膵島移植は、重度の糖尿病患者に対する細胞補充療法です。最近の報告によると、移植後3年後のインスリン非依存性の割合は最大44%1に改善し、 合計60万を超える膵島を投与されたレシピエントの約80%がインスリン非依存性を達成しています2。さらに、最新の共同膵島移植レジストリレポートでは、膵島移植のみを受けた患者の70%以上で、空腹時血糖値が5年間にわたって60〜140 mg / dLに維持されていることが明らかになりました。この研究では、膵島移植単独または腎臓移植後に膵島移植を受けた患者の約90%が、5年以上にわたって重度の低血糖イベントを発症しなかったことも判明しました3。
この治療の臨床成績は改善していますが、最適な移植部位を確立する必要性など、いくつかの制限に対処する必要があります。肝臓は、大量の膵島を収容できる最大の臓器であるため、臨床膵島移植の典型的な移植部位です。ただし、一部の患者では、肝臓が利用できないため(例:.、門脈圧亢進症、肝炎、および/または肝硬変4)、したがって、腎嚢下腔5,6、大網袋7,8,9,10、腸間膜11、胃腸管12、骨格筋13、皮下組織13、骨髄14、および脾臓15を含む他の部位,16,17は、代替移植部位として考えられている。
局所麻酔下で腹腔内移植が容易に行えるため、腹腔内腔は臨床膵島移植の魅力部位であるが、移植時には膵島が腹腔全体に分散するため、膵島生着の検出や生着確認の成功が困難である。したがって、腹腔内腔は理想的な臨床移植部位として広く認識されていません。代わりに、移植されたカプセル化された18 およびバイオエンジニアリングされた膵島19の有効性を調査するための前臨床試験の対照モデルとして頻繁に利用される。しかし、バイオエンジニアリングされた膵島と対照膵島の正確な比較は、正確な生着評価を行う上での課題のために達成することは困難です。
