モントリオールコンセンサスは、胃食道逆流症(GERD)を「胃の内容物を逆流させると不快な症状や合併症を引き起こすときに発症する状態」と定義しています。食道狭窄、バレット食道、食道腺癌などの他の特定の合併症に関連している可能性があります。GERDは、主に経済的地位の高い国で、成人人口の約20%に影響を与えます1。
病理学的逆流(6%以上の酸暴露時間)の歩行pHモニタリングは、症状と酸性または非酸性胃食道逆流の関係を区別することを可能にする2,3.PPI(プロトンポンプ阻害剤)療法に反応しない患者では、pHモニタリングは病理学的胃食道逆流であるかどうか、そして患者が標準的なPPI療法に反応しない理由に答えることができる。現在、さまざまな pH およびインピーダンス モニタリング オプションが提供されています。新しい可能性の1つは、埋め込み型デバイス4,5を使用したワイヤレスモニタリングです。

2対の電極による電気刺激は、イヌ逆流モデル7におけるLESの収縮時間を増加させることが証明された。嚥下時の残留圧力を含むLESの弛緩は、低周波刺激と高周波刺激の両方によって影響を受けなかった。高周波刺激は、より少ない電力を必要とし、バッテリ寿命を延ばすので、明らかな選択です。
下部食道括約筋の電気刺激治療(ET)はGERD患者の治療において比較的新しい概念であるが、この治療法は安全で効果的であることが示された。この形態の治療は、PPI治療の必要性を排除し、食道酸暴露を減少させながら、GERDの症状から有意かつ永続的な救済を提供することが示されている8,9,10.
GERDの診断のための最新式のpHセンサーはBravoデバイス11,12です。推定体積1.7 cm3では、視覚的内視鏡フィードバックの有無にかかわらず食道に直接移植することができ、食道のpHを24時間+モニタリングします。

ここで実証した実験では、標準化されたプラスチックモデルに取り付けられた食道の長い部分を持つ豚の胃を使用しました。ESD(内視鏡的粘膜下膜解剖)、POEM(口腔内視筋切除術)、内視鏡粘膜切除(EMR)、止まり代などの内視鏡的手法を実践する一般的なモデルです。人間の臓器に近づく最も近い解剖学的パラメータについては、体重40〜50kgの豚の胃と食道を使用しました。
