La rééducation reconstructive du maxillaire atrophique à l’aide de greffons est difficile car elle nécessite une bonne technique chirurgicale, une couverture des tissus mous de haute qualité sur la greffe, une coopération importante du patient et des patients dont la santé est favorable à la restauration financière17,18. La pose d’implants dentaires pour la reconstruction chez les patients présentant une atrophie maxillaire représente un défi clin...

Dans les années 1990, Branemark a introduit une technique alternative pour la greffe osseuse, l’implant zygomatique (ZI), également appelé zygomaticus fixture1. Il a été initialement utilisé pour le traitement des victimes de traumatismes et des patients atteints de résection tumorale où il y avait un défaut dans la structure maxillaire. Après maxillectomie, de nombreux patients ont conservé l’ancrage uniquement dans le corps du zygome ou dans l’extension frontale de l’os zygomatique 1,2,3.
Plus récemment, la technique ZI a été largement utilisée chez les patients édentés et dentés avec un maxillaire sévèrement résorbé. L’indication principale pour les implants ZI est un maxillaire atrophique. L’utilisation de quatre ZI dans un système de mise en charge immédiate (prosthodontie fixe) est pratique pour les chirurgiens ayant une vaste expérience clinique, et elle semble représenter une excellente méthode alternative aux techniques de greffe osseuse 2,4. Cependant, il existe des risques lors de la mise en place de ZI, soit à main levée, soit à l’aide d’un gabarit chirurgical pour guider. Les risques comprennent un placement inexact dans l’alvéole, une pénétration de la cavité orbitaire ou de la fosse infra-temporale et un placement inapproprié dans la proéminence zygomatique5. Le placement de plusieurs ZI rend cette chirurgie risquée et difficile à réaliser. Par conséquent, l’amélioration de la précision du placement de ZI est essentielle à son utilisation clinique et à sa sécurité.
Le système de navigation chirurgicale en temps réel offre une approche différente. Il fournit des trajectoires en temps réel et complètement visualisées grâce à l’analyse d’images de tomodensitométrie préopératoire et peropératoire. Avec le système de navigation en temps réel, la précision et la sécurité ont été améliorées grâce à une chirurgie et à un traitement sophistiqués 5,6. Un protocole pratique, réalisable et reproductible a été développé en utilisant le système de navigation chirurgicale en temps réel pour placer avec précision les ZI dans le maxillaire sévèrement atrophique 5,7,8,9,10. Avec ce protocole, nous avons traité des centaines de patients avec des résultats cliniques satisfaisants 5,6,7,8,9,10. Ici, nous présentons le protocole avec les informations détaillées sur la procédure de traitement.

real-time dynamic navigation system for the precise quad-zygomatic implant placement in a patient with a severely atrophic maxilla

Les implants zygomatiques (ZI) sont un moyen idéal pour traiter les cas de maxillaire et de défauts maxillaires édentés sévèrement atrophiques, car ils remplacent une augmentation osseuse importante et raccourcissent le cycle de traitement. Cependant, il existe des risques associés à la mise en place de ZI, tels que la pénétration de la cavité orbitaire ou de la fosse infra-temporale. De plus, le placement de plusieurs ZI rend cette chirurgie risquée et plus difficile à réaliser. Les complications peropératoires potentielles sont extrêmement dangereuses et peuvent entraîner des pertes irréparables. Ici, nous décrivons un protocole pratique, réalisable et reproductible pour un système de navigation chirurgicale en temps réel permettant de placer avec précision des implants quad-zygomatiques dans le maxillaire sévèrement atrophique de patients présentant un os résiduel qui ne répond pas aux exigences des implants conventionnels. Des centaines de patients ont reçu des ZI dans notre service sur la base de ce protocole. Les résultats cliniques ont été satisfaisants, les complications peropératoires et postopératoires ont été faibles et la précision indiquée par la perfusion de l’image conçue et de l’image tridimensionnelle postopératoire a été élevée. Cette méthode doit être utilisée pendant toute la procédure chirurgicale pour assurer la sécurité du placement de ZI.

La patiente inscrite était une femme de 60 ans sans aucune maladie systématique (Figure 1A-D, F). Grâce à la chirurgie de navigation en temps réel, nous pouvons traiter les patients présentant un maxillaire sévèrement atrophique nécessitant une charge immédiate de l’implant, rapidement, avec précision et en toute sécurité. L’un des principaux avantages de cette technique est qu’elle a amélioré la précision de la pose de cet implant zygomatique, ce qui est essentiel à son utilisation clinique et à sa sécurité. Cette technique permet de traiter les patients atteints d’une atrophie sévère du maxillaire due à une dysplasie ectodermique, une tumeur ou une parodontite sévère.Avec le patient anesthésié allongé sur la table d’opération en décubitus dorsal, fixez rigidement la base de référence du crâne au calvaire, avec une seule vis en titane autotaraudée de 1,5 par 6 millimètres. Fixez le tableau de référence à la base et assemblez-le avec trois sphères réfléchissantes marquées. Placez la caméra du système de navigation en position d’une heure pour surveiller la référence du crâne.Pour l’enregistrement, réglez spécifiquement le système de navigation sur le patient individuel à l’aide d’une sonde de positionnement avec une bille réfléchissante sur mesure pour contacter la surface extérieure des mini-vis l’une après l’autre. Affichez ensuite les images axiales et de reconstruction 3D coronales sagittales disponibles sur l’écran de navigation. Pour standardiser le forage avant de l’utiliser en chirurgie, utilisez un bloc d’étalonnage avec des trous de différents diamètres.Les perceuses doivent être fixées directement au bas du bloc par le chirurgien, puis l’assistant doit ajuster l’interface dans le module d’étalonnage. L’équipement produira un son une fois le processus terminé. Après avoir déterminé l’étendue de l’incision à l’aide de la navigation chirurgicale, soulevez le lambeau de toute l’épaisseur, ce qui permet une vue adéquate pour exposer les sites d’implant prévus.La gamme de l’élévation périostée doit contenir la crête alvéole, la paroi latérale du maxillaire et le bord inférieur de l’os zygomatique. À l’aide de la sonde de navigation, trouvez le point d’entrée et utilisez la pièce à main zygoma pour fixer les points d’entrée. Ensuite, trouvez l’entrée de l’os zygomatique avec la sonde et utilisez la pièce à main zygoma pour préparer le point d’entrée de l’os zygomatique.Ensuite, effectuez la procédure de forage en vous assurant qu’elle suit les trajectoires de l’entrée au point de sortie comme prévu. Utilisé la perceuse de 2,9 millimètres en premier pour préparer le chemin du point d’entrée, qui a été localisé à l’aide de la sonde de navigation à l’entrée de l’os zygomatique. Préparez d’abord la mésiale puis la distale.Pour élargir le lit d’implant, utilisez la pièce à main et prolongez le trajet de l’entrée de l’os zygomatique au point terminal conçu à la surface de l’os zygomatique. Demandez à l’assistant de poser une main sur la surface de la paroi orbitale latérale, pour assurer sa sécurité. Ensuite, agrandissez la trajectoire avec une perceuse expansive de 3,5 millimètres de diamètre.Utilisez la barre de mesure et la sonde de navigation pour vérifier la direction et la position de la trajectoire. Identifiez la longueur de l’implant à l’aide de l’outil de mesure. Si la profondeur ne répond pas à l’exigence de la longueur prévue, préparez-la à la profondeur définie.Ensuite, implantez les implants zygomatiques à l’aide d’un outil manuel spécifique, puis utilisez la sonde de navigation pour vérifier le positionnement correct. Après avoir placé des piliers multi-unités et des capuchons de cicatrisation sur les implants, suturez l’incision avec la suture en polypropylène 4-0. Pour l’intégration d’images, obtenez des images postopératoires CBCT et une radiographie panoramique pour évaluer la position de l’implant zygomatique dans les 72 heures suivant la chirurgie.Exporter les données postopératoires vers le logiciel de planification pour superposer l’image de la CBCT postopératoire et du plan chirurgical préopératoire. Comparaison de l’emplacement du point d’entrée, de l’extrémité et de la déviation angulaire. Après la chirurgie, à l’aide du système de navigation, le patient a reçu une restauration temporaire dans les trois jours, qui a résolu à la fois les problèmes esthétiques et de prononciation.La scintigraphie CBCT postopératoire et l’intégration des images ont révélé les erreurs de position de départ et de terminale, ainsi que la déviation angulaire. Veuillez vérifier la position avec la sonde pour vous assurer que le forage est correct. Nous pouvons également utiliser notre outil manuel spécial pour mesurer la distance entre le point de départ et le point cible.Ce protocole encourage les chercheurs à explorer davantage le domaine. De plus en plus d’équipes de recherche tentent de développer de nouveaux systèmes de navigation pour les chirurgies faciales et implantaires maxillaires afin d’augmenter la précision et l’efficacité, ainsi que de raccourcir la courbe d’apprentissage.
