メラノーマは、しばしば他の皮膚部位(皮下転移)だけでなく、リンパ節、肺、肝臓、脳、骨にも広がる攻撃的な形態の皮膚癌です1。過去10年間で、新薬が臨床現場に導入され、転移性黒色腫患者の平均余命の改善に貢献してきました。しかし、応答までの時間や程度の変動、重篤な副作用、獲得した抵抗の反乱など、限界が残っています1。したがって、転移性の広がりを初期段階、すなわち局所リンパ節に到達したときに検出することが重要です。
局所リンパ節(センチネルリンパ節)の生検は、通常、黒色腫細胞の存在を確認するために行われる。しかし、超音波イメージングは、臨床評価を凌駕し、不必要な生検を回避するのに役立つため、転移性関与を検出する非侵襲的な方法として定着しています2,3,4。さらに、超音波画像は、リンパ節の監視、特に高齢および/または併存疾患の場合に適切であると思われる5,6。超音波分析によって検出され、正常リンパ節と転移性リンパ節の区別を可能にする特徴は、サイズ(体積)の増加、楕円形から円形への形状の変化、不規則なマージン、エコー発生パターンの変化、および変化した(増加した)血管新生7。
Tyr::CreER+,BrafCA/+,Ptenlox/lox遺伝子操作マウス(Braf/Ptenマウス)は、転移性黒色腫の組織特異的で誘導可能なモデルとして最近科学界に利用可能になりました8。この動物モデルでは、原発性腫瘍は非常に迅速に発症する:野生型(wt)BrafからBrafV600Eへの切り替えおよびPtenの喪失の誘導後2〜3週間以内に目に見えるようになり、4週間以内に50〜100mm3の体積に達する。次の2週間で、原発性腫瘍の増殖は、他の皮膚部位、リンパ節、および肺における転移性負荷の漸進的な増加を伴う。
Braf/Ptenマウスは、メラノーマ発生に関与するシグナル伝達経路の解剖9,10、起源のメラノーマ細胞の同定11,12,13、標的療法と免疫療法の両方の観点からの新しい治療選択肢の試験など、複数の目的で広く使用されています8,14,15,16。.具体的には、Braf/Ptenマウスを用いて、弱毒化リステリア・モノサイトゲネス(Lmat)が抗黒色腫ワクチンとして作用することを実証した。治療環境で全身投与した場合、Lmatは腫瘍部位に選択的に蓄積するため、全体的な毒性とは関連しない。さらに、原発性黒色腫量の顕著な減少およびリンパ節および肺における転移性負荷の減少を引き起こす。分子レベルでは、Lmatはメラノーマ細胞のアポトーシス死滅を引き起こし、これは少なくとも部分的には、非細胞自律活性(CD4+およびCD8+ Tリンパ球の部位での動員)に起因する16。

我々の方法は、鼠径リンパ節の体積の増加を測定することによって、鼠径リンパ節の転移性関与の増加を監視することを可能にする。この非侵襲的方法は、非常に忍容性が高いため、2週間にわたって同じ実験動物に対して複数の画像化セッションをスケジュールすることができる。この方法は、転移性疾患に対する薬理学的治療の影響を評価するのに理想的である。我々はこの方法をBraf/Ptenメラノーマモデルに適用したが、原理的には転移性癌の様々な前臨床マウスモデルにおける様々な薬剤の有効性を評価するために適応させることができる。この手順を実演するのは、私の研究室のポスドクであるMarianna Vitiello博士と、IFC-CNRのフェロー研究者であるClaudia Kusmic博士です。メラノーマ誘発のために、3マイクロリットルの5ミリモルの4-ヒドロキシタモキシフェンを約1平方センチメートルの背中上部の剃毛皮膚に3日間連続して塗布する。皮膚の塗装から6週間後、ノギスを使用して原発性腫瘍、そして最終的には皮下転移の体積を測定します。次に、超音波画像を使用して、鼠径リンパ節の体積を測定する。原発巣およびリンパ節を目視検査により分析した後、組織学的研究のためにそれらを切除する。麻酔を確認した後、動物を超高周波超音波画像ステーションの加熱ボードに移し、動物を仰臥位に保ちます。ワセリンで潤滑された直腸プローブを使用してボード温度を調整し、マウスの体温を生理学的範囲に維持します。前足と後足を導電性ペーストでコーティングし、ボードに埋め込まれたECGプレート電極にテープで貼り付けます。生理学的パラメータが正しく取得され、表示されていることを確認してください。脱毛剤を塗布して両方の鼠径部から脱毛し、音響結合媒体でそれらをコーティングする。次に、超高周波超音波リニアプローブをイメージングステーションに埋め込まれた特殊な3Dモータにクランプし、プローブの自動的かつ段階的な移動を可能にします。超音波プローブの位置を適切に向けて調整し、鼠径リンパ節のショートアクセス画像を取得し、関心領域を焦点ゾーンに配置します。スキャン距離を2~5ミリメートル、ステップサイズを44マイクロメートルに設定し、リンパ節スライスあたり46~114スキャンステップ、動物1匹あたり1~3分の取得時間を設定します。鼠径リンパ節の全体積を一連の2D Bモード画像としてスキャンし、マイクロメートルスケールのステップサイズでトランスデューサを直線的に動かすことによって複数のレベルで画像を取得し、呼吸および心臓依存性シネループに関する3Dデータを生成します。イメージング後、取得した画像を生の形式でデジタル保存し、さらにオフラインで分析します。超音波画像の後処理のために、3D鼠径リンパ節画像のデータセットを開き、マルチスライス法を選択し、スタートボタンを押して、3D取得中にキャプチャされた各画像に対応するすべてのフレームの現在のフレームとサムネイルの両方を視覚化します。リンパ節が明らかな最初のフレームのサムネールを選択して、輪郭ビューにロードします。輪郭ビューで、マウスを左クリックしてリンパ節の境界に沿ってポイントをドロップします。希望のポイント数を設定したら、右になめて輪郭を完成させます。最初の輪郭が完成したら、サムネイルビューを使用して、輪郭を描く次の画像を選択します。必要に応じて、輪郭の間の複数の画像をスキップして、各 3D ボリュームに必要な手動トレースの数を減らします。ボリューム全体が輪郭を描くまでこのプロセスを繰り返し、次に[完了]をクリックします。3Dモードウィンドウのワークスペースで、画像表示領域の下にある体積測定アイコンをクリックしてリンパ節体積の数値を視覚化し、リンパ節体積の3Dレンダリングで表面ビューをアクティブにします。マウスを4-ヒドロキシタモキシフェンで処理すると、CreER酵素活性が誘導され、ゲノムレベルで野生型ブラフから変異型ブラフV600Eへの切り替えを引き起こし、Ptenは失われる。4-ヒドロキシタモキシフェン治療の4週間後、原発性腫瘍は50〜100立方ミリメートルの体積に達し、それらの成長はノギスによってさらに2週間測定することができ、それによって鼠径リンパ節で観察される色素沈着の漸進的な増加を示す。メラニン沈着は、メラノーマ抗原MelanAおよびBraf V600Eの免疫組織化学染色によって確認されるように、転移したメラノーマ細胞の存在に常に起因する。セグメンテーション段階の終わりに、すべての切片はリンパ節の外部輪郭のオーバーレイを示した。4-HT治療後4、5、および6週間で分析された右鼠径リンパ節の3Dレンダリングをここに示します。また、同じ動物の左右の鼠径リンパ節の経時的な体積の増加も定量化した。超音波イメージングと適切な免疫組織化学染色を組み合わせて、がん細胞の存在を分子レベルで確認することをお勧めします。リンパ節に加えて、皮下転移の体積は、超音波画像を用いて経時的に監視することもできる。2週間の時間枠が短すぎて研究中の薬物の効果を理解できない場合は、より末梢誘導部位または腫瘍を起こしにくい遺伝子型を選択する。
