尿路感染症(UTI)は、ヒトで最も一般的な細菌感染症の一つです。全女性のほぼ半数が生涯に対して症状のUTIを経験する1.膀胱に限られた感染症は、尿頻度の増加、緊急性、血尿、失禁、痛みなどの尿症状を引き起こす可能性があります。感染が上部尿路に上昇すると、患者は倦怠感、発熱、悪寒、背中の痛みで腎盂腎炎を発症する。さらに、UTI患者の最大20%が再発性感染症に苦しむことで、抗生物質感受性2、3、4が劇的に低下する。近年、再発性UTIの治療・予防に対する新しい治療法への関心が高まっています。下尿路の先天的および適応的免疫と、浸潤および植民地化に必要な細菌毒性因子の理解が良いにもかかわらず、治療体制における根本的な変化は毎日の泌尿器科的実践2に翻訳されていない。生体内でUTIの病因と感受性を研究するためには、再現性と非侵襲的なアッセイが不可欠です。
複数の動物のUTIモデルは線虫から霊長類までまで記述されてきたが、マウスモデルは主に5,6を用いる。このモデルは、(雌)マウスの経尿道カテーテル法及びそれに続く細菌懸濁液の植え付け、最も一般的には尿病性大腸菌(UPEC)からなる、膀胱管腔7に直接入る。接種後、細菌負荷は、コロニー形成単位(CFU)を決定することによって伝統的に定量化されてきた。この技術は、死後の臓器の均質化および連続希釈を得るために動物を犠牲にする必要があり、データ出力および再現性を制限する。また、個々の動物における細菌負荷の縦方向のフォローアップは、この技術を用いることは不可能である。
1995年、Tagtagらは生きている動物の病気プロセスを監視するために生物発光タグ付き病原体の使用を示唆した8,9.それ以来、髄膜イメージング(BLI)は、髄膜炎、心内膜炎、骨髄炎、皮膚、軟部組織感染症などの多数の感染モデルに適用されてきた。マウスUTIモデルでは、フォトルフドゥス・ルミネセンスから完全なルクス・オペロン(luxCDABE)を有するUPEC株を13個使用することができる。酵素反応は、酸素の存在下で還元フラビンモノヌクレオチドと反応する長鎖アルデヒドの酸化に依存する細菌ルシファーゼによって触媒され、酸化フラビン、長鎖脂肪酸及び光12を生じる。ルクスオペロンは、基質の合成に必要なルシファーゼおよび他の酵素をコードする。したがって、全ての代謝活性菌は、外因性基質12の注入を必要とせずに青色緑色(490nm)の光を連続的に放出する。ルクスタグ付き細菌によって放出される光子は、非常に敏感で冷却された電荷結合装置(CCD)カメラを使用して捕獲することができる。

本稿では、生体発光イメージングを用いた細菌負荷の解析とその後の縦方向インビボ解析を行うルクスオペロンを用いた泌尿器内細菌の脳内投与について説明する。

コロニー形成ユニットの列挙体は、UTI分野における研究の再現性を制限していた。生物発光イメージングは、膀胱生理学、UTI病態および感受性に関する生体内UTI研究に進む。生物発光イメージングは、感染の進化に関するリアルタイムの洞察を伴う縦方向のフォローアップを可能にする。さらに、必要な動物の数を大幅に減らします。生物発光イメージングは、ヒトで頻繁に起こる再発性または慢性感染症の研究を容易にする。さらに、研究者は、生物発光イメージングを使用して、上昇感染または血液への普及を同定することができます。単一コロニーを得る、 カナマイシン硫酸を補うLBプレート上の接種ループを有する細菌のグリセロールストックをストリークし、摂氏37度で一晩培養する。無菌14ミリリットルポリスチレンラウンドボトムチューブを充填し、仮名菌硫酸塩を加えたLBスープ5ミリリットルのデュアルポジションスナップキャップを使用します。接種ループを持つ単一の細菌コロニーを選び、これをLBスープに加えます。正しい混合を確実にするために10秒間渦。一晩で37°Cで開いた位置にスナップキャップを静かに培養します。インキュベーション後、チューブを10秒間ボルテックスし、細菌培養物の適切な均質化を確実にする。抗生物質を使わずに25ミリリットルの新鮮なLB培地に細菌懸濁液の25マイクロリットルを加えることによって、エルレンマイヤーでサブカルチャーを作る。一晩静かにアーレンマイヤーと文化を閉じます。設置日には、エルレンマイヤーの培養物を50ミリリットルの培養チューブと遠心分離機に注ぎます。上清をデカントし、滅菌PBSの10ミリリットルで細菌ペレットを再懸濁する。10秒間チューブを再び渦を出します。接種の実験濃度を選択し、標準曲線を使用して600ナノメートルで対応するODを決定します。再懸濁された細菌培養物の1ミリリットルを9ミリリットルの無菌PBSに加え、所望のOD 600ナノメートルに達するまで調整する。100マイクロリットルのシリンジに滅菌24ゲージのアンジオカテーテル先端を取り付けます。シリンジに調製した細菌溶液を充填します。1匹の動物を滑者の位置の作業面に置き、設置中に鼻コーンを使用して安定したイオブルラン麻酔を維持します。目の軟膏を適用します。穏やかな圧縮を適用し、上皮部に円形の動きをすることによって残留尿を排出し、その後、70%エタノールで下腹部をきれいにします。カテーテル先端を通常の生理食塩水で潤滑します。人差し指を腹部の非支配的な手に乗せ、そっと上に押します。尿道のカテーテル法を90度の角度で垂直に開始します。抵抗に遭遇したら、さらに挿入する前に水平に傾けます。細菌接種の50マイクロリットルの遅い設置を行います。取り付け後、注射器とカテーテルを数秒間所定の位置に保ち、漏れを防ぐためにゆっくりと引き込みます。実験グループごとに1カテーテルを使用してください。動物を鼻コーンの中のスペイン位置に配置してイメージングの準備をし、実験グループごとに1カテーテルを使用してイメージング用に準備します。必要に応じて、テキスト原稿に記載されているように、抗生物質または実験薬を投与する。BLI取得ソフトウェアを開き、イメージングデバイスで[初期化]をクリックして、カメラとステージコントローラシステムをテストし、充電結合デバイスカメラをマイナス90°Cまで冷却します。[取得]、[自動保存]をクリックします。励起フィルタをブロックに設定し、放出フィルタを開くに設定して、デフォルトの発光設定を確認します。最初の画像を撮影する際の露出時間を自動に設定します。インビボ測定と明るい信号の場合は、露出時間を約30秒に設定します。画像が飽和状態で警告が表示された場合は、露出時間を短縮します。ミディアムビン、F/ストップを選択し、正しい FOV を選択します。マウスを撮像する際の被写体の高さを1センチメートルに設定します。最大5マイル同時に画像を作成し、反射を防ぐためにライトバッフルを使用して動物を分離します。ドアを閉めて[取得]をクリックしてイメージングシーケンスを開始し、実験に関する詳細情報を入力します。マウスをイメージングチャンバーから取り出し、ケージに戻します。麻酔後に完全に回復していることを確認し、次のイメージングサイクルまでケージを換気されたラックに戻します。イメージングソフトウェアを起動し、参照をクリックして実験ファイルをロードします。ツール パレットを使用して、イメージのカラー スケールを調整します。ROI ツールを使用して、画像に関心のある領域を描画し、全体の領域をカバーするのに十分な大きさを確保し、すべての画像に同じサイズを使用します。次にROI測定をクリックして、光強度を定量化します。その後の設置後に得られたマウスの画像は、20,000コロニー形成ユニットを超えて強固に検出可能であり、インノキュラムのコロニー形成ユニットと生体発光in vivoとの間に線形相関が確立されたことを示した。尿路感染症の自然進化は、治療を受けていない対照群で研究された。抗生物質治療が感染動態に及ぼす影響を、抗生物質処理動物群で詳細に可視化した。全光子フラックスで測定した細菌負荷の即時減少は、エンロフロキサシンの最初の投与後に見られた。治療された動物のどれも細菌負荷のその後の上昇を持っていなかった。各動物の全体的な細菌負荷は、丸太の曲線下の領域を用いて計算し、全光子フラックスを変換し、10日間にわたってエンロフロキサシンと未治療動物で処理された動物との間に有意な差を示した。これらの結果は、BLIを使用してUTI感染キネティクスの違いを評価することができるという概念実証を提供する。生物発光イメージングは、細菌や生化学的分析や実験のための尿サンプルの定期的な収集など、他の方法と組み合わせることができる非侵襲的な技術です。ここでは、生物発光イメージングが尿路感染症の疾患経過に関する新しい治療戦略を評価するための強力なツールであることを実証しました。
