In deze studie werd een in vitro model ontwikkeld om het effect van hyperoxie op microbiële longgemeenschappen te bestuderen. Dit model, gebaseerd op kunstmatig sputummedium en dagelijks sparen van serumflessen, handhaaft verhoogde zuurstofconcentraties en ondersteunt de groei van microben geïdentificeerd in sputum van pwCF.
Er zijn verschillende kritische stappen van deze aanpak. Ten eerste is de keuze om filtersterilisatie te gebruiken in plaats van warmtesterilisatie van het kuns...

Cystic fibrosis (CF) is een genetische ziekte die wordt gekenmerkt door een onvermogen om dik slijm uit de longen te verwijderen, wat leidt tot herhaalde infecties en progressieve achteruitgang van de longfunctie die vaak resulteert in de noodzaak van longtransplantatie of de dood. Het luchtwegmicrobioom van mensen met cystische fibrose (pwCF) lijkt de ziekteactiviteit te volgen1, met een vermindering van microbiële diversiteit geassocieerd met nadelige langetermijnresultaten2,3. In klinische studies van pwCF is aanvullende zuurstoftherapie in verband gebracht met meer geavanceerde ziekte4,5, hoewel traditioneel het gebruik van zuurstoftherapie wordt gezien als eenvoudigweg een marker voor de ernst van de ziekte6. Recente studies van een klinische studie van patiënten met respiratoire insufficiëntie hebben aangetoond dat hogere zuurstofniveaus van patiënten paradoxaal genoeg geassocieerd zijn met een toename van ernstige bacteriële infecties en hogere mortaliteit7, wat suggereert dat aanvullende zuurstof kan bijdragen aan de pathogenese van de ziekte. Het effect van aanvullende zuurstof op het cystic fibrosis longmicrobioom en de bijbehorende long- en luchtwegmicrobiële gemeenschappen is niet goed bestudeerd.
Mechanistische studies kunnen vaak niet rechtstreeks op menselijke proefpersonen worden uitgevoerd vanwege logistieke problemen en mogelijke ethische problemen in verband met interventies met onbekend medisch voordeel of schade. Translationele benaderingen die menselijke biospecimens integreren in modelsystemen kunnen in deze gevallen belangrijke biologische inzichten bieden. Hoewel het vermogen om zuurstof te gebruiken of te verdragen van oudsher een belangrijk onderdeel is van microbiële classificatie, is er weinig bekend over hoe de therapeutische introductie van aanvullende zuurstof in het milieu microbiële gemeenschappen in de luchtwegen zou kunnen verstoren. Om licht te werpen op de onbekende effecten van extra zuurstof op de luchtwegmicrobiomen van pwCF, moesten we twee grote uitdagingen aanpakken; ten eerste, de creatie van een kweekmedium dat fysiologisch de samenstelling van CF-sputum benadert; ten tweede, de creatie van een modelsysteem dat het mogelijk maakt om gedurende langere tijd verhoogde zuurstofconcentraties in cultuur te behouden.
Kunstmatige sputummedia (ASM) worden veel gebruikt om longsputum ex vivo8,9,10na te bootsen, maar er is geen duidelijke consensus over een specifiek recept. Dit protocol beschrijft een recept en bereidingsstrategie voor kunstmatig sputummedium, zorgvuldig ontworpen om sputum van pwCF fysiologisch te benaderen. Tabel 1 geeft een overzicht van de gekozen receptwaarden op basis van gepubliceerde literatuur. Chemische basiscomponenten en pH werden gematcht met waarden geïdentificeerd door studies van menselijk CF-sputum11,12,13. Fysiologische voedingsstoffen met een lage concentratie werden toegevoegd met behulp van eigeel, dat werd opgenomen als 0,25% van het uiteindelijke volume10, evenals vitamine- en sporenmetaalmengsels14,15. Mucine, het belangrijkste bestanddeel van sputum16, werd opgenomen bij 1% w / v14. Hoewel arbeidsintensiever, werd filtersterilisatie gekozen boven de meer conventionele praktijk van warmtesterilisatie om potentiële problemen door warmte-geïnduceerde denaturatie van essentiële mediacomponenten te verminderen10. Een bijkomend voordeel van filtersterilisatie is dat het media genereert die transparant zijn (warmtesterilisatie kan troebele media creëren als gevolg van precipitatie en coagulatie van zouten en eiwitten), waardoor dit kunstmatige sputummedium kan worden gebruikt om microbiële groei te volgen op basis van toename van troebelheid.
Dit modelsysteem voor de hyperoxische cultuur is gebaseerd op anaerobe kweektechnieken waarbij zuurstof wordt toegevoegd in plaats van verwijderd, waardoor een model ontstaat voor het effect van aanvullend zuurstofgebruik voor pwCF. Figuur 1 en het bijbehorende zuurstofsparende protocol schetst de componenten van een zuurstofspaarsysteem, dat tegen lage kosten kan worden verkregen bij leveranciers van algemene laboratoria en ziekenhuizen. Dit systeem maakt het mengen van gecomprimeerde zuurstof en lucht tot vaste concentraties variërend van 21% -100% zuurstof mogelijk. De integratie van een zuurstofsensor maakt het mogelijk om de concentratie van het uitgaande gasmengsel te verifiëren en de uitstroomgassamenstelling van eerder gespargeerde serumflessen te controleren om te controleren of de zuurstofcondities binnen het gewenste bereik zijn gehouden.
Dit protocol schetst procedures voor het creëren van een kunstmatig sputummedium, de constructie en het gebruik van een zuurstofspaarsysteem en de toepassing van beide om CF-sputum te kweken onder differentiële zuurstofomstandigheden.

design and development of a model to study the effect of supplemental oxygen on the cystic fibrosis airway microbiome

Van microbiële gemeenschappen in de luchtwegen wordt gedacht dat ze een belangrijke rol spelen bij de progressie van cystische fibrose (CF) en andere chronische longziekten. Microben zijn van oudsher geclassificeerd op basis van hun vermogen om zuurstof te gebruiken of te verdragen. Aanvullende zuurstof is een veel voorkomende medische therapie die wordt toegediend aan mensen met cystische fibrose (pwCF); bestaande studies over zuurstof en het luchtwegmicrobioom hebben zich echter gericht op hoe hypoxie (lage zuurstof) in plaats van hyperoxie (hoge zuurstof) de overwegend aerobe en facultatieve anaerobe microbiële longgemeenschappen beïnvloedt. Om deze kritieke kenniskloof aan te pakken, is dit protocol ontwikkeld met behulp van een kunstmatig sputummedium dat de samenstelling van sputum van pwCF nabootst. Het gebruik van filtersterilisatie, dat een transparant medium oplevert, maakt optische methoden mogelijk om de groei van eencellige microben in suspensieculturen te volgen. Om hyperoxische omstandigheden te creëren, maakt dit modelsysteem gebruik van gevestigde anaerobe kweektechnieken om hyperoxische omstandigheden te bestuderen; in plaats van zuurstof te verwijderen, wordt zuurstof aan culturen toegevoegd door dagelijks serumflessen te sparen met een mengsel van gecomprimeerde zuurstof en lucht. Sputum van 50 pwCF onderging dagelijks sparen gedurende een periode van 72 uur om het vermogen van dit model om differentiële zuurstofcondities te handhaven te verifiëren. Shotgun metagenomische sequencing werd uitgevoerd op gekweekte en niet-gekweekte sputummonsters van 11 pwCF om het vermogen van dit medium te verifiëren om de groei van commensale en pathogene microben te ondersteunen die vaak worden aangetroffen in cystic fibrosis sputum. Groeicurven werden verkregen uit 112 isolaten verkregen van pwCF om het vermogen van dit kunstmatige sputummedium te verifiëren om de groei van veel voorkomende cystische fibrose pathogenen te ondersteunen. We vinden dat dit model een breed scala aan pathogenen en commensalen in CF-sputum kan kweken, een gemeenschap herstelt die sterk lijkt op ongecultiveerd sputum onder normoxische omstandigheden en verschillende kweekfenotypen creëert onder verschillende zuurstofomstandigheden. Deze nieuwe aanpak kan leiden tot een beter begrip van onverwachte effecten veroorzaakt door het gebruik van zuurstof in pwCF op microbiële gemeenschappen in de luchtwegen en veel voorkomende respiratoire pathogenen.

Deze protocollen werden toegepast op 50 slijmoplossende sputummonsters van pwCF die zich presenteerden voor routinezorg aan een poliklinische cystic fibrosis-kliniek in het Massachusetts General Hospital in Boston, Massachusetts. Het spargingproces introduceert het vermogen om onder verschillende zuurstofomstandigheden te kweken.Veranderingen aan kunstmatig sputummedium maken het mogelijk om andere chronische longziekten na te bootsen en microbiële gemeenschappen te modelleren onder andere omstandigheden, zoals verschillende glucoseconcentraties bij gelijktijdige cystische fibrose en diabetespatiënten. Een overzicht van de stappen voor kunstmatige sputum medium bereiding toont het mengen van de bereide ASCM, ASMM en ASBM. De dweilbuffer wordt gebruikt om de pH tot 6,3 te titreren en filtersterilisatie van het medium wordt uitgevoerd in een koude ruimte op een orbitale shaker.Bereid om te beginnen een kunstmatig sputummedium door 250 milliliter ASCM toe te voegen aan de fles van één liter met ASMM. Voeg vervolgens 250 milliliter ASBM toe aan de middelgrote fles. Titreer het medium met een basisbuffer van één kies om een pH van 6,3 op een pH-papier te bereiken.Koel het kunstmatige sputummedium bij vier graden Celsius tot filtratie. Om vervolgens het filtratieproces te starten, brengt u 200 milliliter ongefilterd kunstmatig sputummedium over naar een vacuümfiltratiesysteem met een poriegroottefilter van 0,22 micrometer. Nadat u het filtratiesysteem op de vacuümpomp hebt aangesloten, schakelt u de vacuümpomp in en plaatst u de kamer op een orbitale shaker en schudt u met 90 omwentelingen per minuut in een koude kamer bij vier graden Celsius.Vervang het filter nadat 350 milliliter medium is gefilterd om het verstoppingsprobleem op te lossen. Nadat een aanzienlijke hoeveelheid is gefilterd, op een extra 150 milliliter medium voor filtratie. Herhaal de filtratie met extra kamers totdat alle media zijn gefilterd.Overzicht van zuurstofsparingen die de toevoeging van patiëntensputum aan kunstmatig sputummedium in een geautoclaveerde serumfles aantonen. Spaar vervolgens de afgesloten serumfles met een gasmengsel. Ten slotte wordt de serumfles geïncubeerd om kweek Alec Watts te verkrijgen met het interval van 24 uur voor daaropvolgende meta genomics-sequencing.Om te beginnen met het sparren van serumflessen, etiketteer 500 milliliter geautoclaveerde serumflessen met monsteridentificaties, datum en tijd van inenting en doelzuurstofpercentage. Voeg in een biologische kap 24 milliliter van het kunstmatige sputummedium toe aan elke serumfles die wordt opgezet. Om een voldoende monstervolume voor elke kweekconditie te verkrijgen, verdunt u indien nodig het monster met steriele fosfaat gebufferde zoutoplossing, vervolgens gehomogeniseerd sputum met een naald van 18 gauge en voegt u één milliliter van het patiëntensputum toe aan elke serumfles.Plaats vervolgens met behulp van een steriel pincet de geautoclaveerde rubberen stoppen op de bovenkant van elke serumfles, druk de rubberen stoppen naar beneden zonder de onderkant van de stop aan te raken. Nadat u de flessen van de kap hebt verwijderd, brengt u de aluminium afdichtingen aan en krimpt u deze. Verwijder het middelpunt van de afdichtingen en veeg de bovenkant van de flessen af met een alcoholdoekje en passeer de flesafdichting door een Bunsen-brandervlam.Bevestig nu een steriele naald van 18 gauge op een spuit met een plunjerlijst met een filter, steek eerst de aangebrachte gasafgiftespuit in de fles. Bevestig nu een steriele naald van 18 gauge op de gasafgifte van het systeem en steek de gasafgiftenaald ook in de fles. Leid de T-knooppunten vanuit de tanks door de zuurstofmonitor.Na het verifiëren van de beoogde zuurstofconcentratie die door het systeem stroomt, richt u zich op ongeveer vijf liter per minuut gasstroom. Leid de T-knooppunten van de tanks om naar de gasafgifte. Als het gas door de serumfles begint te stromen, let dan goed op de manometer en als de druk onverwacht toeneemt, sluit u het systeem onmiddellijk uit.Na het hardlopen sparren zuurstof gedurende één minuut door de serumfles met vijf liter per minuut, de instellingen maken 10 luchtuitwisselingen mogelijk en zorgen ervoor dat de interne atmosfeer de gewenste concentratie van 100% zuurstof bereikt. Verwijder de gasafgifte, 18 gauge naald. Nadat u de druk in de serumfles tot één atmosfeer hebt laten opbouwen, verwijdert u onmiddellijk de gasafgiftenaald.Incubeer de serumflessen in een incubator shaker van 37 graden Celsius met 150 rotaties per minuut gedurende intervallen van 3, 24 uur. Alec Watts worden genomen op elk 24-uurs interval voor downstream analyse en monsters worden elke keer gere resparged. Het voorbeeld hier laat zien hoe een cultuur eruit kan zien voor en na incubatie.De uitstroom zuurstof- en pH-waarden werden gemeten in de loop van het kweekproces voor sputummonsters die op 37 graden Celsius werden gehouden. Met beide spaarintervallen van 12 uur en 24 uur werden de zuurstofconcentraties in de loop van de tijd ongeveer gehandhaafd met een matige daling van de zuurstofconcentratie waargenomen voor alle drie de omstandigheden. De gemeten pH-waarden lagen binnen het fysiologische bereik zonder significante veranderingen in de loop van de tijd.De vergelijking van microbiële belasting, diversiteit en samenstelling van de gemeenschap tussen ongecultiveerd en gekweekt sputum onthulde een 20-voudige toename van microbiële belasting in gekweekt sputum. Diversiteitsstatistieken gaven het behoud van de samenstelling van de gemeenschap aan met minimale wereldwijde verschillen die door het cultuurproces werden geïntroduceerd. Vergelijkende analyse van de 120 microbiële soorten in gekweekt en ongecultiveerd sputum werd uitgevoerd door meta-genomische sequencing.46 van deze soorten werden geïdentificeerd in zowel ongekweekte als gekweekte monsters. Terwijl 35 uitsluitend werden geïdentificeerd in niet-gekweekte monsters en 39 uitsluitend werden geïdentificeerd in gekweekte monsters. Absorberende groeicurven werden gegenereerd voor veel voorkomende CF-longpathogenen gekweekt in kunstmatige sputummedia uit patiëntmonsterisolaten onder een normale zuurstofconcentratie van 21%.Geen verandering in de tijd in de optische dichtheid bij 600 nanometer in de ASM alleen negatieve controle wees op contaminatievrije cultuur. De typische waargenomen groeicurvepatronen geven aan dat ASM-medium kan worden gebruikt voor het genereren van groeicurven met behulp van optische methoden. De metagenomische sequencing wordt hier gebruikt om de samenstelling van microbiële gemeenschappen te beschrijven.Andere technieken zoals metatranscriptomische sequencing kunnen worden gebruikt om genexpressie of metabolomica te evalueren om de productie van metabolieten te evalueren.
