복강경 및 로봇 보조 기술이 살아있는 기증자 신장 절제술에 널리 적용되었지만 신장 이식은 여전히 주로 기존의 개방 기술을 사용하여 수행됩니다. 그러나 최근에는 신장 이식을위한 최소 침습적 접근법이 점점 더 많이 사용되고 있습니다. 전통적인 개복 수술과 비교하여 최소 침습 신장 이식은 수술 부위 감염, 절개 탈장 및 상처 열개의 위험이 낮을뿐만 아니라 입원 기간이 짧습니다 12,13,14,15,1...

신장 이식은 복막 투석 또는 혈액 투석에 비해 생존 연장과 삶의 질 향상에 기여합니다1. 개방형 접근법이 신장 이식의 표준 절차이지만 로봇 보조 기술이 최근에 채택되었습니다 2,3,4. 특히, 로봇 보조 신장 이식(RAKT)은 개방 신장 이식에 비해 수술 후 통증 최소화, 미용 개선, 상처 감염 감소, 입원 기간 단축등 몇 가지 장점이 있습니다5. 또한 최소 침습적 접근 및 로봇 기술을 통해 외과의는 병적 비만 환자에서 신장 이식을 안전하게 수행 할 수 있습니다 6,7,8,9. 그러나, 그 복잡성 때문에, RAKT는 작동 시간, 기능적 결과, 및 안전성에서 충분한 재현성을 달성하기 위해 학습 곡선을 필요로 한다10.
여러 혈관을 가진 동종 이식편은 일반적으로 혈관 재건이 필요하며, 이로 인해 추위와 따뜻한 허혈 시간이 연장됩니다. RAKT의 기술적 과제에도 불구하고 유럽의 다기관 연구에 따르면 여러 혈관이 있는 동종 이식편을 사용하는 RAKT는 기술적으로 실현 가능하며 유리한 기능적 결과를 가져옵니다11. 혈관 문합 중에 신장 동종 이식편을 골반 내측에 배치하는 것이 더 일반적이지만, 이전 보고서 4,5,6,7,8,9에 따르면 동종 이식편은 이 프로토콜에서 장골 혈관 측면의 복막 주머니에 배치되었습니다. 문합 중에 동종 이식편을 내측으로 넣고 복막 주머니로 뒤집는 것이 안전 할 수 있지만,이 기술은 경험이없는 외과의에게는 익숙하지 않을 수 있습니다. 또한, 복막 주머니의 동종 이식편과 신장 혈관을 적절한 위치에 놓고 혈관 문합을 수행하는 것이 더 편리합니다. 이 문서에서는 뒤집기 없이 RAKT에 대한 단계별 절차를 설명합니다.

robot-assisted kidney transplantation

이 논문은 살아있는 기증자의 로봇 보조 신장 이식 (RAKT)에 대해 설명합니다. 로봇은 환자의 갈라진 다리 사이에 도킹되어 앙와위 트렌델렌부르크 위치에 배치됩니다. 신장 동종 이식편은 살아있는 기증자가 제공합니다. 혈관 문합 전에 요관에 double-J 스텐트를 삽입하여 신장 동종 이식편을 준비하고 얼음으로 포장 된 거즈로 싸서 문합 온도를 낮 춥니 다. 로봇 카메라용 12mm 또는 8mm 포트와 로봇 암용 8mm 포트 3개가 배치됩니다. 장골 혈관과 방광을 해부하기 전에 요근 위의 양쪽 복막 플랩을 들어 올려 신장 동종 이식편을 위한 복막 주머니가 만들어집니다. 6cm Pfannenstiel 절개를 통해 신장을 복막 주머니에 삽입하고 오른쪽 장골 혈관 측면에 삽입합니다. 
불독 클램프로 외부 장골 정맥을 클램핑 한 후 독 절개술을 시행하고 이식편 신장 정맥을 6/0 폴리 테트라 플루오로 에틸렌 봉합사로 끝에서 측면 연속 방식으로 외부 장골 정맥에 문합합니다. 이식편 신장 정맥을 클램핑 한 후, 장골 정맥이 손상됩니다. 그 다음에는 외부 장골 동맥의 클램핑, 동맥 절개술, 6/0 폴리 테트라 플루오로 에틸렌 봉합사를 사용한 동맥 문합, 이식편 신장 동맥의 클램핑 및 외부 장골 동맥의 클램핑이 이어집니다. 그런 다음 재관류가 수행되고 Lich-Gregoir 기술을 사용하여 요관 방광 절제술이 수행됩니다. 복막은 폴리머 잠금 클립으로 몇 위치에서 닫히고 폐쇄 흡입 배수구는 작업 포트 중 하나를 통해 배치됩니다. 기복막을 수축시킨 후 모든 절개가 닫힙니다.
