非外傷性くも膜下出血(SAH)は脳卒中の一種であり、全症例の約5%を占めています。非外傷性SAHの最も一般的な原因は、SAHの85%を占める動脈瘤(aSAH)の突然の破裂です。その他の原因としては、動脈静脈奇形の破裂、凝固障害、脳周囲出血における静脈の破裂などがあります1。発生率は10万人年あたり9人で、死亡率は約3人に1人、SAH 2,3後の日常生活の支援が必要です。
最初の安定化と診断の確認後、治療は出血の重症度に依存します。最も重症の患者は、脳内圧(ICP)を下げるために心室に脳室外ドレーンを挿入し、神経集中治療室に入院し、そこで綿密に監視されます。患者は血管造影を受けて(可能性のある)動脈瘤を特定し、その後、再出血を防ぐために動脈瘤をコイル状またはクリップします4。薬理学的療法の多数の試験にもかかわらず、カルシウムチャネル拮抗薬であるニモジピンだけが転帰を改善することを示しています5。現在、複数の臨床試験が進行中です。広範なリストについては、Daouらによるレビューを参照してください6。
動脈瘤の破裂は、これまでに経験した中で最悪の頭痛または雷鳴頭痛の突然の発症として説明されています。破裂により、ICPが急激に上昇し、続いて脳血流(CBF)が減少します。この減少は脳の全体的な虚血をもたらし、意識の喪失をもたらす可能性があります。このより機序的な経路は、血液の血管外漏出要素の開始された分解とともに、サイトカイン放出および自然免疫系の活性化を引き起こし、無菌の神経炎症をもたらす。さらに、血液脳関門の破壊は、脳浮腫およびイオン恒常性の乱れをもたらし、しばしば観察される。早期脳損傷(EBI)と呼ばれるこれらすべての変化は、最初の数日以内に発生し、ニューロンの喪失とアポトーシスをもたらします7。
aSAHに苦しむ患者の約1/3は、4日目から14日目の間に遅発性脳虚血(DCI)を発症します8。DCIは、発作や再出血などの他の原因が除外されている場合、焦点性、神経学的障害のデビュー、またはグラスゴー昏睡スケールで最低1時間続く最低2ポイントの低下のいずれかとして定義されます。DCIは、aSAH9後の死亡リスクの増加と機能的転帰の低下に関連しています。脳動脈の狭窄である脳血管れん縮(CVS)は、何十年にもわたってDCIに関連していることが知られており、以前はDCIの唯一の理由であると考えられていました。それ以来、CVSはDCIの発症なしに発生する可能性があることが示されており、微小血管血栓症および狭窄、皮質拡散抑制、およびEBIの炎症反応を含むより多くの要因がそれ以来同定されている10,11,12。
EBIとDCIは病気の経過と苦しんでいる患者の転帰に大きな影響を与えるため、動物モデルは再現性を持ちながら、これらを可能な限り模倣する必要があります。研究者は、マウスから非ヒト霊長類まで、さまざまな動物でさまざまなモデルを使用して、aSAHをシミュレートしようとしました。Sprague-DawleyおよびWistar野生型ラットは現在最も一般的に使用されている実験動物であり、最も一般的なモデルは血管内穿孔モデル、システルナ-マグナ二重注入モデル、そして最後に本稿13で説明する交叉前単回注入モデルである。
交叉前の単回注入モデルは、もともと他の実験モデルの欠点のいくつかに対抗するためにPrunellらによって開発されました14。手術は、習得すると再現性が高く、動物間のばらつきを最小限に抑えます。このモデルは、血液注入後のICPの突然の上昇を含む複数の点でヒトのSAHを模倣し、CBFの低下による一時的な全体的な虚血をもたらします15,16。それは、ヒトのほとんどのaSAHが発生する前循環に影響を及ぼします17。死亡率は、研究と注入された血液の量に応じて10%〜33%の範囲です14,18。遅延細胞死および神経炎症は2日目および7日目に検出することができ、それによってEBIおよびDCIの結果を研究するための変数を提供する19,20。
