1 וכי כ -80% מהמושתלים המקבלים יותר מ -600,000 שווה ערך לאיים בסך הכל משיגים עצמאות אינסולין2. יתר על כן, בדו"ח האחרון של מרשם השתלות האיים השיתופיים, התגלה כי רמות הגלוקוז בדם בצום נשמרו על 60-140 מ"ג/דצ"ל במשך תקופה של 5 שנים ביותר מ-70% מהחולים שעברו השתלת איון בלבד. המחקר גם קבע כי כ-90% מהחולים שקיבלו השתלת איון בלבד או השתלת איון לאחר השתלת כליה לא פיתחו אירועי היפוגליקמיה חמורים במשך למעלה מ-5 שנים3.

לעומת זאת, השימוש ברקמת שומן לבנה תוך-צפקית בשקית האומנטלית8, במזנטריה ובמיקומים חוץ-כבדיים אחרים דווח היטב 10,20,21,22,23 ורבים מהמחקרים שבחנו את תפקודם של איים מהונדסים ביולוגית שהושתלו באמצעות רקמת שומן לבנה הצליחו לדווח על תוצאות טיפוליות מבטיחות20,24,25, 26. מכיוון שהשימוש ברקמת שומן אפידידימלית מקל על איתור איים מושתלים, פותחה "שיטת השתלת האיים המכוסים בשומן", תוך שימוש ברקמת שומן אפידידימלית, כדי להתגבר על המגבלות של השתלת איון תוך-צפקי. במאמר זה מתוארת השתלת איון מכוסה שומן באמצעות רקמת שומן אפידידימלית.
