Het gepresenteerde protocol beschrijft het experimentele beroertemodel van fototrombose door de intacte schedel te verlichten met een 561 nm laser, met een eerdere intraperitoneale injectie van Rose Bengal. Tot voor kort was het gebruik van dit model laag, maar neemt het gestaag toe.

Ischemische beroerte blijft een belangrijkste doodsoorzaak en verworven volwassen invaliditeit in ontwikkelde landen in de 21e eeuw goed voor ongeveer 2,7 miljoen sterfgevallen in 2017 wereldwijd1. Zelfs met de enorme inspanningen van de wetenschappelijke gemeenschap zijn er weinig behandelingen beschikbaar. Bovendien zijn deze reeds beperkte opties met zulke hoge uitsluitingscriteria niet toegankelijk voor veel patiënten, wat resulteert in een dringende behoefte aan nieuwe behandelingen om het functionele herstel na een beroerte te verbeteren.
Gezien de onbekwaamheid van in vitro modellen om de complexe interacties van de hersenen te repliceren, zijn diermodellen essentieel voor preklinisch beroerteonderzoek. Muizen zijn het meest gebruikte diermodel op het gebied van beroerteonderzoek. De meeste van deze muismodellen zijn gericht op het induceren van infarcten door het blokkeren van de bloedstroom in de middelste cerebrale slagader (MCA), omdat de meerderheid van de menselijke beroerte laesies zich in het MCA-gebiedbevinden 2. Hoewel deze modellen de laesies van menselijke beroertes beter samenvatten, omvatten ze geconvuleerde operaties met een hoge variabiliteit van het infarctvolume.
Sinds Rosenblum en El-Sabban's voorstel van het fototrombotische model in 19773, en later de toepassing van dit model op ratten Watson et al.4, is het op grote schaal gebruikt in ischemisch beroerteonderzoek5,6. Het fototrombotische beroertemodel induceert een lokaal en gedefinieerd corticale infarct als gevolg van de fotoactivatie van een lichtgevoelige kleurstof die eerder in de bloedstroom werd geïnjecteerd. Dit veroorzaakt lokale trombose van de vaten in de gebieden die aan licht worden blootgesteld. Kortom, bij blootstelling aan licht van de geïnjecteerde lichtgevoelige kleurstof wordt gelokaliseerd oxidatief letsel van het endotheelcelmembraan geïnduceerd, wat leidt tot trombocytenaggregatie en trombusvorming, gevolgd door lokale verstoring van de cerebrale bloedstroom7.
Het belangrijkste voordeel van deze techniek ligt in de eenvoud van uitvoering en de mogelijkheid om de laesie naar het gewenste gebied te leiden. In tegenstelling tot andere experimentele beroertemodellen is kleine chirurgische expertise nodig om het fototrombotische beroertemodel uit te voeren, omdat de laesie wordt geïnduceerd door verlichting van de intacte schedel. Bovendien kunnen de goed afgebakende grenzen ( figuur 2A en figuur 5B) en de flexibiliteit om de laesie naar een specifiek hersengebied te induceren, de studie van cellulaire reacties binnen het ischemische of intacte corticale gebiedvergemakkelijken 8. Om deze redenen is deze aanpak geschikt voor de studie van cellulaire en moleculaire mechanismen van corticale plasticiteit.
In de afgelopen decennia is de groeiende bezorgdheid over het gebrek aan reproduceerbaarheid tussen onderzoeksgroepen bedacht als de zogenaamde replicatiecrisis9. Na de coördinatie van de eerste preklinische gerandomiseerde gecontroleerde multicenter studie in 201510, een voorgesteld instrument om preklinisch onderzoek11,12,13te verbeteren , werd bevestigd dat een van de oorzaken van falende reproduceerbaarheid tussen preklinische studies van onafhankelijke laboratoria het ontbreken van voldoende standaardisatie van experimentele beroertemodellen en uitkomstparameters14was . Toen het ImmunoStroke-consortium werd opgericht (https://immunostroke.de/), een samenwerking die tot doel heeft hersen-immuuninteracties te begrijpen die ten grondslag liggen aan de mechanistische principes van beroerteherstel, was de standaardisatie van alle experimentele beroertemodellen tussen elke onderzoeksgroep essentieel.
Hier wordt de gestandaardiseerde procedure beschreven voor de inductie van het fototrombotische model zoals gebruikt in het bovengenoemde onderzoeksconsortium. Kortom, een dier onderging verdoving, kreeg intraperitonaal een Rose Bengal injectie (10 μL/g) en de intacte schedel, 3 mm links van bregma, werd onmiddellijk verlicht door een 561 nm laser gedurende 20 min (Figuur 1). Bovendien wordt een gerelateerde histologische en gedragsmatige methode gerapporteerd om het resultaat van de beroerte in dit model te analyseren. Alle methoden zijn gebaseerd op standaard operationele procedures die in het laboratorium zijn ontwikkeld en gebruikt.

Beroerte is een belangrijke doodsoorzaak en verworven volwassen invaliditeit in ontwikkelde landen. Ondanks uitgebreid onderzoek naar nieuwe therapeutische strategieën, blijven er beperkte therapeutische opties voor beroertepatiënten. Daarom is meer onderzoek nodig voor pathofysiologische trajecten zoals post-beroerte ontsteking, angiogenese, neuronale plasticiteit en regeneratie. Gezien het onvermogen van in vitro modellen om de complexiteit van de hersenen te reproduceren, zijn experimentele beroertemodellen essentieel voor de analyse en daaropvolgende evaluatie van nieuwe medicijndoelen voor deze mechanismen. Bovendien zijn gedetailleerde gestandaardiseerde modellen voor alle procedures dringend nodig om de zogenaamde replicatiecrisis te overwinnen. Als een inspanning binnen het immunostroke onderzoeksconsortium wordt een gestandaardiseerd fototrombotisch muismodel beschreven met behulp van een intraperitoneale injectie van Rose Bengal en de verlichting van de intacte schedel met een 561 nm-laser. Dit model maakt de prestaties van beroerte bij muizen mogelijk met toewijzing aan elk corticale gebied van de hersenen zonder invasieve chirurgie; dus, waardoor de studie van beroerte in verschillende gebieden van de hersenen mogelijk is. In deze video worden de chirurgische methoden van beroerte-inductie in het fototrombotische model samen met histologische analyse gedemonstreerd.

Het hier beschreven model is een fototrombotisch beroertemodel van Rose Bengalen injectie en intacte schedelverlichting gedurende 20 minuten, bij een constante golflengte van 561 nm en een uitgangsvermogen van 25 mW bij de vezel. Hoewel de volledige fototrombotische operatie 30 minuten duurt, wordt het dier onder lage anesthesie gehouden en is de hersenbeschadiging matig. Ongeveer 10 minuten na overdracht naar hun kooien waren alle dieren wakker, vrij bewegend in de kooi en interactie met nestgenoten.

De fototrombose beroerte model benadering kan door geschikt voor cellulaire en moleculaire studies van corticale plasticiteit vanwege de beperkte grenzen en flexibiliteit om letsel te induceren in verschillende gebieden van de cortex. Drie belangrijkste voordelen onderscheiden het fototrombosemodel van andere beroertemodellen, de mogelijkheid om de laesie naar het gewenste gebied te leiden, het is hoge reproduceerbaarheid en lage mortaliteit. Een goede rose bengalen dosering is van cruciaal belang voor het succes en voor de stabiliteit van het model.Resultaten zijn afhankelijk van technische specificaties zoals laservermogen, timing van kruistoepassingen en optische shilling van de niet-aangetaste cortex. Begin met het oplossen van rozenbengalen in 0,9% zoutoplossing voor een uiteindelijke concentratie van 10 milligram per milliliter. Steriliseer alle instrumenten zorgvuldig met behulp van een hot lead sterilisator en desinfecteer alle oppervlakken voor en na de operatie met een microbiële desinfecterende spray.Bereid een spuit gevuld met zoutoplossing voor om het operatiegebied gehydrateerd te houden en bereid het anesthesiegas voor. Meet het lichaamsgewicht van de muis om de dosis rozenbengalen aan te passen die moet worden geïnjecteerd. Stel de bijbehorende feedbackgestuurde verwarmingskussen in om de lichaamstemperatuur van de muis te handhaven.Zodra de muis volledig is verdoofd en in het stereotactische frame is bevestigd, plaatst u de rectale sonde voorzichtig om de lichaamstemperatuur van de muis tijdens de chirurgische procedure te controleren. Breng de Dex panthenol oogzalf aan op beide ogen en reinig de huid en de omliggende vacht met desinfecterend middel. Gebruik een schaar om een enkele longitudinale incisie van twee tot twee en een halve centimeter te maken en trek je terug om de schedel bloot te leggen.Gebruik katoen om het periosteum voorzichtig te verwijderen en de coronale hechtingen te lokaliseren. Draag een veiligheidsbril tijdens het inschakelen van de 561 nanometer laser en markeer de plus drie millimeter naar links, schakel vervolgens de laser uit en haak de sticker met een gat van vier millimeter diameter in de gemarkeerde coördinaten. Injecteer de muis intrapariëtaal met rozenbengaal.Plaats de laserstraal op vier tot vijf centimeter afstand van de schedel. Schakel de laser in en verlicht de schedel gedurende 20 minuten. Rehydrateer de schedel door twee druppels van 0,9% zoutoplossing aan te brengen.Na het hechten van de wond plaatst u de muis in een herstelkamer op 37 graden Celsius om te herstellen van anesthesie. Plaats de muis na een uur terug in de kooi in een temperatuur gecontroleerde kamer. De kristal violette vlek op seriële coronale hersensecties werden gebruikt voor de infarct volume entry analyse na 24 uur van de beroerte inductie.De variabiliteit van dit beroertemodel was laag en het gemiddelde infarctvolume was 29,3 kubieke millimeter, wat 23% van de ene hersenhelft vertegenwoordigt. De fototromboselaesie veroorzaakte een matige en langdurige sensorische motorische stoornis die wordt aangegeven door de samengestelde neuroscore. Beroertedieren hadden een significante verandering in de neuroscore 24 uur na de operatie.Hoewel de verschillen aanhielden, verbeterden de beroertemuizen in de loop van de tijd. Bij de muizen die werden blootgesteld aan de rozenbengalen plus verlichting, daalden het lichaamsgewicht en de lichaamstemperatuur 24 uur na de operatie. Binnen drie dagen na de operatie werd echter herstel waargenomen.Ischemische veranderingen werden bevestigd met behulp van laserbeeldvorming. De resultaten gaven aan dat de roze bengaalse of laserverlichting alleen geen laesie veroorzaakte, terwijl de gelijktijdige toepassing van beide een rond hypoperfusiegebied genereerde dat werd omgeven door de smalle oligomere zone. De kristalviolet- en tunnelkleuring voor de beoordeling van het infarctvolume 24 uur na de operatie onthulde geen weefselschade, noch in de rozenbengalen of laserverlichtingsoperaties.Aan de andere kant genereerde de rozenbengale plus laserverlichting een goed afgebakende laesie. Het is belangrijk om het lichaamsgewicht van het dier te meten om de roos bengaalse injectie aan te passen. De juiste conditie van lijkschouwers is belangrijk om een consistente laesie tussen dieren te induceren.Omdat dit protocol de schedel intact laat en zelfs kan worden uitgevoerd door schedelvensters, kan het worden gecombineerd met breed veld of multifoton in vivo beeldvorming.
