再発流産(RM)は、妊娠24週前の24週前の2回以上の妊娠の喪失と定義することができる。この頻繁な生殖状態は、世界中のカップルの最大1%に影響を及ぼします2,3.病態生理は多因子的であり、子宮内膜および/または胎盤の発達に影響を与える母体駆動の原因(主に異常な胚核型による)および母体駆動の原因に分けることができる。この症状は、親の遺伝的異常、子宮異常、プロトロンボティック状態、内分泌因子、および免疫障害4に起因する可能性がある。
近年、免疫エフェクター細胞機能障害は妊娠初期の病因に関与している5.これは、月経周期、移植、妊娠初期の子宮内膜中の免疫細胞の特定の集団を、妊娠初期の特定の役割を持つ特定の集団を解明する多くの調査に影響を与えました。これらの免疫細胞の中でも、子宮ナチュラルキラー(uNK)細胞は、特に栄養芽球浸潤と血管新生6の過程において、胚移植および妊娠時に重要な役割を果たす。研究は、RM7,8を有する女性の子宮内膜におけるuNK細胞密度の増加を示しているが、この知見は流産のリスクの増加と関連していなかった9。しかし、この刺激研究は、RM10,11を有する女性の子宮内膜における他の免疫細胞型(マクロファージ、子宮樹状細胞など)の密度を評価する研究を刺激した。それにもかかわらず、RMを有する女性の包床注入子宮内膜における免疫細胞密度に有意な変化があるかどうかは不明である。

女性の2つのグループは、肥沃な制御女性(FC)と原因不明の再発流産(RM)を持つ女性を含むプリンス・オブ・ウェールズ病院から募集されました。肥沃なコントロールは、自発的流産の既往歴のない少なくとも1人の出生を持つ女性と定義され、RM女性は妊娠20週前に≥2回連続流産の既往歴を持つ女性と定義された。2つのグループの被験者は、(a)20歳から42歳までの年齢、(b)非喫煙者、(c)定期的な月経周期(25〜35日)および正常子宮構造、(d)子宮内膜生検の前に少なくとも3ヶ月間ホルモン療法を使用しない、(e)ヒステラによるヒドロサルピンスの使用なし。さらに、募集されたすべての被験者は、正常な核作法、正常な3次元超音波超音波ヒステロスピンゴグラム、10 IU/L<2日目の卵胞刺激ホルモン、30nmol/L>中黄体プロゲステロン、正常甲状腺機能、およびα性抗凝固剤および抗心リジンIgGおよびIgM抗体に対して陰性を試験した。
