Hoewel laparoscopische en robotondersteunde technieken op grote schaal zijn toegepast voor nefrectomie van levende donoren, worden niertransplantaties nog steeds voornamelijk uitgevoerd met behulp van conventionele open technieken. De laatste tijd wordt echter steeds vaker een minimaal invasieve aanpak voor niertransplantatie toegepast. In vergelijking met traditionele open chirurgie heeft minimaal invasieve niertransplantatie een lager risico op chirurgische site-infectie, incisiehernia en wonddehiscentie, evenals korte...

Niertransplantatie draagt bij aan een langere overleving en een betere kwaliteit van leven in vergelijking met peritoneale dialyse of hemodialyse1. Hoewel de open benadering de standaardprocedure is voor niertransplantatie, zijn robotondersteunde technieken onlangs aangenomen 2,3,4. Specifiek heeft robot-assisted kidney transplantation (RAKT) verschillende voordelen ten opzichte van open niertransplantatie: minimale postoperatieve pijn, betere cosmesis, minder wondinfecties en korter verblijf in het ziekenhuis5. Bovendien stellen minimaal invasieve toegang en robottechnologie chirurgen in staat om veilig niertransplantaties uit te voeren bij morbide zwaarlijvige patiënten 6,7,8,9. Vanwege de complexiteit vereist RAKT echter een leercurve om voldoende reproduceerbaarheid te bereiken in de bedrijfstijd, functionele resultaten en veiligheid10.
Allografts met meerdere bloedvaten vereisen meestal vasculaire reconstructie, wat leidt tot langdurige koude en warme ischemische tijden. Ondanks de technische uitdagingen van RAKT, meldde een Europese multicenter studie dat RAKT het gebruik van allografts met meerdere vaten technisch haalbaar is en leidt tot gunstige functionele resultaten11. Hoewel het vaker voorkomt om het nierallenitret mediaal in het bekken te plaatsen tijdens vasculaire anastomose, werd volgens eerdere rapporten 4,5,6,7,8,9, het allograft op de peritoneale pouch lateraal aan de iliacale vaten in dit protocol geplaatst. Hoewel het veilig kan zijn om een allograft mediaal te plaatsen tijdens anastomose en het om te draaien naar de peritoneale buidel, is deze techniek misschien niet bekend bij onervaren chirurgen. Bovendien is het handiger om vasculaire anastomose uit te voeren met het allograft in de peritoneale buidel en niervaten in de juiste positie. robot-assisted kidney transplantation

Dit artikel beschrijft robot-assisted kidney transplantation (RAKT) van een levende donor. De robot wordt gecoupeerd tussen de gescheiden benen van de patiënt, geplaatst in de liggende Trendelenburg-positie. Nier allografts worden geleverd door een levende donor. Vóór vasculaire anastomose wordt het nierallenit bereid door een dubbele J-stent in de urineleider in te brengen en de temperatuur voor de anastomose wordt verlaagd door deze in een ijspakt gaas te wikkelen. Een 12 mm of 8 mm poort voor de robotcamera en drie 8 mm poorten voor robotarmen zijn geplaatst. Een peritoneale pouch wordt gemaakt voor het nier allograft door de peritoneale flappen aan beide zijden over de psoas-spier te verhogen voordat de iliacale vaten en blaas worden ontleed. Een Pfannenstiel-incisie van 6 cm wordt gemaakt om de nier in de peritoneale buidel in te brengen, lateraal naar de rechter iliacale vaten. 
Na het klemmen van de externe iliacale ader met Bulldogs-klemmen, wordt een venotomie uitgevoerd en wordt de nierader van het transplantaat op een end-to-side continue manier naar de externe iliacale ader geanostomoseerd met een 6/0 polytetrafluorethyleen hechting. Na het klemmen van de nierader van het transplantaat wordt de iliacale ader gedemplampeerd. Dit wordt gevolgd door het klemmen van de uitwendige iliacale slagader, arteriotomie, arteriële anastomose met een 6/0 polytetrafluorethyleen hechting, klemmen van de nierslagader van het transplantaat en het ontlichten van de uitwendige iliacale slagader. Reperfusie wordt vervolgens uitgevoerd en ureteroneocystostomie wordt uitgevoerd met behulp van de Lich-Gregoir-techniek. Het peritoneum wordt op enkele locaties afgesloten met polymeer vergrendelingsclips en een gesloten zuigafvoer wordt door een van de werkpoorten geplaatst. Na het leeglopen van het pneumoperitoneum zijn alle incisies gesloten.

We hebben een routinematig klinisch traject opgezet voor ontvangers die RAKT in ons centrum hebben. Renale Doppler echografie wordt uitgevoerd een dag na de transplantatie en technetium-99m diethyleentriamine penta-azijnzuur nierscan twee dagen na de transplantatie. In dit protocol hebben we stapsgewijze procedures voorgesteld voor robotondersteunde niertransplantatie zonder om te draaien. Het is natuurlijker om vasculaire anastomose uit te voeren met een allograft in de peritoneale buidel en de niervaten in de juiste positie. Deze techniek kan worden toegepast op andere vaste orgaantransplantaties, zoals de lever en de alvleesklier.We hopen het nut van minimaal invasieve chirurgische benaderingen met behulp van het robotsysteem op het gebied van transplantatie uit te breiden. Robotondersteunde niertransplantatie kan alleen worden bestudeerd na enige opleiding over het robotsysteem. Het is beter om hulp te krijgen van een ervaren chirurg.Voordat u met de procedure begint, bereidt u de robotarmen voor met de chirurgische instrumenten om intra-abdominale dissectie uit te voeren. Plaats monopolaire gebogen scharen in arm twee, fenestrated bipolaire tangen in arm drie en ProGrasp-tangen in arm vier. Start de operatieve procedure door een incisie te maken in het peritoneum langs de rechter paracolische goot om een zakje voor het nierallograft te maken.Na de incisie ontleedt u de rechter interne iliacale vaten over hun gehele lengte en omringt u elk vat met een vaatlus. Voor ureteroneocystostomie, ontleed de blaas in de rechterhoek en scheid de blaas van de peritoneale incisie. Open een dop van de gelpoort en steek geslurpt ijs door de zes centimeter Pfannenstiel-incisie, gevolgd door de nier-allograft gewikkeld in het ijsvolle gaas.Plaats vervolgens het allograft op het peritoneale zakje lateraal aan de iliacale vaten aan de rechterkant. Met de bediening van ProGrasp-tangen in robotarm vier klemt u de rechter externe iliacale ader door bulldogklemmen. Gebruik vervolgens Potts Scissors op een lineaire of schuine manier om een venotomie te maken gezien de diameter van de nierader.Gebruik een 6/0 ePTFE hechting aan anastomose aan nierader allograft naar de rechter externe iliacale ader op een end-to-side continue manier. Maak een knoop aan het caudale uiteinde van de aderen, voordat u intraluminaal en continu de achterwand van de externe iliacale ader en nierader hecht. Hecht daarna de voorste wand van de iliacale ader op een continue manier.En spoel het lumen met de gehepariniseerde normale zoutoplossing. Gebruik vervolgens een Silastic-buis door de gelpoort om de anastomose te knopen. Klem de allograft nierader vast met een bulldogklem.Verwijder vervolgens de juiste externe iliacale ader, gevolgd door het klemmen van de rechter externe iliacale slagader met bulldogklemmen. Maak een arteriotomie in de rechter externe iliacale slagader door een rond gat te maken met Potts Scissors zonder een arteriële stoot. En ga verder met het anastomose van de allograft nierslagader naar de rechter externe iliacale slagader, zoals aangetoond.Als er geen duidelijke bloeding is op de anastomoseplaats, dek dan de allograft nierader en slagader los. En verwijder het ijspakte gaas. Gebruik vervolgens een irrigatiebuis om warme normale zoutoplossing aan te brengen op het allograft via de gelpoort.Om ureteroneocystostomie uit te voeren volgens de Lich-Gregoir-techniek, plaatst u het distale uiteinde van de dubbele J-ureterale stent in de urineblaas. Begin bij de achterste hoek van het ingesneden blaasslijmvlies en voer een continue hechting uit met behulp van een 6/0 polydioxanon hechting. En maak een hechting in de voorste hoek.Voer van de voorste hoek naar de achterste hoek een continue hechting uit. En gebruik een 4/0 Polyglactin multi-filament absorbeerbare hechting om de anti-refluxtunnel van de detrusorspier op een onderbroken manier te sluiten. Met behulp van polymeer vergrendelingsclips bedekt u met tussenpozen het nieralleograft met het ingesneden peritoneum langs de rechter paracolische goot.Robot-geassisteerde niertransplantatie, of RAKT, werd uitgevoerd op 21 ontvangers, waaronder drie zwaarlijvige patiënten. Eén patiënt leed aan primaire niet-functie als gevolg van renale veneuze trombose. De graftectomie werd drie dagen na de niertransplantatie uitgevoerd en er werd geen vertraagde transplantaatfunctie waargenomen bij de patiënt.Een klein aantal patiënten moest vóór de niertransplantatie een angioplastiek van de nierslagader of nierader op de achterbank ondergaan. De gemiddelde ziekenhuisopname dagen na de niertransplantatie was 7,4. De gemiddelde geschatte glomerulaire filtratiesnelheid, of eGFR, een maand na RAKT was 74,9 milliliter per minuut per vierkante meter.In tegenstelling tot eerdere rapporten, plaatsten we het nierallenit aan de laterale kant van de iliacale vaten vóór vasculaire anastomose, vergelijkbaar met de conventionele open techniek. Deze techniek kan helpen om onverwachte torsie of knikken van niervaten te voorkomen. Na het plaatsen van het nier allograft aan de laterale zijde, kunnen veneuze en arteriële anastomosen worden uitgevoerd onder de juiste hoek en lengte.Het is noodzakelijk om de klinische resultaten op lange termijn te vergelijken, waaronder de overleving van nierallograft en biopsie-bewezen acute afstoting tussen robotondersteunde en open niertransplantatie, vooral bij patiënten met een hoog immunologisch risico.
