私たちの経験では、患者との詳細な議論は、将来のコンプライアンスを評価するために不可欠です。神経支配時間、運動学習能力、および患者の可用性に応じて、リハビリテーションプロセスは9〜15ヶ月かかる可能性が高い。患者が上肢機能の改善に努力していない、または別の装置(例えば、身体駆動義肢)をよりよく利用する可能性があるとします。その場合、時間的(そしておそらく財政的)コミットメントに価値があるとは思わないかもしれません。リソースを節約するために、手順に強い関心を表明し、完全なリハビリテーション手順が予想される場合にのみ機能目的で手術を行う患者のみを含めることを強くお勧めします。最後に、手術、治療、およびフィッティングの費用は、その時点でカバーされる可能性が高いです。

上肢の高い切断は、補綴物交換のための課題を提供する1.肘関節機能とは別に、アクティブ義肢系には、義手の開閉、理想的には回旋/上着および/または手首の伸展/屈曲も含まれるべきである。しかしながら、標準的な筋電装置の制御は、通常、2つの筋肉からの入力信号のみに依存している。これらは伝統的に、経上腕骨切断後の上腕二頭筋および三頭筋筋、および上腕骨切断後のラティシムス背部および胸部の主要筋肉である3。すべての人工関節を制御するために、切断者は(例えば、2つの筋肉の共収縮を使用して)活動的な関節を切り替える必要があります1。これは安定した制御パラダイムを提供するが、その結果生じる遅く直感的でない制御を伴う重大な制限が続き、これは2つ以上の人工関節4の同時運動を許さない。これは、補綴物の機能を制限し、肘の上切断後の高い補綴物放棄率の理由の1つである5。

TMRが義肢機能9を有意に改善できるという幅広い合意があるが、切り株内の複数の筋肉の選択的活性化および適切な制御は、特に術後早期に患者に課題をもたらす。切断後の多感覚フィードバックの減少と組み合わせた補綴制御のこの強化された複雑さは、外科的処置から完全に利益を得るために特定のリハビリテーションを必要とする。ここでは、最近の勧告10に基づいて、治療介入のための段階的なガイドラインが提供される。介入の概要と、理想的な設定でかかる推定時間は、 図 1 にあります。

記載されたリハビリテーションプロトコルは、ウィーン医科大学の臨床現場で実施され、その実現可能性および転帰は、最近発表された臨床試験で評価された9。報告された9として、30人の患者がTMR手術およびその後のリハビリテーションの実現可能性を評価するための試験に参加した。 図5 は、これら30人の患者のうち、11人が義肢装具 を介して 機能を改善する手段ではなく、疼痛治療としてTMRを受けたことを示している。当初義肢装具を目指していた残りの19人の患者のうち、5人は義肢の高額な費用(75,000〜150,000ユーロと推定される)、リハビリテーションのための不十分な時間、または義肢の重量が高いため、それに反対する決定を下した。ある患者では、術中探査により、世界的な上腕神経叢損傷が明らかになり、さらなる神経移動が不可能になった。この患者は、身体を動かす装置を使い続けました。補綴リハビリテーションを受けている残りの13人の患者のうち、10人はフォローアップ評価のために利用可能であった。

アウトカムは、サウサンプトンハンドアセスメント手順(SHAP)19、アクションリサーチアームテスト(ARAT)20,21、および服ピン再配置テスト(CPRT)6,26を使用して評価されました。これらの評価は、補綴物の機能を評価するために一般的に使用されるテストです。評価は、最終的な補綴物フィッティングの少なくとも6ヶ月後に行われた。さらに、患者は義肢装具の着用習慣について尋ねられた。
Salminger et al.9によって記載されているように、TMR手術後の10人の患者の評価は、40.5±8.1のSHAPスコア(健康な上肢のスコアが約100)およびARATスコア20.4±1.9(57が最大スコアであり、0が上肢機能なしを表す)を明らかにした(表1)。CPRTでは、患者は34.3秒から14.4秒以内にタスクを完了するこ±ができました。彼らは毎日義足を着用し、1日3〜10時間の着用時間を報告しました。
