관절 연골과 기저 뼈 파괴로 인한 골관절염(OA)은 장애 1,2로 이어지는 심각한 문제로 남아 있습니다. 혈액과 신경 공급이 없으면 연골자가 치유 능력은 일단 손상되면 최소화됩니다 3,4. 지난 수십 년 동안 자가 연골세포 이식(ACI)에 기초한 치료법은OA 치료에서 약간의 진전을 이루었습니다5. 연골 세포 분리 및 확장을 위해서는 OA 관절의 비 중량 부위에서 작은 연골을 채취하여 연골에 손상을 입힐 필요가 있습니다. 또한, 절차는 확장 된 연골 세포(6)를 이식하기위한 두 번째 수술을 요구할 것이다. 따라서 연골 손상없이 OA 치료를위한 1 단계 요법이 광범위하게 연구되고 있습니다.
중간엽 줄기세포(MSC)는 OA 치료 7,8을 위한 유망한 대안으로 제안되었습니다. 여러 조직에서 유래 한 MSC는 특정 자극으로 연골 세포로 분화 할 수 있습니다. 중요하게도, MSC는 항염증9을 통해 면역 반응을 조절할 수 있다. 따라서 MSC는 연골 결함을 복구하고 특히 염증 환경에서 면역 반응을 조절함으로써 OA 치료에서 상당한 이점을 가지고 있습니다. 중간엽 줄기세포(MSC)에서 분비되는 엑소좀은 연골 손상 및 골관절염 치료의 유망한 후보로 제안되었습니다. 임상 적용을 위한 엑소좀은 대규모 생산이 필요합니다. 이를 위해 인간 활액 MSC(hSF-MSC)를 마이크로캐리어 비드에서 성장시킨 다음 동적 3차원(3D) 배양 시스템에서 배양했습니다. 이 프로토콜은 3D 동적 배양을 활용하여 SF-MSC 배양 상청액에서 대규모 엑소좀을 성공적으로 획득했습니다. 엑소좀을 초원심분리로 수확하고 투과전자현미경, 나노입자 투과분석법 및 웨스턴 블로팅으로 검증하였다. 또한 엑소 좀의 미생물 학적 안전성이 검출되었습니다. 엑소좀 검출 결과는 이 접근법이 우수제조관리기준(GMP) 등급 엑소좀을 많이 생산할 수 있음을 시사합니다. 이러한 엑소좀은 엑소좀 생물학 연구 및 임상 골관절염 치료에 활용될 수 있습니다.
