LTx는 또한 장기 이식 및 환자 결과 측면에서 다른 고형 장기 이식을 추적합니다2. 허혈성 재관류 손상(IRI)에 의해 가장 흔하게 발생하는 원발성 이식편 기능 장애(PGD)는 LTx 후 30일 사망률 및 이환율의 주요 원인을 나타내며 만성 이식편 기능 장애의 위험을 증가시킵니다 6,7. IRI를 줄이고 안전한 운송 시간을 연장하기 위한 노력은 환자 결과를 개선하는 데 가장 중요합니다.
ESLP(Ex situ lung perfusion)는 이러한 한계를 완화할 수 있는 가능성을 보여준 혁신적인 기술입니다. ESLP는 이식 전에 기증자 폐의 보존, 평가 및 재조정을 용이하게 합니다. 확장 기준 기증자(ECD) 폐 이식 후 만족스러운 장단기 결과를 나타내어 LTx에 적합한 기증자 폐 수의 증가에 기여했으며 일부 센터에서 장기 이용률이 20% 증가했습니다 8,9,10. LTx에 대한 현재의 임상 표준인 저온 정전기 보존(CSP)과 비교하여 ESLP는 몇 가지 이점을 제공합니다: 장기 보존 시간이 6시간으로 제한되지 않고, 이식 전에 장기 기능 평가가 가능하며, 지속적인 장기 관류로 인해 장기 기능을 최적화하는 관류액에 변형이 이루어질 수 있습니다11.
인간용으로 설계된 현재 ESLP 장치의 대다수는 양압 환기(PPV)를 사용합니다. 그러나 최근 문헌에 따르면 이 환기 전략은 음압 환기(NPV) ESLP보다 열등하며 PPV는 더 심각한 인공호흡기 유발 폐 손상을 유발합니다12,13,14,15. NPV-ESLP는 사람과 돼지의 폐 모두에서 전염증성 사이토카인 생성, 폐부종, 수포형성 등 다양한 생리학적 영역에서 양압 상피내외 폐 관류(PPV-ESLP)와 비교했을 때 우수한 장기 기능을 나타낸다15. NPV-ESLP에서 전체 폐 표면에 걸친 흉부내압의 균질한 분포는 이러한 이점의 기초가 되는 중요한 요인으로 제안되었습니다15,16. 전임상 효능 외에도 NPV-ESLP의 임상적 안전성과 타당성이 최근 임상 시험에서 입증되었습니다17. 새로운 NPV-ESLP 장치를 사용하여 12개의 확장 기준 기증자 인간 폐를 성공적으로 보존, 평가 한 후 100 % 30 일 및 1 년 생존으로 이식했습니다.
