Selv om laparoskopiske og robotassisterte teknikker har blitt mye brukt for levende donor nefrektomi, er nyretransplantasjoner fortsatt hovedsakelig utført ved hjelp av konvensjonelle åpne teknikker. Nylig har imidlertid en minimal invasiv tilnærming til nyretransplantasjon blitt stadig mer brukt. Sammenlignet med tradisjonell åpen kirurgi har minimal invasiv nyretransplantasjon lavere risiko for infeksjon på operasjonsstedet, snittbrokk og sårdehiscens, samt kortere sykehusinnleggelse 12,13,14,15,16.<sup class="xr...

Nyretransplantasjon bidrar til forlenget overlevelse og bedre livskvalitet sammenlignet med peritonealdialyse eller hemodialyse1. Selv om den åpne tilnærmingen er standardprosedyren for nyretransplantasjon, har robotassisterte teknikker nylig blitt vedtatt 2,3,4. Spesielt har robotassistert nyretransplantasjon (RAKT) flere fordeler fremfor åpen nyretransplantasjon: minimal postoperativ smerte, bedre kosmesis, færre sårinfeksjoner og kortere sykehusopphold5. Videre gjør minimal invasiv tilgang og robotteknologi det mulig for kirurger å trygt utføre nyretransplantasjoner hos sykelig overvektige pasienter 6,7,8,9. På grunn av sin kompleksitet krever RAKT imidlertid en læringskurve for å oppnå tilstrekkelig reproduserbarhet i driftstid, funksjonelle resultater og sikkerhet10.
Allotransplantater med flere kar krever vanligvis vaskulær rekonstruksjon, noe som fører til utvidede kalde og varme iskemiske tider. Til tross for de tekniske utfordringene ved RAKT, rapporterte en europeisk multisenterstudie at RAKT ved bruk av allotransplantater med flere fartøy er teknisk mulig og fører til gunstige funksjonelle resultater11. Selv om det er mer vanlig å plassere nyreallograft i bekkenet medialt under vaskulær anastomose, ifølge tidligere rapporter 4,5,6,7,8,9, ble allograft plassert på peritonealposen lateral til iliac-karene i denne protokollen. Selv om det kan være trygt å sette et allograft medialt under anastomose og vende det til bukhinnen, kan denne teknikken ikke være kjent for uerfarne kirurger. robot-assisted kidney transplantation

Denne artikkelen beskriver robotassistert nyretransplantasjon (RAKT) fra en levende donor. Roboten er forankret mellom pasientens parterte ben, plassert i den bakre Trendelenburg-posisjonen. Nyre allografts er levert av en levende donor. Før vaskulær anastomose fremstilles nyreallograften ved å sette inn en dobbel-J-stent i urineren, og temperaturen for anastomosen senkes ved å pakke den inn i en ispakket gasbind. En 12 mm eller 8 mm port for robotkameraet og tre 8 mm porter for robotarmer er plassert. En peritoneal pose er opprettet for nyre allograft ved å heve peritoneal klaffene på begge sider over psoas muskelen før dissekere iliac fartøy og blære. Et 6 cm Pfannenstiel snitt er laget for å sette nyrene inn i peritoneal posen, lateral til høyre iliac fartøy. 
Etter klemming av den ytre iliacvenen med Bulldogs-klemmer, utføres en venotomi, og transplantatnyrevenen anastomoseres til den eksterne iliacvenen på en ende-til-side kontinuerlig måte med en 6/0 polytetrafluoretylensutur. Etter klemming av transplantatnyrevenen, blir iliacvenen avkortet. Dette etterfølges av klemming av arteria iliac, arteriotomi, arteriell anastomose med 6/0 polytetrafluoretylensutur, klemming av nyrearterien transplantat og nedtrapping av arteria iliaca eksterna. Reperfusjon utføres deretter, og ureteroneocystostomi utføres ved hjelp av Lich-Gregoir-teknikken. Bukhinnen er lukket på noen få steder med polymerlåseklips, og et lukket sugeavløp plasseres gjennom en av arbeidsportene. Etter deflatering av pneumoperitoneum lukkes alle snitt.

Vi setter opp et rutinemessig pasientforløp for mottakere som har RAKT på vårt senter. Renal Doppler ultralyd utføres en dag etter transplantasjon og technetium-99m dietylentriamin penta-eddiksyre renal skanning to dager etter transplantasjon. For venøs tromboembolismeprofylakse påføres en intermitterende pneumatisk kompresjonsanordning i løpet av de første 24 timene etter RAKT. Foley kateter fjernes på den fjerde postoperative dagen. I denne protokollen har vi foreslått trinnvise prosedyrer for robotassistert nyretransplantasjon uten å snu. Det er mer naturlig å utføre vaskulær anastomose med allograft i bukhinneposen og nyrekarene i riktig posisjon. Denne teknikken kan brukes på andre faste organtransplantasjoner, som lever og bukspyttkjertel.Vi håper å utvide nytten av minimalt invasive kirurgiske tilnærminger ved hjelp av robotsystemet innen transplantasjon. Robotassistert nyretransplantasjon kan studeres først etter å ha fått litt utdanning om robotsystemet. Det er bedre å få hjelp fra en erfaren kirurg.Før du starter prosedyren, må du forberede robotarmene med de kirurgiske instrumentene for å utføre intra-abdominal disseksjon. Plasser monopolar buet saks i arm to, fenestrert bipolar tang i arm tre og ProGrasp Tang i arm fire. Start den operative prosedyren ved å lage et snitt i bukhinnen langs høyre parakoliske rennestein for å lage en pose for nyreallograftet.Etter snittet, disseker de riktige indre iliac-fartøyene langs hele lengden og omkranser hvert fartøy med en fartøysløyfe. Lag en knute i den kaudale enden av venene, før intraluminalt og kontinuerlig suturering av bakre vegg av den ytre iliacvenen og nyrevenen. Etterpå sutur den fremre veggen av iliac-venen på en kontinuerlig måte.Og skyll lumen med heparinisert normal saltvann. Bruk deretter et silastisk rør gjennom gelporten for å knytte anastomosen. Klem allograft nyrevenen med en bulldogklemme.Deretter de-klemme høyre ytre iliac vene, etterfulgt av klemming av høyre ekstern iliac arterie med bulldog klemmer. Lag en arteriotomi i høyre ytre iliac arterie ved å lage et rundt hull med Potts Scissors uten arteriell stans. Og fortsett å anastomose allograft nyrearterien til høyre ekstern iliac arterie, som vist.Hvis det ikke er påvist blødning på anastomosestedet, må du skru av allograft nyrevenen og arterien. Og fjern det ispakkede gasbindet. Bruk deretter et vanningsrør for å påføre varm normal saltvann til allograft gjennom gelporten.For å utføre ureteroneocystostomi i henhold til Lich-Gregoir-teknikken, sett den distale enden av den doble J-ureterale stenten inn i urinblæren. Og bruk en 4/0 Polyglactin multi-filament absorberbar sutur for å lukke detrusor muskel anti-reflux tunnel på en avbrutt måte. Ved hjelp av polymerlåseklips dekker du periodisk nyreallograften med det innskårne bukhinnen langs høyre parakoliske rennestein.Robotassistert nyretransplantasjon, eller RAKT, ble utført på 21 mottakere, inkludert tre overvektige pasienter. Én pasient fikk primær ikke-funksjon på grunn av renal venetrombose. Transplantatektomi ble utført tre dager etter nyretransplantasjonen, og det ble ikke observert forsinket transplantatfunksjon hos pasienten.Et lite antall pasienter måtte angioplasteres av nyrearterien eller nyrevenen på bakbordet før nyretransplantasjon. Gjennomsnittlig sykehusinnleggelse dager etter nyretransplantasjonen var 7,4. Gjennomsnittlig estimert glomerulær filtreringshastighet, eller eGFR, en måned etter RAKT var 74,9 milliliter per minutt per meter kvadrat.I motsetning til tidligere rapporter, plasserte vi nyreallograft på lateral side av iliac-karene før vaskulær anastomose, lik den konvensjonelle åpne teknikken. Denne teknikken kan bidra til å forhindre uventet vridning eller kinking av nyrekar. Etter plassering av nyreallograft på lateral side, kan venøse og arterielle anastomoser utføres i riktig vinkel og lengde.Det er nødvendig å sammenligne de langsiktige kliniske resultatene, inkludert overlevelse av nyreallotransplantat og biopsi-bevist akutt avstøtning mellom robotassistert og åpen nyretransplantasjon, spesielt hos pasienter med høy immunologisk risiko.
