技術的な側面から、手順で最も困難なステップは、門脈カニューレーションです。他のステップに関しては、良い習慣が観察され、一般的な予防措置が取られれば、分離の安定性を改善することができる。したがって、各ステップの推論を理解することは、外科医が処置中に起こり得る様々な変数に応答することができるように重要である。

このラット肝細胞単離プロトコールは、消化アプローチ前の肝切除に基づいている。このプロトコルは、細胞単離のためのコンパクトでポータブルな統合灌流システムを導入します。この技術は、コラゲナーゼ再循環を含み、コラゲナーゼ使用の量を減らし、高い重力収率および肝細胞特異的機能を確保しながら、消化時間の延長においてより柔軟性を提供する。ラットをフードに入れてください。腹筋を切った後、湾曲した鉗子の裏側で腸を右に押します。門脈と胆管の周りに縫合糸をループさせるための湾曲した鉗子の先端で胆管の下の膜を壊します。絹の外科用縫合糸を使用して、静脈が左右に分岐して異なる肝葉になる直前に、肝臓に近い門脈の周りに非常に緩い結紮を行います。最初の合字から約2〜3センチメートル上流、胃静脈が門脈から分岐する直前に、2対の湾曲した鉗子の先端で門脈の下の組織に穴を開ける。次に、カニューレ中に門脈を支えるために穴を伸ばします。毎分約3ミリリットルの流量でポンプ速度を4RPMに下げることで、肝臓への圧力蓄積を防ぎます。静脈内カテーテルからのカルシウムフリーバッファーの流出が遅い点滴に減少していることを確認してください。門脈を鉗子で優しく支えながら、針の斜めを上に向けて静脈内カテーテルを持ちます。そして、針を門脈に10〜20度の角度で挿入する。その後、ベベル全体が静脈の内側になるまでゆっくりと進みます。正しい挿入は、肝臓の色のブランチングをもたらすはずです。カニューレを針の上に進め、親指と中指でカテーテルを持ちながら人差し指で翼を後ろに引いて、カニューレの後ろに針を引っ込めます。静脈クリップで門脈をカテーテルに固定し、門脈の下のクリップで灌流の流れの乱れを避けます。次に、肝臓の圧力蓄積を防ぐために肝下部下大静脈を切断し、正しい切断を確実にするために血液噴出インパルスを観察します。ポンプ速度を 38 RPM に上げ、流量を毎分 約 33 ミリリットルにします。そして、肝下大静脈を鉗子で2~3秒間閉じて肝臓を3回洗い流してから、再度開きます。すべての肝葉を灌流するために、カニューレ先端の位置を調整し、門脈が異なる肝葉に分岐する前に配置されていることを確認してください。次に、門脈の周りの緩い結紮を分岐点の少し上流に締め付け、カニューレの位置を門脈に固定して、硬い針がカニューレを支持する点、斜めと側孔の間に3つのノットを作ることでバッファの逆流を防ぎます。肝切除後、肝臓をカルシウムフリーの緩衝液で12分間灌流させる。次に、カルシウムフリーバッファーのローラークランプを締め、コラゲナーゼバッファーのローラークランプを緩めて灌流バッファーを変更します。コラゲナーゼ緩衝液が肝臓に到達した後、肝上大静脈を2〜3秒間閉じて再び開封して肝臓を3回洗い流す。カテーテルを支えながら横隔膜を鉗子で保持し、誤って剥離しないように注意して、コラゲナーゼ緩衝液の再循環のためにビーカーの上のステージに肝臓を慎重に動かします。肝臓を12分間消化し、肝臓が滑らかな茶色の質感を失い、どろどろになったら灌流を止める。必要に応じて消化時間を延長します。肝臓の消化後、門脈を切断し、カニューレを慎重に除去する。そして、肝臓を冷たいDMEMに移して肝細胞を単離する。湾曲した鉗子の裏側を使って肝臓を優しく叩いて、キラキラしたカプセルを壊して剥がします。細胞が培地中で完全に解離するまでDMEM中で肝臓を穏やかに揺らすことによって肝細胞を解放する。肝細胞の生存率は、トリップおよびブルーカウンティングによって決定されるように88〜94.8%であった。体重200〜300グラムのラットからの肝細胞の収量は、単離当たり10〜8番目の細胞の5倍までであった。代表的な画像は、未精製肝細胞懸濁液および精製肝細胞懸濁液を示している。単離された肝細胞の純度は約96.8%であり、これは細胞核の総数に対してアルブミン陽性蛍光染色細胞を計数することによって定量した。偽色で示した代表的な画像では、緑はアルブミンを表し、青はアルブミンを含む細胞核を示す。アルブミンを含まない細胞核は赤色に強調表示されている。サンドイッチ培養中の肝細胞は、別個の胆汁小管を形成し、良好な細胞接触を有していた。アルブミン、尿素およびシトクロムP450アッセイによって示されるように、肝細胞は高い尿素およびアルブミン分泌を示した。3日目、肝細胞はCYP1A2、CYP2B1/2、CYP3B2に対して高い酵素活性を示した。門脈に針を挿入する前に精神的に準備してください。挿入時に、外科医はIVCを切断した後にポンプ速度が増加するまでパルスなしで続ける必要があります。
