心原性ショックは、重度の心機能障害による末端臓器灌流の減少を特徴とする臨床状態です。心原性ショックの最も広く受け入れられている定義は、心原性ショック試験(SHOCK)1 および心原性ショックを伴う心筋梗塞のための大動脈内バルーンサポート(IABP-SHOCK-II)試験2 に基づいており、次のパラメーターが含まれています。
1.収縮期血圧<90mmHg≥30分間、または昇圧剤および/またはSBPを維持するための機械的サポート≥90mmHg
2.終末臓器低灌流の証拠(尿量<30 mL / hまたは冷たい四肢)
3.血行動態基準:心臓指数≤2.2 L / min / m2 および肺毛細血管ウェッジ圧≥15mmHg
急性心筋梗塞(AMI)は心原性ショックの最も一般的な原因であり、症例の約30%を占めています3。早期侵襲的血行再建術を伴うAMI患者の治療の進歩にもかかわらず、心原性ショックの死亡率は依然として高いままです4。冠動脈灌流の改善と左心室作業の減少を示す拡張期増強のメカニズムは、1958年に最初に実証されました5。その後、1962年にIABPの最初の実験プロトタイプが開発されました6。6年後、Kantrowitzら7 は、内科的治療に反応しないAMIおよび心原性ショックの4人の患者におけるIABP使用の最初の臨床経験を発表しました。
IABPの作用機序には、拡張期中のバルーンの膨張と収縮期中のデフレが含まれます。これは2つの重要な血行動態の結果をもたらします:バルーンが拡張期で膨張すると、大動脈内の血液は大動脈根に向かって近位に移動し、それによって冠状動脈血流が増加します。バルーンが収縮期に収縮すると、真空または吸引効果を引き起こし、後負荷を減らし、心拍出量を増大させます8。IABPによって引き起こされる血行力学的変化を以下に示します9(表1):

IABPの主な適応症は、心原性ショック(AMIおよび虚血性および非虚血性心筋症、心筋炎などの他の原因による)、心室中隔欠損症または重度の僧帽弁閉鎖不全症などのAMIの機械的合併症、高リスク経皮的冠動脈インターベンション中の機械的サポート11、重要なCAD患者の冠状動脈バイパス手術への架け橋として、心肺バイパスから離乳できないこと、および決定または次のような高度な治療法への架け橋として 左心室補助装置(LVAD)または末期心不全における心臓移植12,13,14,15。IABPの使用に対する禁忌には、反脈動で悪化する可能性のある中等度または重度の大動脈閉鎖不全症、デバイスの最適な動脈アクセスと配置を妨げる重度の末梢血管疾患、および解離12,15などの大動脈病変が含まれます。
IABPデバイスは、ユニットを制御するためのコンソールとバルーンを備えた血管カテーテルで構成されています。
コンソールには、次の 4 つのコンポーネントが含まれています。
a)バルーンのトリガー信号を処理および決定するのに役立つモニターユニット。信号は、心電図(ECG)トリガーまたは圧力信号トリガーのいずれかです。
b)コントロールユニット:トリガー信号を処理し、ガスバルブを作動させてインフレまたはデフレを支援します。
c)ヘリウムを含むガスボンベ。二酸化炭素は代替手段ですが、ヘリウムよりも好ましくありません。ヘリウムは密度が低く、より急速な膨張とデフレでより良いバルーン膨張特性を提供します16。
d)ガス供給を支援するバルブユニット。
IABP(バルーン)カテーテルは、距離マーキング付きの7-8.5F血管カテーテルです。カテーテルは先端にポリエチレンバルーンが取り付けられています。バルーンサイズは20〜50mLの範囲で変化し得る。理想的なバルーンは、左鎖骨下動脈から腹腔動脈の離陸までをカバーする長さを持ち、膨張した直径は下行大動脈の直径の90〜95%です。成人患者で最も一般的に使用されるバルーンサイズ(高さ5'4 "/ 162 cmから6'/ 182 cm)は40mLです。50 mLバルーンは、身長5'/152 cmから5'4"/162 cmの患者12,17の患者>6'/182cmおよび34cmのバルーンに使用されます(表2)。
