Die rekonstruktive Rehabilitation des atrophischen Oberkiefers mit Transplantaten ist schwierig, da sie eine gute Operationstechnik, eine Abdeckung von hochwertigem Weichgewebe über dem Transplantat, eine erhebliche Patientenkooperation und Patienten mit einer für die endgültige Wiederherstellung günstigen Gesundheit erfordert17,18. In den 1990er Jahren führte Branemark eine alternative Technik zur Knochentransplantation ein, das Jochbeinimplantat (ZI), das auch als Zygomaticus-Vorrichtung1 bezeichnet wird. Es wurde zunächst zur Behandlung von Traumaopfern und Patienten mit Tumorresektion eingesetzt, bei denen ein Defekt in der Oberkieferstruktur vorlag. Nach der Maxillektomie behielten viele Patienten die Verankerung nur im Körper des Zygoms oder in der frontalen Verlängerung des Jochbeins 1,2,3.
In jüngerer Zeit wurde die ZI-Technik häufig bei zahnlosen und gezähnten Patienten mit einem stark resorbierten Oberkiefer eingesetzt. Die Hauptindikation für ZI-Implantate ist ein atrophischer Oberkiefer. Die Verwendung von vier ZI in einem Sofortbelastungssystem (festsitzende Prothetik) ist für Chirurgen mit breiter klinischer Erfahrung praktisch und scheint eine ausgezeichnete Alternative zu Knochentransplantationstechniken darzustellen 2,4. Es gibt jedoch Risiken bei der Platzierung von ZIs, entweder freihändig oder mit einer chirurgischen Schablone zur Führung. Zu den Risiken gehören eine ungenaue Platzierung innerhalb der Alveole, das Eindringen in die Orbitalhöhle oder die infratemporale Fossa und eine unangemessene Platzierung innerhalb der zygomatischen Prominenz5. Die Platzierung mehrerer ZIs macht diese Operation riskant und schwierig durchzuführen. Daher ist die Verbesserung der Präzision der ZI-Platzierung entscheidend für den klinischen Einsatz und die Sicherheit.
Das chirurgische Echtzeit-Navigationssystem bietet einen anderen Ansatz. Es liefert Echtzeit- und vollständig visualisierte Trajektorien durch die Analyse präoperativer und intraoperativer Computertomographie-Bilder. Mit dem Echtzeit-Navigationssystem wurden sowohl Präzision als auch Sicherheit durch ausgefeilte Chirurgie und Behandlungverbessert 5,6. Mit dem chirurgischen Echtzeitnavigationssystem wurde ein praktisches, praktikables und reproduzierbares Protokoll entwickelt, um ZIs präzise im stark atrophischen Oberkiefer 5,7,8,9,10 zu platzieren. Mit diesem Protokoll haben wir Hunderte von Patienten mit zufriedenstellenden klinischen Ergebnissen 5,6,7,8,9,10 behandelt. Zygomatische Implantate (ZIs) sind ein idealer Weg, um Fälle von stark atrophischen zahnlosen Ober- und Oberkieferdefekten zu behandeln, da sie einen ausgedehnten Knochenaufbau ersetzen und den Behandlungszyklus verkürzen. Es gibt jedoch Risiken im Zusammenhang mit der Platzierung von ZIs, wie das Eindringen in die Orbitalhöhle oder die infratemporale Fossa. Darüber hinaus macht die Platzierung mehrerer ZIs diese Operation riskant und schwieriger durchzuführen. Mögliche intraoperative Komplikationen sind äußerst gefährlich und können irreparable Verluste verursachen. Hier beschreiben wir ein praktikables, realisierbares und reproduzierbares Protokoll für ein Echtzeit-OP-Navigationssystem zur präzisen Platzierung von quad-zygomatischen Implantaten im stark atrophischen Oberkiefer von Patienten mit Restknochen, das nicht den Anforderungen herkömmlicher Implantate entspricht. Hunderte von Patienten haben in unserer Abteilung auf der Grundlage dieses Protokolls ZIs erhalten. Die klinischen Ergebnisse waren zufriedenstellend, die intraoperativen und postoperativen Komplikationen waren gering und die Genauigkeit, die durch die Infusion des entworfenen Bildes und des postoperativen dreidimensionalen Bildes angezeigt wurde, war hoch. Diese Methode sollte während des gesamten chirurgischen Eingriffs angewendet werden, um die Sicherheit der ZI-Platzierung zu gewährleisten.

Die eingeschriebene Patientin war eine 60-jährige Frau ohne systematische Erkrankungen (Abbildung 1A-D, F). Mit Hilfe der Echtzeit-Navigationschirurgie können wir Patienten mit einem stark atrophischen Oberkiefer, der eine sofortige Implantatbelastung erfordert, schnell, genau und sicher behandeln. Diese Technik ermöglicht die Behandlung von Patienten mit stark atrophischem Oberkiefer aufgrund von ektodermaler Dysplasie, Tumor oder schwerer Parodontitis.Wenn der anästhesierte Patient in Rückenlage auf dem Operationstisch liegt, befestigen Sie die Schädelreferenzbasis mit einer einzigen selbstschneidenden Titanschraube von 1,5 x 6 Millimetern fest an der Kalvaria. Befestigen Sie das Referenzarray an der Basis und montieren Sie es mit drei markierten reflektierenden Kugeln. Platzieren Sie die Kamera des Navigationssystems in der Ein-Uhr-Position, um die Schädelreferenz zu überwachen.Stellen Sie für die Registrierung das Navigationssystem gezielt auf den einzelnen Patienten ein, indem Sie eine Positioniersonde mit einer maßgeschneiderten reflektierenden Kugel verwenden, um die Außenfläche der Minischrauben nacheinander zu berühren. Mit dem 2,9-Millimeter-Bohrer wurde zunächst der Weg vom Eintrittspunkt vorbereitet, der sich mit Hilfe der Navigationssonde zum Eintritt des Jochbeins befand. Als nächstes vergrößern Sie die Flugbahn mit einem expandierenden Bohrer mit einem Durchmesser von 3,5 Millimetern.Verwenden Sie die Messleiste und die Navigationssonde, um die Richtung und Position der Flugbahn zu überprüfen. Bestimmen Sie die Länge des Implantats mit dem Messgerät. Wenn die Tiefe nicht den Anforderungen der geplanten Länge entspricht, bereiten Sie sie auf die eingestellte Tiefe vor.Als nächstes implantieren Sie die Jochbeinimplantate mit einem speziellen manuellen Werkzeug und verwenden Sie dann die Navigationssonde, um die korrekte Positionierung zu überprüfen. Nachdem Sie mehrgliedrige Abutments und Heilkappen auf die Implantate gesetzt haben, vernähen Sie den Schnitt mit der Polypropylen-Naht 4-0. Für die Bildintegration erhalten Sie postoperative DVT-Scan-Bilder und ein Panorama-Röntgenbild, um die Position des Jochbeinimplantats innerhalb von 72 Stunden nach der Operation zu beurteilen.Exportieren Sie die postoperativen Daten in die Planungssoftware, um das Bild der postoperativen DVT und des präoperativen Operationsplans zu überlagern. Vergleichen der Position des Einstiegspunkts, des Endpunkts und der Winkelabweichung. Nach der Operation erhielt der Patient über das Navigationssystem innerhalb von drei Tagen eine provisorische Restauration, die sowohl die ästhetischen als auch die Ausspracheprobleme ansprach.Postoperative DVT-Abtastung und Bildintegration zeigten die Start- und Klemmenpositionsfehler sowie die Winkelabweichung. Bitte überprüfen Sie die Position mit der Sonde, um sicherzustellen, dass das Bohren korrekt ist. Wir können auch unser spezielles manuelles Werkzeug verwenden, um die Entfernung vom Startpunkt zum Zielpunkt zu messen.Dieses Protokoll ermutigt Forscher, das Feld weiter zu erforschen. Immer mehr Forschungsteams versuchen, neue Navigationssysteme für Kiefer- und Zahnimplantatoperationen zu entwickeln, um die Genauigkeit und Effizienz zu erhöhen und die Lernkurve zu verkürzen.
