Denne protokollen vil tillate enhver etterforsker å studere antitumoraktiviteten og noen av de underliggende mekanismene til immunmodulerende legemidler i et in vivo tumormusemodellsystem. Et av de fremvoksende områdene av kreftimmunterapi er bruken av inflammatoriske cytokiner for å aktivere pasientens immunsystem mot deres tumorceller. Flere proinflammatoriske cytokiner (dvs. interferon-alfa (IFNα), interleukin-2 (IL-2) og interleukin-1 (IL-1)) kan montere signifikant antitumorimmunitet, noe som har generert interesse for å utforske antitumoregenskapene, samt sikkerheten til cytokinbaserte legemidler. Spesielt Interleukin-1 alfa (IL-1α) er et proinflammatorisk cytokin kjent som mestercytokinet av betennelse1. Siden oppdagelsen av dette cytokinet på slutten av 1970-tallet har det blitt undersøkt som et kreftmiddel samt et hematopoietisk legemiddel for å behandle de negative effektene av kjemoterapi2. I løpet av slutten av 1980-tallet ble det utført flere prekliniske og kliniske studier for å bestemme anticancer effekter av IL-1α 3,4,5,6. Disse studiene fant lovende antitumoraktivitet av rekombinant IL-1α (rIL-1α) mot melanom, nyrecellekarsinom og ovariekarsinom. Toksisitet, inkludert feber, kvalme, oppkast, influensalignende symptomer og mest alvorlig dosebegrensende hypotensjon ble imidlertid ofte observert. Dessverre dempet disse doserelaterte toksisitetene entusiasmen for videre klinisk bruk av rIL-1α.
For å forsøke å løse det kritiske problemet med IL-1a-mediert toksisitet, vil polyanhydrid nanopartikkel (NP) formuleringer som muliggjør kontrollert frigjøring av IL-1a ved overflateerosjonskinetikk bli undersøkt. Disse NP-formuleringene er ment å høste fordelene av antitumoregenskapene til IL-1α samtidig som dosebegrensende bivirkningerreduseres 7. Polyanhydrider er FDA-godkjente polymerer som nedbrytes gjennom overflateerosjon, noe som resulterer i nesten nullordens frigjøring av innkapslede midler 8,9,10,11,12. Amfifile polyanhydrid-kopolymerer som inneholder 1,8-bis-(p-karboksyphenoksy)-3,6-dioksaoktan (CPTEG) og 1,6-bis-(p-karboksyphenoksy) heksan (CPH), har blitt rapportert å være gode leveringssystemer for ulike nyttelaster i onkologi og immunologibasert forskning 8,12. I den følgende protokollen 20:80 CPTEG: CPH NP lastet med 1,5 vekt% rIL-1α (IL-1a-NP) vil bli brukt til å studere antitumoraktiviteten og toksisiteten til dette cytokinet i en musemodell av HNSCC.
Det overordnede målet med følgende prosedyrer er å vurdere antitumoraktiviteten til IL-1a-NP på HNSCCs. De beskrevne prosedyrene, inkludert vurdering av tumorvekst og overlevelse, kan brukes på ethvert immunmodulerende middel av interesse. Disse prosedyrene bør utføres i en syngen musemodell med et intakt immunsystem13 for å maksimere klinisk relevans. IL-1a-NP-toksisitet vil også bli vurdert ved å måle endringer i sirkulerende nivåer av proinflammatoriske cytokiner og dyrevekt. Det er mange metoder for å bestemme in vivo legemiddeltoksisitet; Imidlertid involverer de mest brukte metodene måling av serumenzymer for organtoksisitet og histologiske endringer i disse organene. For å utføre histologiske analyser må dyret imidlertid ofres, noe som vil påvirke forsøkets overlevelseskurver. Derfor vil denne protokollen inneholde en protokoll for innsamling av blod fra levende mus for måling av cytokiner i serumprøver. Det oppsamlede serumet kan brukes til måling av eventuelle ønskede serumanalytter for organtoksisitet. Flerfarget flowcytometri vil bli brukt til å forstå endringene i immuncellepopulasjonen i tumorens mikromiljø og immuncellemigrasjon til lymfeknuten. Andre metoder kan benyttes for å identifisere immunceller, inkludert immunhistokjemi og/eller immunfluorescens av konserverte avsnitt14. Imidlertid kan disse teknikkene være tidkrevende og kjedelige å utføre på et stort antall dyr. Samlet sett vil følgende metoder tillate etterforskere å studere antitumorimmunresponsen og potensielle mekanismer for immunostimulerende midler for kreftbehandling.

Cytokinbehandling er en lovende immunterapeutisk strategi som kan produsere robuste antitumorimmunresponser hos kreftpasienter. Det proinflammatoriske cytokinet interleukin-1 alfa (IL-1α) har blitt evaluert som et anticancermiddel i flere prekliniske og kliniske studier. Imidlertid har dosebegrensende toksisitet, inkludert influensalignende symptomer og hypotensjon, dempet entusiasmen for denne terapeutiske strategien. Polyanhydrid nanopartikkel (NP) -basert levering av IL-1α vil representere en effektiv tilnærming i denne sammenheng, da dette kan tillate en langsom og kontrollert frigjøring av IL-1a systemisk samtidig som toksiske bivirkninger reduseres. Murine orofaryngeale epitelceller som stabilt uttrykker HPV16 E6/E7 sammen med hRAS og luciferase (mEERL) celler ble injisert subkutant i høyre flanke av C57BL/6J mus. Når svulster nådde 3-4 mm i hvilken som helst retning, ble en 1,5% IL-1a - lastet 20:80 1,8-bis (p-karboksyphenoksy) -3,6-dioksaoktan: 1,6-bis (p-karboksyphenoksy) heksan (CPTEG: CPH) nanopartikkel (IL-1a-NP) formulering administrert til mus intraperitonealt. Tumorstørrelse og kroppsvekt ble kontinuerlig målt inntil tumorstørrelse eller vekttap nådde eutanasikriteriene. Blodprøver ble tatt for å evaluere antitumorimmunresponser ved submandibulær venepunktur, og inflammatoriske cytokiner ble målt gjennom cytokinmultipleksanalyser. I denne studien ble antitumoraktiviteten til polyanhydrid IL-1α i en syngen musemodell av HNSCC undersøkt.
