Het is cruciaal voor het verkrijgen van nauwkeurige gegevens - die de sensorische drempels van de hond weerspiegelen - dat de hond zo kalm, ontspannen en voldoende gepositioneerd is voor elke meting. Kwantitatieve sensorische testen (QST) beoordelen de reacties die worden opgewekt door extern toegepaste stimuli; Het wordt gebruikt om de functie van het somatosensorische systeem bij mens en dier te evalueren1. Mechanische stimuli in de vorm van punctaatdruk of diepe druk worden toegepast als een verhoogde stimulus. De sensorische drempel wordt bepaald als de kracht die een psychofysische reactie oproept1. Warme of koude thermische stimuli kunnen worden gebruikt als een opgevoerde stimulus of als een stimulus met een vaste intensiteit. De sensorische drempel wordt bepaald als de temperatuur waarbij er een respons is of de latentie om op de stimulus te reageren. Punctate druk sensorische drempels worden gemeten met behulp van elektronische von Frey anesthesiometers of von Frey haarfilamenten, diepe druk wordt gemeten met behulp van handheld druk algometers, en thermische sensorische drempels worden bepaald met behulp van een verscheidenheid van contact thermode systemen.
QST geeft informatie over het functioneren van zowel perifere als centrale sensorische paden en kan worden gebruikt om veranderingen in deze sensorische paden (algoplasticiteit) in verschillende ziekteprocessen te evalueren, met name die welke chronische pijn veroorzaken1. Meissner's bloedlichaampjes detecteren punctate druk, en het gevoel wordt overgedragen door Aβ afferente vezels op niet-schadelijke niveaus en Aδ afferente vezels wanneer de stimulus van een schadelijke intensiteit 1,2 is. Diepe druk wordt gedetecteerd door Pacinische bloedlichaampjes en overgedragen door C-afferente vezels, schadelijke warmte wordt gedetecteerd door Ruffini-bloedlichaampjes en overgedragen door Aδ- en C-afferente vezels, en schadelijke kou wordt gedetecteerd door Krause-bloedlichaampjes en overgedragen door C-afferente vezels 1,2. QST kan worden gebruikt om zowel remming (verminderde gevoeligheid, hypoesthesie) als facilitering (verhoogde gevoeligheid, hyperesthesie) van deze receptoren en routes te detecteren. Bij honden is QST gebruikt om veranderingen in sensorische drempels secundair aan acute dwarslaesie 3,4,5, Chiari-achtige misvorming en syringomyelie6, craniale kruisbandruptuur 5,7 en artrose (OA) 8,9,10 te evalueren. Bovendien hebben sommige studies QST gebruikt om pijnverlichting te beoordelen die wordt geboden door bepaalde pijnstillers 6,11,12,13 en chirurgische procedures 14. Deze studies hebben belangrijke inzichten opgeleverd in de mechanismen van pijnsensatie bij honden, zoals bewijs voor perifere en centrale sensibilisatie na een operatie en ziekten die chronische pijntoestanden veroorzaken, zoals craniale kruisbandruptuur en artrose. Deze informatie kan helpen bij het verbeteren van de detectie en behandeling van pijn bij honden.
Validatiestudies van mechanische en hete thermische QST bij honden hebben een goede haalbaarheid, herhaalbaarheid en betrouwbaarheid van QST-resultaten in de loop van de tijd aangetoond bij normale honden en honden met chronische pijn van OA 8,9,15,16. Verschillende studies hebben echter een slechte herhaalbaarheid en betrouwbaarheid van koude thermische en af en toe von Frey QST 1,15,17 gevonden. Deze studies gebruikten verschillende apparatuur en methodologie, maar leverden bewijs dat mechanische en hete thermische QST een nauwkeurige, semi-kwantitatieve methode is voor het meten van sensorische drempels bij honden. Aandacht voor precieze details, inclusief de instelling van de metingen, is echter van cruciaal belang voor het optimaliseren van QST bij honden, waardoor een gestandaardiseerd protocol voor QST nodig is. Sanchis-Mora et al. beschreven een sensorisch drempelonderzoeksprotocol (STEP) voor mechanische en warme en koude thermische QST, maar ondervonden problemen met honden die niet reageerden op de koude thermische QST of de hoogste gramkracht von Frey-filament die in de studie werd gebruikt17. Het volgende protocol biedt een standaardmethode voor mechanische en hete thermische QST bij honden; Dit protocol kan sensorische drempels beoordelen bij normale honden of honden met verschillende ziekteprocessen die het somatosensorische systeem beïnvloeden. De ontwikkeling van gestandaardiseerde protocollen kan het mogelijk maken om resultaten tussen studies en meta-analyses van gegevens te vergelijken om het nut van QST in de diergeneeskunde te verbeteren.

Kwantitatieve sensorische testen (QST) worden gebruikt om de functie van het somatosensorische systeem bij honden te evalueren door de respons op toegepaste mechanische en thermische stimuli te beoordelen. De QST-operator en -geleider moeten vertrouwd en comfortabel zijn met het omgaan met honden en het interpreteren van de gedragsreacties van honden op mogelijk pijnlijke stimuli om het eindpunt van het testen te bepalen, stress te verminderen en de veiligheid tijdens het testproces te behouden.

Mechanische en thermische QST is uitgevoerd om sensorische drempels te detecteren bij zowel onderzoeks- als klanteigen honden onder verschillende klinische omstandigheden, waaronder normale, gezonde honden, honden met chronisch pijnlijke aandoeningen zoals artrose, honden met acute dwarslaesie en om postoperatieve pijn en effectiviteit van analgetica te beoordelen.
