血管内大動脈瘤修復(EVAR)は、開放大動脈瘤修復よりも優れた早期死亡率結果を示しています1。このアプローチは侵襲性が低いが、エンドリーク、移植片の移動、骨折2による中長期的な再介入率が高くなる可能性がある。したがって、より良いEVARサーベイランスは、良好な中長期的結果を達成するために不可欠です。
現在のガイドラインは、二重超音波および三相CTA3の日常的な使用を示唆している。動的で時間分解されたコンピュータ断層撮影血管造影(d-CTA)は、EVARサーベイランスに使用される比較的新しいモダリティです4。d-CTAの間、造影剤注入後の時間減衰曲線に沿って異なる時点で複数のスキャンが取得されるため、時間分解イメージングという用語が使用される。このアプローチは、従来のCTA5よりもEVAR後のエンドリークの特性評価において優れた精度を示しています。時間分解取得の利点は、選択された関心領域(ROI)6におけるハウンズフィールド単位の変化を定量的に分析できることです。
d-CTAでエンドリークを正確に特性評価することのさらなる利点は、スキャンを介入中の画像融合に使用でき、さらなる診断血管造影の必要性を最小限に抑える可能性があることです。画像融合は、以前に取得した画像をリアルタイムの透視画像に重ね合わせて血管内処置をガイドし、その後造影剤の消費と放射線被ばくを減らす方法です7,8。3DダイナミックCTAスキャンを用いたハイブリッド手術室(OR)での画像融合は、(1)3D-3D画像融合:3D d-CTAが術中に取得された非コントラストコーンビームCT画像と融合される2D-3D画像融合、(2)3D d-CTAが双平面(前後および横方向)の透視画像と融合される2D-3D画像融合。2D-3D画像融合アプローチは、3D-3D技術と比較して放射線を大幅に低減することが示されています9。

time-resolved, dynamic computed tomography angiography for characterization of aortic endoleaks and treatment guidance via 2d-3d fusion-imaging

エンドリークは、血管内大動脈瘤修復後も依然として大きな課題であり、エンドリークに給餌している血管の位置と種類に関して誤診される可能性があります。従来のCT画像化とは異なり、動的時間分解CT血管造影は、ヨウ素造影剤注入後の複数の時点における動脈瘤嚢を可視化する利点を有する。したがって、この技術は、EVAR後のエンドリークおよびエンドリークの発生源に寄与するすべての血管を特定するのに役立ち、血管が画像化された後にエンドリーク療法を導くために使用することができる。動的CTAイメージングでは、ヨウ素化造影剤の静脈内ボーラスの後に複数回のスキャンが必要です。放射線被ばくを緩和するために、画像化は、以前に移植されたステントグラフトの領域にのみ焦点を合わせるように調整される。研究によると、このモダリティは、エンドリークに対する従来の三葉CTAイメージングよりも診断精度が優れていることが示されています。診断用CTR MR画像を手続き内介入ガイダンスに利用することは、まだ日常的で標準的な治療ではありません。大動脈内漏出の診断および治療には、多くの場合、異なるC-Armアングラクションで取得した複数の2D血管造影画像が含まれる。このダイナミックCTイメージングでは、2D-3D画像融合などの画像融合技術を用いて、画像からの3D情報を術中の2D透視画像と組み合わせることができ、介入治療を容易にすることができ、これはしばしば介入処置中の放射線被ばくおよびコントラストの使用を制限する。実際のスキャンを開始する前に、ステントグラフト型におけるエンドリークの存在について、以前の画像化研究を確認してください。画像取得の場合は、患者をCTスキャナテーブルに仰臥位させてから、100錫錫フィルタを使用してトポグラムおよび非コントラストCT画像取得を実行して放射線被ばくを低減し、ダイナミックCTAスキャンの関心領域を選択します。次に、腹部大動脈内のステントグラフトの上方に関心領域を配置してコントラスト到達時間を確認するタイミングボーラスを行う。次いで、末梢静脈アクセスを通して10〜20ミリリットルのコントラストを注入し、続いて毎分3.5〜4ミリリットルの流量で50ミリリットルの生理食塩水注入を行い、タイミングボーラススキャンを取得する。次に、タイミングボーラスからのコントラスト到達時間と事前の撮像所見とに基づいてスキャンの分布と回数を計画する。キロボルト、スキャン範囲などのイメージングパラメータを最適化して、放射線被ばくを低減します。次に、動的CTA取得のために70〜80ミリリットルの造影剤を注入し、続いて末梢アクセスを介して100ミリリットルの生理食塩水注入を行う。前述のタイミングボーラスに基づく遅延時間を使用して、ダイナミックCTA画像取得を開始します。息止めは不要で、画像取得の所要時間は30〜40秒の範囲です。画像解析では、専用ソフトウェアの「体動補正を整列させる」メニュー項目を選択して、ダイナミックCTA画像間の呼吸運動アーチファクトを最小限に抑えます。次に、定性分析を実行します。大動脈の最大不透明化が起こったときのCT画像の軸方向スライスをチェックして、明らかな内漏を解釈する。次に、スキャンをマルチプレーナ再構成モードで分析します。エンドリークが疑われる場合は、エンドリークに焦点を合わせ、時間スケールを使用して時間分解画像を見て、エンドリークの発生源を推測します。定量分析を行うには、時間減衰曲線関数をクリックし、ステントグラフトの上の領域を選択し、TAC関数を使用して円を描きます。次に、エンドリーク領域を選択し、そこにも円を描きます。取得した時間減衰曲線を解析し、エンドリーク特性を求めます。大動脈ROI曲線におけるエンドリークのピーク値までの時間を差し引いて、ピーク値までのデルタ時間を得る。選択したダイナミックCTAスキャンをロードし、ハイブリッドORワークステーションでエンドリークの可視性を最大限に高めます。次に、腎動脈骨、内腸骨動脈骨、内漏れ腔、腰椎動脈、または下腸間膜動脈などの重要なランドマークに手動で注釈を付けます。ワークステーションで2Dから3Dへの画像融合を選択し、患者の前後および斜めの透視画像を照会する。これを行うには、手術台のジョイスティックでCアームを必要な角度に動かし、シーン取得ペダルを踏みます。自動画像登録を使用して、ステントグラフトを透視画像を含む3DダイナミックCTAスキャンのマーカーに電子的に位置合わせし、必要に応じて3D後処理ワークステーションで手動で絞り込みます。2Dから3Dへの画像融合を確認して受け入れ、ダイナミックCTAのマーカーをリアルタイムの2D透視画像にオーバーレイします。動的CTAイメージング後の定性的解析は、慢性閉塞性肺疾患および高血圧症を有する82歳の男性患者において持続的な1A型エンドリークを示した。さらに、定量的時間減衰曲線解析は、ROI大動脈のピーク値まで12.2秒の時間、ROIエンドリークのピーク値まで15.4秒の時間を示し、3.2秒のピーク値を作成しました。肥満、脳卒中、腎不全、高血圧、高脂血症、冠状動脈疾患の病歴を有する62歳の男性患者において、動的CTAは、両側L3腰椎動脈からの2型エンドリークを流入血管として嚢肥大を明らかにした。時間減衰曲線解析は、L3椎骨のレベルでROI大動脈のピーク値まで7.3秒、ROIエンドリークの24.6秒の時間を示し、ピーク値に17.3秒の時間を作り出した。適切なキロボルトの選択は、適切な画質を確保するために重要です。キロボルトが低すぎると、画像が最適でなくなります。スキャンのタイミングも重要です。これは、後で開始される集録によってタイミング誤差が発生し、定性分析に影響を与える可能性があるためです。根底にある臨床的問題に対処するために、CTであろうとMRIであろうと、イメージングプロトコルは個々の患者に合わせて実際にカスタマイズすることができます。多くの場合、それは以前の画像所見に基づいて行われません。大動脈内漏洩のこの特定のケースでは、タイプ1の内漏洩の疑いがある場合は、時間減衰曲線の初期の段階でより多くのスキャンを取得することが推奨される。例えば、患者が動脈瘤嚢の後半に頻繁に現れる2型エンドリークを有することが疑われる場合、時間減衰曲線の後期段階でより間隔をあけたスキャンを行うことが推奨される。以前の画像解析研究が利用できない場合、たとえば、インデックスフォローアップスキャンの場合、スキャンはタイミングボーラスからの時間減衰曲線に沿って均等に分配することができます。動的CT血管造影を使用して、複数の流入および流出血管を同定することができる。これは、エンドリーカーの理解を深め、治療をターゲットにするのに役立ちます。この技術は、大動脈内漏出を診断するための定量的アプローチを可能にする。ピークまでの時間は、タイプ1とタイプ2のエンドリークを区別するために使用できます。動的CT透視画像融合により、エンドリーク塞栓術、放射線被ばく、およびコントラスト体積消費のガイダンスを低減することができます。動的時間分解CTAは、エンドリークタイプおよび流入容器を適切に特徴付けることができる。これは、定性的な色素の到着時間と定量分析を組み合わせて使用すると、複雑なエンドリークの場合に特に有用であり、エンドリークタイプを区別することができます。私たちの経験では、動的CTイメージングは、エンドリーク治療中に追加の画像融合ガイダンスを提供することも示されています。このような動的時間分解CT画像化は、大動脈解離、末梢動脈疾患、動静脈奇形または壁内血腫などの他の動的疾患プロセスの将来の画像化においても有用であり得る。
