多嚢胞性卵巣症候群(PCOS)は、生殖年齢の女性の12%〜20%で起こる一般的な障害である1,2。これは、多嚢胞性卵巣、不規則な月経および高アンドロゲン血症の臨床または実験室の証拠を含む可変的な形型をもたらす複雑な疾患であり、典型的には、女性が3つの基準のうちの2つを満たすと診断される3。PCOSの主な側面とその病因の重要な要因は、病気を持つ女性に見られる代謝変性です。PCOSを有する女性は、インスリン抵抗性、耐糖不耐症、肥満、およびメタボリックシンドロームの発生率が高い3,4,5,6。インスリン抵抗性は、疾患の現れであるだけでなく、卵巣における黄体形成ホルモンの作用を増強することによってその病態に寄与すると考えられ、それによってアンドロゲン産生の増加に繋がる7,8。インスリン抵抗性はいくつかの可能な起源を有すると考えられているが、研究は、それがインスリン受容体シグナル伝達の異常なパターンに起因する可能性を示唆しています 9,10.研究は、高インスリン法順性クランプ11、12、13、14、15の金標準技術を用いてPCOS患者におけるインスリン抵抗性を評価した。PCOSを持つ女性は、BMIに関係なく、コントロールと比較してインスリン抵抗性のレベルが高い。ブドウ糖産生に対するインスリン制御は、過剰なグルコース産生をもたらすインスリン抵抗性の障害において障害される。例えば、糖尿病患者は糖新生の割合を増加させ、グリコーゲン分解16の抑制を損なった。さらに、糖尿病ラット17ではグルコース産生の抑制障害が認められている。クランプ研究はインスリン抵抗性の測定を与えることができますが、PCOSでは断食状態と供給状態でのグルコース産生の直接測定に焦点を当てた研究はほとんどありません。これには、非放射性同位体グルコーストレーサーの注入と質量分析による測定の使用が必要です。

本研究では、同位体グルコース注入を用いたPCOSマウスにおける空腹時及びグルコースが豊富な状態における肝グルコース産生率を測定することを目的とした。これらの技術は、グルコース動態を含む幅広い実験に適用することができる。
