Neonatale bacteriële meningitis is een veel voorkomende pediatrische infectieziekte. Escherichia coli (E. coli) met een K1 capsule is de meest voorkomende Gram-negatieve ziekteverwekker die neonatale bacteriële meningitis veroorzaakt, goed voor ongeveer 80% van de totale incidentie1,2,3. Ondanks de vooruitgang in de antimicrobiële chemotherapie en ondersteunende zorg, is bacteriële meningitis nog steeds een van de meest verwoestende aandoeningen met hoge morbiditeit enmortaliteit 4.
Het optreden van neonatale bacteriële meningitis begint meestal met bacteriëmie veroorzaakt door het binnendringen van pathogene bacteriën in de perifere circulatie van de lokale laesies van de pasgeborenen, gevolgd door penetratie door de bloed - hersenbarrière (BBB) in de hersenen, wat resulteert in de ontsteking van de hersenvliezen4. Het begin van bacteriëmie hangt af van de interactie tussen bacteriën en gastheerimmuuncellen, waaronder neutrofielen en macrofagen, enz. Neutrofielen, die goed zijn voor ~ 50-70% van de witte bloedcellen, zijn de eerste verdedigingslinie tegen bacteriële infecties5,6. Tijdens de invasie van bacteriën worden de geactiveerde neutrofielen gerekruteerd naar de besmettelijke locaties en geven reactieve zuurstofsoorten (ROS) vrij, waaronder het superoxide anion, waterstofperoxide, hydroxylradicalen en singlet zuurstof7. De mitochondriën is de belangrijkste plaats van ROS-productie in eukaryotische cellen, en verschillende oxidasen (bijv. nicotinamide adenine dinucleotidefosfaat (NADPH) oxidasecomplex, lipoxygenasesysteem, eiwitkinase C en cyclooxygenasesysteem) bemiddelen de productie van ROS9,10. Escherichia Coli is de meest voorkomende gramnegatieve bacterie die neonatale meningitis veroorzaakt. Het optreden van neonatale bacteriële meningitis begint puur met de bacteriëmie en vervolgens dringen de bacteriën in het bloed door de bloed-hersenbarrière om meningitis te veroorzaken. Neutrofielen zijn de eerste verdedigingslinie tegen de bacteriële infecties.Tijdens bacteriële invasie geven de actieve neutrofielen ROS vrij. Centrifugeer de perifere bloedmonsters bij 2000 RPM gedurende vijf minuten.Na centrifugeren worden de bloedmonsters verdeeld in drie lagen. Gooi het supernatant volledig weg en resuspendeer de sedimenten met 500 microliter magnetische sorteerbuffer voor maximaal 100 miljoen cellen.Spoel de kolom af met een magnetische sorteerbuffer van drie milliliter. Breng de celsuspensie aan op de kolom zodat de neutrofielen die door de magnetische kralen zijn gelabeld, aan de kolom kunnen worden bevestigd. Spoel de niet-specifieke specifieke cellen af door drie keer drie milliliter buffer toe te voegen, zodra het kolomreservoir elke keer leeg is.Verwijder de kolom van de magneetscheider en plaats deze in een nieuwe buis. Voeg vijf milliliter magnetische sorteerbuffer toe aan de kolom. Duw de cellen met een magnetisch label naar buiten met behulp van een zuiger.Centrifugeer de buis bij 1,200 RPM gedurende vijf minuten. Aspireer het supernatant volledig en resuspendeer het sediment met één milliliter kweekmedium. Bepaal het celnummer met een teller.De celconcentratie is aangepast tot 2 miljoen per milliliter in het kweekmedium dat de fluorescentiesonde DHE bevat. Plaats de neutrofielen gedurende 30 minuten in de incubator om de DHE-sonde te laden. Wijs de celophanging toe aan een 96-put zwarte microplaat met 200 microliter per put.Schakel de microplaatlezer in. Stel de temperatuur in op 37 Graden Celsius, kies fluorescentie leesmodus, kinetisch leestype, stel de fluorescentiegolflengte in. Kies het plaatformaat.Bepaal het leesgebied. Meet de fluorescentie-intensiteit elke vijf minuten gedurende een uur. Schud de plaat drie seconden voor elke meting.Haal de microplaat uit de incubator. Voeg PMA- of E44-stammen toe met drie herhalingen. Plaats de plaat in de microplaatlezer, druk op de startknop om de test onmiddellijk te starten.Met behulp van het protocoloverzicht in dit artikel werden de neutrofielen geïsoleerd uit het menselijke perifere bloed en geladen met fluorescentiesonde DHE. Door toevoeging van PMA vertoonde intracellulair ROS bij neutrofielen een significante toename van 20 tot 40 minuten en bereikte het een piek na 60 minuten. Maar ik denk dat E44-stammen, de ROS-productie onmiddellijk plaatsvindt en de tijd afhankelijk verhoogt.De expressie van ROS veroorzaakt door E44-stam is vergelijkbaar met die veroorzaakt door PMA. De detectie van ROS-productie bij neutrofielen kan bijdragen aan de verdere studie van de bacteriocidale mechanismen van neutrofielen. In dit protocol bieden we een eenvoudigere manier om de ROS-productie bij neutrofielen in realtime te detecteren.De eenvoudige methode kan ook worden gebruikt om de ROS-productie in andere hosts-celtypen te monitoren tijdens interacties tussen gastheer en bacterie.
