この手法は、前後の研究デザインを実装する研究や、特定の集団を特徴付けたい研究に特に適しています。主な利点は、この手法が比較的費用効果が高く、ほとんど痛みがないことです。同じ日に同じ参加者に対して複数の生検を行うことができます。この技術は、脂肪組織生理学の理解、特に疾患および代謝機能の理解を助けることができる。まず、実験室に、清潔で拭き取り可能な非多孔質の表面を備えた適切な個室と、参加者が仰臥位で横たわることができる清潔で快適なベッドがあることを確認してください。次に、70%エタノールスプレーと清潔なペーパータオルを使用して、生検手順に必要なすべての表面をきれいにします。次に、生検が行われる場所と生検を受ける人の手の届くところに適切な鋭利な使い捨てビンとバイオハザードウエストバッグを置きます。参加者が研究室に到着する前に、洗浄したばかりの一般医療用トロリーに手順に必要な機器を設置します。書面によるインフォームドコンセントを取得した後、参加者に書面によるアンケートに記入して、この手順で使用される材料にアレルギーがないことを確認するように依頼します。参加者の身元を口頭で確認した後、参加者が実行する手順と、あざ、痛み、感染症などの潜在的な二次的副作用を理解していることを確認してください。書面によるインフォームドコンセントに加えて、参加者から口頭での同意も取ります。次に、麻酔薬と生検の投与がどのように感じるかを強調して、参加者に手順を説明します。また、参加者が続行に慣れていることを確認してください。繰り返しになりますが、参加者が局所麻酔薬、特定の金属、甲殻類にアレルギーがないこと、および参加者がいかなる形の抗凝固薬を服用していないことを確認してください。参加者が仰臥位に横たわったら、臍の外側約5〜10センチメートルの生検部位を特定します。次に、標準的な医療ガイドラインに従って石鹸と温水で手を洗ってください。また、清潔なトロリーまたは作業エリアに滅菌シートを置き、シートの外縁にのみ触れるように注意します。適切な無菌技術を使用して滅菌手術用手袋を着用してください。次に、アシスタントに残りの機器を開いてもらい、触れたり汚染したりすることなく、準備された滅菌シートに落とすようにします。滅菌包装からアイテムを取り出すときは、アシスタントがアイテムに触れないようにしてください。次に、少量のヨウ素ベースの溶液を滅菌ガーゼに分注するようにアシスタントに指示します。次に、提案された生検部位から外側に移動するらせん状に皮膚を消毒します。余分な液体がある場合は、新鮮な滅菌ガーゼで拭いて取り除きます。局所麻酔バイアルの内容と有効期限をアシスタントに口頭で確認します。次に、開いたバイアルを逆さまに保持するようにアシスタントに指示します。そして、21ゲージの針を使用して、5ミリリットルの局所麻酔薬を注射器に引き込みます。針を鋭いスピンに廃棄し、シリンジに気泡がないことを確認します。次に、26ゲージの針をシリンジに当て、気泡を排出します。次に、腹部の皮膚と脂肪組織を腹壁から離して静かにつまみ、皮膚の表面に対して10度以下の角度で針を皮下組織に水平に挿入します。次に、シリンジのプランジャーとさらに0.5ミリリットルを引き出します。シリンジに血液が現れた場合は、針を引き抜いて別の角度で再挿入し、直径2〜4ミリメートルのブレブを上げて挿入領域に麻酔をかけます。針を皮下組織に進め、1ミリリットルのリドカインを扇形のパターンで投与します。26ゲージの針を取り外して廃棄します。次に、21ゲージの針をシリンジに当て、残りの4ミリリットルのリドカインを扇形のパターンで投与します。滅菌メスを使用して5分後、生検領域をそっと突っ込んで局所麻酔薬が有効になったことを確認し、麻酔領域の境界を特定します。その後、さらに1〜2分待ってから再評価します。麻酔が効いていることを確認したら、皮膚と脂肪組織をそっとつまんで、皮膚に1〜2ミリメートルの小さな穴を開けます。次に、14ゲージの針を5ミリリットルまたは10ミリリットルのシリンジに当てます。次に、皮膚と脂肪組織をそっとつまみながら、皮膚の表面に対して10度以下の角度で、麻酔領域のほぼ中央にある脂肪組織に穿刺を通して針をそっと挿入します。プランジャーを約2.5ミリリットルのマークまで引き出して吸引します。次に、針を素早く、前後に動かして脂肪組織の断片をスライスすることにより、生検を収集します。30秒後、針とシリンジを90度にひねり、この手順を繰り返して脂肪組織の断片を分解し、吸引によってシリンジに吸引します。約1分後、針を外し、シリンジの内容物をウェインボートを覆うガーゼの層に空にして、血液の飛び散りを防ぐために針の内腔が下を向いていることを確認します。示されているように、生検収集の手順を最大3回繰り返し、毎回手順を繰り返す前に、参加者が続行することに満足していることを確認してください。追加のサンプルを収集しながら、分析または保存のために以前に収集されたサンプルを処理して準備するようにアシスタントに指示します。満足のいくサンプルを収集したら、穿刺創の上に1〜2層の滅菌ガーゼを置きます。次に、保冷剤を置き、約10分間しっかりと圧力をかけ、恒常性を誘導します。恒常性が起こったら、滅菌ガーゼでヨウ素ベースの溶液または乾燥した血液を拭き取り、吸収パッドを備えた接着剤創傷被覆材を部位に塗布します。参加者が気分が良いことを確認した後、生検サイトのアフターケアについて口頭および書面による指示を提供し、参加者が今後数日間打撲傷を示す可能性が高いことを強調します。使用済みの鋭利物や汚染された材料を指定された鋭利物または臨床廃棄物容器に片付け、70%エタノールを使用して生検手順で使用されるすべての表面を洗浄し、使い捨ておよび非使い捨ての寝具をそれぞれ適切な臨床バッグに入れるか、廃棄または洗浄します。この研究では、39人の皮下脂肪組織生検が11人の健康な正常体重の女性で実施されました。この手順を使用して、平均192ミリグラムのサンプルが得られた。臍に近すぎる生検部位を選択すると、不必要に広範囲の打撲傷につながる可能性があります。脂肪組織生検後に参加者が経験した合併症のリストがここに要約されています。針が筋肉のより深い層に穴を開けたり、皮膚の反対側から穿孔したりしないように、常に皮膚をつまんでいることを確認してください。練習は、実際の参加者でそれを試みる前に、訓練を受けた医師によってトレーニングが実行されることを完璧に保証します。また、脂肪組織を破壊するために針を挿入するときは、生検を行う人がサンプルを分解するのに十分確実であることを確認してください。
