Dit protocol stelt elke onderzoeker in staat om de antitumoractiviteit en enkele van de onderliggende mechanismen van immunomodulerende geneesmiddelen in een in vivo tumormuismodelsysteem te bestuderen. Hier werd een syngeneisch subcutaan tumormodel gebruikt, dat verschillende voordelen heeft ten opzichte van orthotopische modellen, waaronder het technisch eenvoudige protocol, eenvoudige monitoring van tumorgroei, minder morbiditeit bij dieren en hogere produceerbaarheid. Een van de opkomende gebieden van kankerimmunotherapie is het gebruik van inflammatoire cytokines om het immuunsysteem van patiënten tegen hun tumorcellen te activeren. Verschillende pro-inflammatoire cytokines (d.w.z. interferon-alfa (IFNα), interleukine-2 (IL-2) en interleukine-1 (IL-1)) kunnen een significante antitumorimmuniteit opbouwen, wat interesse heeft gewekt in het onderzoeken van de antitumoreigenschappen en de veiligheid van op cytokine gebaseerde geneesmiddelen. Interleukine-1 alfa (IL-1α) in het bijzonder, is een pro-inflammatoir cytokine dat bekend staat als het hoofdcytokine van ontsteking1. Sinds de ontdekking van dit cytokine in de late jaren 1970, is het onderzocht als een middel tegen kanker en een hematopoëtisch medicijn om de negatieve effecten van chemotherapie te behandelen2. Tijdens de late jaren 1980 werden verschillende preklinische en klinische studies uitgevoerd om de antikankereffecten van IL-1α 3,4,5,6 te bepalen. Deze studies vonden veelbelovende antitumoractiviteit van recombinant IL-1α (rIL-1α) tegen melanoom, niercelcarcinoom en ovariumcarcinoom. Toxiciteiten, waaronder koorts, misselijkheid, braken, griepachtige symptomen en de meest ernstig dosisbeperkende hypotensie werden echter vaak waargenomen. Helaas hebben deze dosisgerelateerde toxiciteiten het enthousiasme voor verder klinisch gebruik van rIL-1α getemperd.
Om te proberen het kritieke probleem van IL-1α-gemedieerde toxiciteiten aan te pakken, zullen polyanhydride nanodeeltjes (NP) formuleringen worden onderzocht die de gecontroleerde afgifte van IL-1α door oppervlakte-erosiekinetiek mogelijk maken. Deze NP-formuleringen zijn bedoeld om de voordelen van de antitumoreigenschappen van IL-1α te plukken en tegelijkertijd dosisbeperkende bijwerkingen te verminderen7. Polyanhydriden zijn door de FDA goedgekeurde polymeren die afbreken door oppervlakte-erosie, wat resulteert in bijna nul-orde afgifte van ingekapselde middelen 8,9,10,11,12. Amfifiele polyanhydride copolymeren die 1,8-bis-(p-carboxyfenoxy)-3,6-dioxaoctane (CPTEG) en 1,6-bis-(p-carboxyfenoxy)hexaan (CPH) bevatten, zijn uitstekende toedieningssystemen gebleken voor verschillende payloads in oncologisch en immunologisch onderzoek 8,12. In het volgende protocol zullen 20:80 CPTEG:CPH NP's geladen met 1,5 wt.% rIL-1α (IL-1α-NP's) worden gebruikt om de antitumoractiviteit en toxiciteit van dit cytokine te bestuderen in een muismodel van HNSCC.
Het algemene doel van de volgende procedures is om de antitumoractiviteit van IL-1α-NP's op HNSCCs te beoordelen. De beschreven procedures, waaronder het beoordelen van tumorgroei en overleving, kunnen worden toegepast op elk immuunmodulerend middel van belang. Deze procedures moeten worden uitgevoerd in een syngene muismodel met een intact immuunsysteem13 om de klinische relevantie te maximaliseren. IL-1α-NP toxiciteit zal ook worden beoordeeld door veranderingen in circulerende niveaus van pro-inflammatoire cytokines en dierlijk gewicht te meten. Er zijn veel methoden om in vivo geneesmiddeltoxiciteit te bepalen; de meest gebruikte methoden omvatten echter de meting van serumenzymen voor orgaantoxiciteit en histologische veranderingen in die organen. Om histologische analyses uit te voeren, moet het dier echter worden opgeofferd, wat de overlevingscurves van het experiment zal beïnvloeden. Daarom zal dit protocol een protocol bevatten voor het verzamelen van bloed van levende muizen voor het meten van cytokines in serummonsters. Het verzamelde serum kan worden gebruikt voor het meten van gewenste serumanalyten voor orgaantoxiciteit. Multicolor flowcytometrie zal worden gebruikt om de veranderingen in de immuuncelpopulatie in de micro-omgeving van de tumor en de migratie van immuuncellen naar de lymfeklier te begrijpen. Andere methoden kunnen worden gebruikt om immuuncellen te identificeren, waaronder immunohistochemie en/of immunofluorescentie van geconserveerde rubrieken14. Deze technieken kunnen echter tijdrovend en vervelend zijn om uit te voeren op een groot aantal dieren. Over het algemeen zullen de volgende methoden onderzoekers in staat stellen om de antitumor immuunrespons en potentiële mechanismen van immunostimulerende middelen voor kankerbehandeling te bestuderen.

Cytokinetherapie is een veelbelovende immunotherapeutische strategie die robuuste antitumor immuunresponsen bij kankerpatiënten kan produceren. Het pro-inflammatoire cytokine interleukine-1 alfa (IL-1α) is geëvalueerd als een middel tegen kanker in verschillende preklinische en klinische studies. Dosisbeperkende toxiciteiten, waaronder griepachtige symptomen en hypotensie, hebben het enthousiasme voor deze therapeutische strategie echter getemperd. Op polyanhydride nanodeeltjes (NP) gebaseerde toediening van IL-1α zou in deze context een effectieve aanpak zijn, omdat dit een langzame en gecontroleerde afgifte van IL-1α systemisch mogelijk kan maken en tegelijkertijd toxische bijwerkingen kan verminderen. Hier wordt een analyse beschreven van de antitumoractiviteit van IL-1α-geladen polyanhydride NP's in een hoofd-hals plaveiselcelcarcinoom (HNSCC) syngene muismodel. Murine orofaryngeale epitheelcellen die stabiel HPV16 E6/E7 tot expressie brengen samen met hRAS- en luciferase (mEERL) cellen werden subcutaan geïnjecteerd in de rechterflank van C57BL/6J-muizen. Zodra tumoren 3-4 mm in elke richting bereikten, werd een 1,5% IL-1a - geladen 20:80 1,8-bis (p-carboxyfenoxy)-3,6-dioxaoctane:1,6-bis(p-carboxyphenoxy)hexaan (CPTEG: CPH) nanodeeltje (IL-1α-NP) formulering intraperitoneaal toegediend aan muizen. Tumorgrootte en lichaamsgewicht werden continu gemeten totdat tumorgrootte of gewichtsverlies euthanasiecriteria bereikte. Bloedmonsters werden genomen om antitumor immuunresponsen te evalueren door submandibulaire venapunctie, en inflammatoire cytokines werden gemeten door middel van cytokine multiplex assays. Tumor- en inguinale lymfeklieren werden gereseceerd en gehomogeniseerd tot een eencellige suspensie om verschillende immuuncellen te analyseren via multicolor flowcytometrie. Deze standaardmethoden zullen onderzoekers in staat stellen om de antitumor immuunrespons en het potentiële mechanisme van immunostimulerende NP's en andere immunotherapiemiddelen voor kankerbehandeling te bestuderen.

In deze studie werd de antitumoractiviteit van polyanhydride IL-1α in een syngene muismodel van HNSCC onderzocht. Recombinant IL-1α (rIL-1α) vertraagde significant de groei van mEERL-tumoren (figuur 1A), hoewel gewichtsverlies werd waargenomen bij de behandelde muizen, dat werd hersteld na stopzetting van de behandeling (figuur 1B).
